Employee was using a large impact wrench and the weld on the bolt broke, driving the wrench downward, pinning the hand. The right middle and ring fingers were fractured and surgical repair was required.

Employee had thumb between two hydraulic hoses. He stepped away and his thumb was caught and it popped. He was seen at Golden Health and diagnosed with a strain and placed on restricted duty. Follow up on 10/01/08 resulted in a full duty release for 10/02/08, and employee missed one scheduled work day due to the doctor visit.
A hose/fitting became disconnected from a tank (acid delivery trailer)
EE was cleaning rock spill with skidsteer. Hit dump pocket with skidsteer stopping machine and bruising shin.
Employee had two trays in the sample feed prep and one tray got stuck. She was using her hand to get it loose and the tray moved, crushing her hand between the tray and the side wall. Surgery required, hardware placed in the right pinky area, stitches. The employee will lose time from work.
Employee was using a wrench to remove a u-joint from a lube truck. He exerted great force on the wrench and strained his right arm. He was seen at the ER, diagnosed with a strain and placed on restricted duty.
Employee was hammering on a pipe, standing on some grating. The vibration of the hammering caused the grating to move from under the employee, and it fell off the supporting beam. The employee fell and was caught by his harness. He was placed on restricted duty beginning 05/19/08.
Wet rocks in mikle picking area. Slipped & wrenched back resulting in a back sprain (diagnosed on 4-29-08). Off work unil at least May 5, 2008.
Employee was lifting a spare tire onto a flatbed truck and strained her back. The pain got progressively worse and she sought treatment at the ER on 03/08/08. She was diagnosed with a back strain and put off work.
EE was walking to workstation and tripped over a chunk of ice in the walkway. She fell to the ground landing on her wrists and knees.Diagnosed with contusion and strain, released to full duty.MRI testing discovered a fracture/free floating body in the knee, scheduled for surgery.MRI results reviewed 03/19/08.
Employee slipped on ice and fell, breaking her arm. She was seen at the ER, and taken in to surgery for repair of break. She was placed off work for a minimum of 2 weeks.
A Process Mill employee was assisting in disassembling a valve. His hand slipped off the body of the valve and was pinched between the body of the valve and the cage. His 3rd finger on his left hand was cut. He was seen at the ER and in order to minimize the chances of infection, was placed on restricted duty, unable to use the hand.
Employee was stocking bits on a drill. He went to put an old bit in a box and it started to fall. He tried to 'push' the bit into the box, and pinched his right hand between the bit and the box. He was seen at the ER and x-rays were positive for fracture and the nail was removed and the cut stitched. Employee released to restricted duty.
Employee caught his thumb between a chock block back and a holder bracket on a haul truck. He was given a tetanus shot and placed on restricted duty.
Employee was moving traffic signs and his back started to hurt. He was moving large signs that were cemented in to tires. He was seen at the ER and was diagnosed with a lumbar strain and put on restricted duty.
A mechanic received an injury to his finger while unloading a raditor. Raditor was being positoned near the front of the bulldozer by a service truck crane when the mechanic inadvertenly got his finger pinched between the raditior and the bulldozer push arm. Injured mechanic was taken to the hospital for medical treatment.
Employee stepped over some piping onto a liner and twisted his ankle. He was seen at the ER and directed to follow-up with a foot and ankle specialist. On 01/30/2008 he was placed on restricted duty.
Employee was dismounting his service truck after fueling a welder and slipped off the bumper, landing on his side on the vise mounted on the bumper. Initial xrays did not show a fracture, but subsequent xrays on 01/05/09 showed two cracked ribs. Employee is released to full duty.
Employee was rolling up an air hose when the hose got turned on by another hose hitting the valve. The employee was struck in the face by the hose. He was seen at the ER where 3 stitches were required to close the laceration on his lip, and was diagnosed with a fractured nose and tooth.
While employee was walking on uneven ground she turned her head to check the high wall and tripped
Employee was helping to install long support when a drop of acid water from the back fell into his left eye. He was seen at the ER and given preventative eye drops to use as needed and released to full duty.
Employee was walking when a drop of acid from the back fell into his right eye. The employee was seen at the ER where preventative eye drops were given to take as needed and the employee was released to full duty.
Employee was changing teeth on a loader bucket. He threw an old tooth into the loader bucket, and a piece of metal broke off, embedding in his left arm. He was seen at the ER where the metal was removed and the cut stitched. Released to full duty.
Employee pinched right index finger between reamer bit and hammer. Laceration to finger.
While lifting the right front wheel position on a Hitachi Haul truck to remove jackstand from under the front bumper, the injured employee was struck above the right eye by a 6""x5""x1.5"" steel spacer placed on top of the lifting device. Resulting injury required 14 stiches to close the wound, the injured employee was treated and released to full duty the same day.
Employee was carrying a floc sparge up stairs, slipped on the ice and fell to the grating and the sparge landed on his hand. Laceration to right ring finger closed with stitches. Released to full duty.
Employee made claim for carpal tunnel syndrome. After medical and factual investigation, claim was accepted for workers' compensation benefits. Employee began losing time after surgery on 03/06/09.
Employee made a claim for silicosis on 09/15/2008. After a medical and factual investigation, the claim was accepted for workers' compensation benefits on 03/06/2009 resulting in this report for a lost time.
