Haul truck KMT027 was hauling waste from the 3825 SFW to the 3765-1625. While the operator was backing up at the 3765-1625 to dump his load he smelled smoke, opened his door and noticed flames coming out from under truck. The operator exited the cab, notified dispatch of the fire. The mine was evacuated.

Ee was mounting a piece of equipment and the step broke. Ee fell jarring right leg and hip and twisting lower back. He was seen at ER and released to full duty. On follow up he was released to restricted duty and ultimately lost time on 12/09 due to surgery. Injury was determined work related on 12/28/15.
Employee was lifting a NKM screen. She noticed the screen was caught up and tried to maneuver it, the screen pinched her hand between a rail and the screen causing a fracture to her rt. little finger.
Ee was fixing shotcrete equipment, pressure released and employee was struck in face and chest by shotcrete. He was seen at the ER and diagnosed with chemical burns in both eyes. He was referred to an eye doctor for further treatment. Put off work.
Ee was using an air powered drill; it slipped and pinched his left hand between the tool and the tire. Ee sought treatment on 08/03 when he was diagnosed with a fracture to the metacarpal bone and released to restricted duty. The site is unable to accommodate the restrictions so the ee is put off work.
Aligning Membrane on boiler tubes, pad eye weld failed, pad eye struck employee in right knee cap.
Ee was sitting in haul truck being loaded. Large boulder fell onto tail-board, jolting the truck forward and the ee hyperextended his left elbow. Seen at ER and put off work, diagnosed with a soft tissue injury.
Ee was using a pry bar to adjust a piece of equipment. He felt a tearing sensation and pain in the right abdominal area. He was diagnosed with a torn muscle identified by CT scan and released to restricted duty. Further follow up on 02/23 resulted in the ee being put off work.
Material fell from under truck the ee was cleaning, striking the ee on the head and causing him to fall to the ground. He struck his head on the concrete. He was seen by the doctor, diagnosed with a minor concussion and put off work. Returned to work full duty on 01/23/15.
Ee stepped over bench in the dry, tripped and fell, catching self with left arm. Strained hand, wrist, elbow, shoulder. Ee seen at clinic, diagnosed with a chip fracture and possible soft tissue tear, referred to orthopedist and placed on restricted duty.
Ee troubleshooting leak on torque converter of mucker when A/C unit fell, pinning him. His coworker removed the unit and the ee was seen by a doctor for left arm, shoulder and upper torso pain. He was released to full duty. On 06/24/15, MRI results revealed a labrum tear and the ee was released to restricted duty pending surgery, making this a reportable injury.
Ee was fixing a leak in the paste plant and got shotcrete in his boots. He noticed that his feet and lower legs started burning. He was seen at the hospital and diagnosed with 1st and 2nd degree burns on both feet and lower legs. He was put off work and is attending physical therapy.
Ee was walking into phase 2 tunnel, slipped on ice, and fell. He hurt his left shoulder and knee. He sought treatment on 01/26 and was released to restricted duty. Diagnostics being done for a possible rotator cuff tear.
EE was walking in to MCC building when he slipped and fell, injuring his shoulder. Seen at ER, given a sling and referred to orthopedist. Returned to restricted duty.
Ee tripped going through lab door and caught herself when she fell. Seen at the ER and diagnosed with a fracture to the wrist. She was released to restricted duty.
Ee was closing hatch on a mixer truck when some shotcrete flew up into his eyes. He was seen at the ER, diagnosed with a corneal abrasion, and prescribed antibiotic medication. He was released to work with no driving for two days and then full duty.
Ee was filling drill with water using an air driven water pump. When the ee turned the pump on, a part of the pump broke free, striking the ee in the lower left leg. Seen at clinic, diagnosed with a large hematoma, and released to restricted duty.
EE was demolishing brick from wall of horizontal vessel using a rivet buster. Two bricks dislodged from wall sliding down radius of vessel hitting EE's foot and knocking him off balance. EE let go of rivet buster with left hand to regain balance. Rivet buster came down on EE left index finger. EE received 15 stitches and tetanus shot. EE returned to work on light duty.
Ee was removing intake pipe on a 930e Haul Truck. The pipe fell and pinched the ee's finger between the pipe and the frame. Seen at ER, diagnosed with a fracture and laceration to the tip of the left index finger. Released to restricted duty.
2 EEs were carrying a heater, load shifted and strained 1 EEs mid-back. WorkCare called, advised self-care. Incident was reported 1 shift after occurrence. 5.8.2015 Dr released EE w/care not to aggravate back due to findings of Xray & MRI as soft tissue injury. 5.11.2015 F/U appt EE was again release care not to aggravate back. F/U on 5.18.2015 EE was given restriction. F/U 6.1.15.
Employee was cutting ty-wrap holding coils of fiber together with utility knife and slipped cutting back of thumb. Employee had no lost time.
Ee was using a buffer on the inside of a tire and it came loose. Ee grabbed buffer and pulled back, cutting the right upper knee. Ee was seen at the ER and the laceration was closed with stitches. Ee released to full duty.
Ee was removing outer insulation on a high voltage cable when the cable slipped and the knife he was using struck his right forearm causing a laceration. Ee was seen at the ER where stitches were required to close the wound. Released to full duty.
EE loading guide support parts on deck, pinched finger between guide support piece and deck. Laceration to right ring finger, stitches needed to close wound. Released to full duty.
Employee looked up into a vessel that was being cleaned to check on personnel when he heard a noise that didn't sound right. When he looked up into the vessel he got sulfur dust in his eyes. He did have face shield and safety glasses on. Flushed eyes and went to clinic after shift for irritation. Diagnosed with conjunctivitis. Given an antibiotic and steroid drops.
On April 16, 2015, while employee was lifting a generator (with two other employees) he felt a pain in his abdomen. He continued to work without any complaints and declined medical treatment until May 13, 2015. His first medical appointment was on May 19, 2015 at which time he was diagnosed with a hernia that was surgically repaired on May 27, 2015.
After tripping out, locked bit in centralizer and spun bit off rod. Noticed that the pin end of the rod was braking off and went to check it. Didn't notice that rod was barely hanging onto the striker bar. Stuck finger under the rod to check and the rod fell off the striker bar and pinned the finger between the rod and the bit that was in the centralizer.
Ee was using a utility knife to cut open a box. The ee was not wearing gloves, and the knife slipped, cutting the ee's left thumb. He was seen by a doctor and stitches were required to close the small laceration. Ee was released to full duty.
EE complained of rash on both lower legs. Seen at the clinic and diagnosed with contact dermatitis. EE released to full duty, but cannot be in contact with or exposed to shotcrete.
Cleaning plugged discharge chutes on screens with shovel. Repetitive striking and stabbing over course of shifts caused soreness and swelling in rt forearm that got progressively worse. Seen by doctor on 07/18, diagnosed with tendonitis. Released to full duty but required to wear splint.
Ee notified on supervisor on 04/16/15 of wrist pain that had increased over time. EMG studies done on 04/27/15 positive for carpal tunnel in the left wrist, and the ee referred to an orthopedic surgeon. Appt with orthopedic surgeon set for 05/06/15. EE had carpal tunnel surgery on 5/7/15. He was unable to work following the surgery until 6/9/15.
Ee reported on 03/26/15 that he noticed a rash on his wrists and calves. He was seen by the dr on 03/27/15, who was unable to determine if the rash was related to work exposure. After investigation, the rash was determined compensable by workers' comp on 04/06/15.
Ee saw orthopedist for pain that has been increasing in right wrist and thumb. He was diagnosed with tenosynovitis and will be having surgery for the condition.
On Thursday, 28 May 2015, at about 3:19am Officer stated that she was not feeling well. As she was walking away from a vehicle she collapsed. Barrick EMTs were notified, she was then transported to a Reno hospital by helicopter. The reason for collapse is unknown at this time. MSHA was notified as a precaution.
