Cardboard boxes containing items for storage were put on a shelf below a thermostat controlled heater in a storage conex. The heater was not on when the boxes were placed, but turned on when the temperatures changed. The cardboard became hot and caught on fire. The fire was contained to the front of the conex and was extinguished upon the arrival of fire-fighting equipment.

The fall of ground occurred at approximately 12:30pm in the 3920 SFW travel way area. The ground fall was approximately 6' in width 5' in length and 3' in depth. The drift is approximately 15' wide by 15' tall. It was determined that some roof bolts in the area had corroded over time and gave way. There were no injuries, equipment damage, change in ventilation, or entrapment.
A fall of ground occurred at 4430-10R06 heading. There was an 8 foot round shot on 3-11-12 night shift. Ground support was not installed and fall condition was discovered by shot crete crew on 3-13-12. There were no injuries, equipment damage, change in ventilation, or blocked access as a result.
It was found that a water pipe attached to the side of the Meikle shaft had a missing clamp and loose bolts. Barrick took precautionary measures by shutting down the shaft.A 3rd party structural P.E. was used to determine if it was hazardous, it was confirmed that the pipe was secure and not hazardous. This form was filled out under advisement of the MSHA field office supervisor.
Employee was putting bolts in drill holes using a hand drill and impact gun. After the 3rd bolt he felt pain in his upper back. He was seen at the ER and diagnosed with a muscle strain. He subsequently was lost time.
EE was walking to the collar, slipped on snow covered ice and fell to the ground, pulling his abdomen and straining his back. Seen at the ER and diagnosed with an umbilical hernia. The hernia will be repaired and we will be filing an amended 7000-1 with updated days.
Injured ee and other ee had an altercation. Injured ee struck by other ee on side of head. Injured ee worked remainder of shift, woke up next morning with increased neck/head pain. Seen at ER and put off work.
Employee was energizing an airline, he turned on the wrong valve and that airline whipped off the hooks and hit him in the left lower leg causing a tib-fib fracture.
Ee was working and dust and water was blown into his face. Foreign body in left eye. Seen at ER, referred to ophthalmologist, put off work for one shift. Returned to work full duty 06/11/12.
Employee got pinched between a blasting mat and the forklift rack. Laceration to back of right thigh and possible crush injury.
While disconnecting a hose from the carbon truck, the hose came loose from fitting and threw the employee off balance. He fell to the ground, fracturing the right wrist. Will have surgery on 04/16/12 for repair. Currently working restricted.
Employee stepped out of loader onto a rock and twisted his right ankle. Heard a pop. Seen at the podiatrist, diagnosed with two avulsion fractures, placed off work.
Haul truck 622 caught fire going up the 2nd NW ramp. The truck was loaded at shovel 191 by the previous crew. This was the operator's first run of the shift. The operator evacuated the truck, receiving burns to his hands and face.
Employee attempted to move a crane block into position to attach to the rigging, inadvertently positioning his right arm between the crane block and the carbon screen. The employee fractured the ulna and radius of his right arm.
Employee was taping a hole in the bottom of a carbon bag. The bag became unbalanced and fell on the employee. He was diagnosed with fractured pelvis and fractured vertebra. Put off work.
While removing lug nuts from a haul truck wheel the employees thumb got caught between the lug nut tool and the wheel causing a crush injury.
Employee was lifting a crucible rack and placing it on a table. Strained back. Seen at ER, diagnosed with strain, released to restricted duty.
Emp was breaking bolt loose with a breaker bar and the bolt broke loose. The bar caught the right thumb and bent it backwards. Seen at the ER, diagnosed with a fracture to the right thumb. Cast at the orthopedist, released to restricted duty.
Ee was fixing a vent bag using a scissor deck. Using lever to raise scissor deck, something caught on the lever causing it to keep raising, thumb got pinched. Seen at the ER and wound closed with stitches. Released to restricted duty.
Employee stepped off ladder of carbon truck to cement step and felt pain in his right calf. Seen at ER, diagnosed with a strain and released to restricted duty.
As the contract employee and his co-worker were driving up the ramp transporting material they needed for a job, the driver noticed that some material had fallen off the equipment so the driver stopped and asked the contract employee if he could please get off and pick it up. The contract employee set his foot on top of a small rock causing his foot to roll.
Employee was lifting a transmission onto the head of the drill. Employee then felt soreness in his back. The drill was shut down and the employee and his co-worker went to the surface for further investigation.
Employee was reaching into the back seat of a service truck to put his hot gloves and meter down when he felt a pop in his left elbow. It started to ache, swell and burn and he was seen at the orthopedist the following day. Diagnosed with a strain and placed on restricted duty.
Employee was lining up connector on push arm with bar when the connection slipped, he lost his balance and fell back against a tool box. He was seen at the ER and released to restricted duty. Continued on restricted duty on follow-up at clinic.
Ee got off the bus at the welding shop and was walking to the truck shop to begin his shift when he slipped on the ice and fell, injuring his left knee. He was seen at the ER, referred to an orthopedist who placed him on light duty.
Lifted lid of hydraulic compartment, it came open and crushed left hand between lid and cab. No fractures, hand splinted and employee released to restricted duty.
At the start of the shift a haul truck operator was walking from the crew van to the equipment ready line. While walking, the operator's leg slipped forward resulting in a strain of the right knee. There were winter like conditions at the incident scene (snow/ice/frozen ground). The ready line area had been bladed the previous shift.
Employee was walking in the drift and tripped over a rock, falling to the ground. She slid on both wrists and left hand. X-rays were negative for fracture, given a splint for left wrist and released to restricted duty.
While securing tires on bed of transport truck with straps associate slipped on ice and injured left arm and hit head. Diagnosis on Nevada form C-4 is concussion due to fall, caused partially by acute renal failure...
Employee was measuring the needed length vertically of a 6 inch carbon steel pipe through floor grading to connect to an existing 6 inch carbon steel pipe which was supported by a 4X4 approximately 6 inches off the floor. The pipe slipped off the support and the end of the pipe caught the right hand between it and the grading lacerating the finger through the glove.
Employee was changing drive belts. While replacing the guard he caught his finger between two parts of the guard. Taken to NNRH. Stitches required to close the laceration. Released to full duty.
While changing teeth on a dozer, a piece of metal flew off, embedding in the employee's left forearm. Seen at the ER, x-ray and incision required to remove. No stitches, released to full duty.
When changing a broken cable, the cable snapped and caught the right middle finger. Seen at ER and treated for a fractured fingertip and laceration requiring stitches. Released to full duty.
While using hydraulic ram to press shaft, pin slipped and hit employee on safety glasses causing a laceration to the bridge of his nose. He received stitches to close the wound and was released to full duty.
Volt meter was placed on bucket controls of bucket truck and fell when controls were moved. Meter fell onto 3 pot bank and instantly arced and blew up causing lineman to duck down and take cover where he hit his lip on unknown object inside of bucket. He required 3 stitches to lower lip at hospital was released that day and returned to work next day.
Ee was walking from his tool box across the shop. Some cleaner was being sprayed onto a part and overspray went into ee's eyes. Seen at ER, diagnosed with a small corneal abrasion, and released to full duty. Prescription eye drops given.
The Employee was using a chop saw to cut a piece of I-beam, the Employee was wearing a Face Shield & Safety Glasses and the wind was blowing. The Employee lifted up the Face Shield and a piece of debris went into the Left Eye causing a Corneal abrasion.
Employee shut the compartment door to the greaser on a 26T haul truck and smashed his thumb in the process.
Ee was retracting a steering cylinder. Thumb was struck by a hammer. Laceration to right index finger required sutures. Ee released to full duty.
Hot welding slag fell from weld and burned through leather boot causing 2nd degree burns to the 3rd and 4th toe on his left foot
A lube technician hydraulically raised the cab of a Caterpillar D300 lube truck. While placing the cab support safety strut in position, the cab suddenly fell striking the employee's hand.
Employee was operating a cutting torch when blowback from the torch caused a piece of slag to enter his left ear. He was seen at Golden Health on 02/08 and referred to a specialist for confirmation of diagnosis. On 02/14 he was seen by Ear Nose and Throat Dr who confirmed a perforated eardrum.
The employee tried to move a piece of steel plate 30' by 8"" by 1/4 inch, the employee grabbed one corner of the plate lifted it approximately one foot and felt a pop on his lower back. The employee was taken to the hospital to get medical attention. X-rays showed no significant damage other than sore muscle.
