Contractor dry had significant damage to main structure and contests. No injuries were sustained in this incident. The fire originated to a fixed Wall heater mounted on the North West corner of the structure. These findings were determined by the Nevada State Fire Marshall.
Electricians were in 4330 north post splicing cable to a MCL back together when the power went off to fan. Electrician and supervisor walked through drifts to locate problem. They stopped by barricade for an Inactive area and saw muck from a fall of ground. The fall of ground affected ventilation to heading downstream, no one was working in heading.
Rodeo hoist is operated from the Meikle hoist control room. There was a loss of communication to operate the hoist safely. A new communication relay was installed, the hoist was then run several series of tests. The hoist was brought back up up on line and fuctional at 3:00 PM 06/01/2009.
Employee bent over to close drain hatch on spraymec and strained back. Seen at the ER, released to full duty. Upon follow-up on 11/27/2009, released to light work.
Employee was moving a shovel cable with his hot gloves when he received an electrical shock. He was seen at the ER, diagnosed with a calf muscle strain secondary to electrical shock and released to full duty. On follow-up, he was placed on restrictions to include the use of crutches. He went lost time on 11/20/2009.
Employee was climbing onto loader, slipped and caught himself with his right arm. He reported to his supervisor but did not seek treatment. On 11/24/2009 he was diagnosed with a rotator cuff tear resulting from this incident. He was released to full duty pending surgery for rotator cuff repair on 12/17/2009.
Employee was removing a piece of drill steel from a loader bucket. When he pulled the steel and it came loose, it struck the left side of his left leg below the knee. The employee was given a brace and released to work.
Employee was walking the 3880 laydown. He stepped wrong on uneven ground, rolled his left ankle, heard a pop and fell to the ground. He was seen at the ER, diagnosed with a severe strain and released to restricted duty, no weight bearing on the left ankle.
A loader operator removed the radiator cap of a 992-G loader he was operating, causing hot coolant to spray on his face, right arm and hand. The operator was wearing, approved hard hat, safety glasses, and cotton gloves at the time of the incident.
Employee was having his lunch break. He was finishing a can of oranges when the can slipped and he went to catch it and cut his right index finger, cutting the tendon. He will need surgery to repair and is scheduled for 10/22/09. Released and is able to work full duty until then.
Employee was driving haul truck off waste dump and saw other truck coming up ramp into his lane. The roads were wet from water trucks. Efforts to avoid collision failed. Employee was transported to NNRH ER, diagnosed with a neck strain and bruised shoulder and released to light duty.
A piece of flex pipe fell from a forklift and struck the employee on the left shoulder. The employee fell to the ground. He was diagnosed with a fractured clavicle and an abrasion to the right elbow. He was released to light duty.
Employee was sitting in haul truck being loaded when the shovel rocked the truck. The employee felt pain in his back that didn't go away. He was seen at the ER, diagnosed with a strain and released to restricted duty.
Employee was climbing down the ladder of a water truck when he felt a 'pop' in his left hand/wrist. Seen at ER where xrays were negative for fracture, the hand was splinted and he was placed on restricted duty until seen by an orthopedist. Was released to full duty on 08/04/09.
Employee was driving haul truck up ramp when the truck came to a complete stop, jarring the employee in the cab causing pain in neck, right shoulder, and arm. Employee was released to full duty at ER. Follow-up on 08/04/09 resulted in a restricted duty release.
Employee was chipping cement from truck 35. He overextended his arms while holding a tool and felt pain is his lower right back. He was seen at the ER, diagnosed with a lumbar strained and placed on light duty for one day.
Employee stepped off a tractor onto uneven ground and rolled her left ankle. She felt and heard an audible pop and had instant pain. She was seen at the ER and referred for follow-up. She was seen at a foot and ankle specialist, diagnosed with a fibular fracture, and released to restricted duty with a boot and crutches.
Employee stepped off the hot change bus, was not paying attention to foot placement, and stepped into a rut, twisting his right ankle. He was seen by a specialist on 03/04/09 who put him on restricted duty, sedentary work only.
Employee was replacing a cutting edge on a bobcat. He reached down to grab a bolt to steady it, when it slipped and he cut his finger on a piece of metal. He received 10 stitches and was released to light duty.
EE was disconnecting a fuel hose, when he pulled back and felt something pop in his left shoulder. He was seen at the ER and referred to a specialist, diagnosed with a shoulder strain and released to restricted duty.
EE was walking to the concrete pad at the ADR building, slipped on the ice and fell. He was seen at the ER, placed on restricted duty and referred to a specialist. The specialist continued restricted duty for 1 week.
Employee was operating a Milwaukee heavy duty drill; he was drilling a hole into a 2x4 for an anchor point. As the employee was drilling he let off of the trigger when he was about to drill through the 2x4 which resulted in bit binding.
Employee and coworker were moving bolts. The coworker picked up 4 bolts, one slipped and struck the employee in the upper right lip The cut required stitches to close. Employee was released to full duty.
While removing a tire/wheel assembly from utility tractor, EE's left middle finger was contacted between the wheel and hub on the tractor, resulting in a crushing injury and avulsion to the tip of the finger. First aid was provided and Injured was taken for medical attention. Sutured and splinted and returned to work on his next scheduled shift.
Employee was riding in a tractor underground on the 3720. He felt a drop of acid water hit his safety glasses and go into his left eye. He was seen at Elko Eye Center, given eye drops as a preventative measure, and released to full duty.

Employee was loading 12"" steel weighing 52 pounds on the bed of her truck and dropped it. It bounced and smashed her right thumb between the steel and bed of the truck. Laceration required 2 stitches. Released to full duty.
Employee experienced a standard threshold shift in hearing that was confirmed by an Ear, Nose, and Throat doctor on 09/28/2009.
Occupational noise exposure. Hearing loss less than 2 decibels since baseline. (Audiologist determination non occupational).
