Accident type, without injuries

The main conveyance in the Rodeo shaft had malfunctioned, which caused the hoist to stop. There were 2 people on the conveyance when the hoist stopped. There were no injuries in this incident. It was later identified as a mechanical relay failure. The 2 miners were removed from the main conveyance using the emergency hoist and brought to surface at 05:31.

At approximately 3:15am on the 3765-1325 level in North Post. A 12'X 10' area of the back had fallen out passed the 12 foot swellex bolts of an advancing drift round. There were no injuries. The area was barricaded.

Crew was in muck cycle of shaft hoistman raised muck bucket off chairs hoist went down, incident is still under investigation cause is not known.

At approximately 6:15pm the Meikle MaryAnn tripped out due to an electrical issue. At the time of the incident 2 miners were on the MaryAnn. At ~7:30pm power was restored to the MaryAnn and the miners were safely hoisted to the collar.

The main conveyance in the Rodeo shaft had malfunctioned, which caused the hoist to stop. There were 12 people on the conveyance when the hoist stopped. There were no injuries in this incident. The problem was identified and fixed a short time later. The 12 employees were brought to surface at approximately 9:35 am that same day.

Struck by falling object

The employee was helping to move a large steel plate, one of the chains slipped and the plate fell on his right foot, partially amputating the right great toe. Surgery was required and the toe was amputated at the first knuckle. Employee placed off work. This will be 150 lost days for the amputation at the first knuckle of the great toe.

Ee was climbing a ladder onto a loader when he felt a pop in his right calf and had immediate pain. Seen at ER, referred to orthopedist who placed him on sedentary duty with a boot and crutches. Site unable to accommodate sedentary duty.

Ee was reeling up a hose reel and it came loose and fell on the employee's hand. Fractured 4th finger on right hand. Surgery required. Ee put off work.

Security Officer went to Entrance Lane 3 to retrieve paperwork from truck driver, as she stepped up on the small berm and reached for the paperwork she fell to knee. She stated she was fine, later she was sore and went to the Emergency Room. She only had a bruised left knee and hand.

Injured EE was clearing up the job site & was placing metal refuse into an open trailer when a piece of metal refuse dislodged from the refuse pile, came down & struck EE's left forearm above his glove. His left forearm was lacerated. First aid was administered at the job site & he was transported to the hospital. Prognosis after surgery regaining full use of arm.

Ee was pulling a hose off a reel for fueling, turned and felt a twinge in his back. He continued to work and as the day went on, felt more pain and reduced range of motion. He was seen by a chiropractor and has been off work receiving treatment.

Ee was driving haul truck underground, hit bump in the road, causing her hard hat to come off, hit the roof of the cab, and then hit her head. Laceration requiring stitches and put off work after diagnosis of concussion.

Over-exertion in wielding or throwing objects

While changing dozer ripper tooth, ee hit pin with 4 lb hammer and piece of metal embedded in left arm near elbow. Originally determined ok to not remove, but orthopedic surgeon recommended removal due to location of metal near joint. Surgery on 07/24/14. Ee full duty until that day.

Employee dismounted his drill and stepped onto a rock that rolled under his foot. He started falling, caught himself partially, but his left knee hit the ground. Seen at Golden Health and put off work pending an evaluation by an ortho.

While grinding off paint on a suspended load, employee step back on an extension cord cap, lost his balance and fell on his right knee.

While cleaning a hole, the lip of the bucket caught on the rib on the way down, striking a rock, jarring the employee in the cab. He felt pain in his upper and lower back. He was examined by a physician and placed off work pending follow up.

Employee was lifting LN 25 welding machine to employee inside tank. Lifting machine to shoulder height, employee felt something in neck, shoulder.

Struck by falling object

Ee was changing cutting edges and one fell onto the ee's right foot. He was seen by a doctor and diagnosed with three broken toes. Follow-up with foot specialist resulted in restricted duty. Ee put off work when restrictions could not be accommodated.

Ee was hosing down in the autoclave, she slipped on the wet floor and fell, landing on right knee, left knee hyperextended. Ee felt pain in left knee. Referred to orthopedist and was put restricted duty and an MRI is ordered.

Truck driver was unhooking hose from outlet valve on storage tank in order to hook into inlet valve. While driver was pull hose off valve the position of the whip check caused the hose to suddenly lift up catching the driver off guard and hitting his mouth. The drivers front top two teeth were chipped from the impact.

While walking from the bus to the autoclave, ee slipped on mud, hyperextending his knee. His knee swelled and became painful. He was seen by a doctor and referred to an orthopedist who has put him off work pending an MRI.

Ee was climbing an access ladder when he bumped his head on an overhead pipe, jarring him and making him lose his grip on the ladder. He fell backwards onto the concrete. He did not lose consciousness. He was evaluated by a physician and diagnosed with a fractured rib.

Over-exertion in lifting objects

Ee was lifting a bundle of hydraulic hoses while another ee was packing a plate under the hoses. The weight shifted on the load and the ee twisted his back, feeling immediate pain. He was seen at the ER, diagnosed with a lumbar strain and referred to the clinic. He missed 2 days of work.

Struck against a moving object

Ee was watering the 4560 upper west griffin ramp. He turned at the top of the ramp and went back down. The ee was unable to maintain control of the equipment and the truck contacted the rib several times, coming to an abrupt stop causing injuries to the operator. Ee was seen at the ER and diagnosed with fractured ribs and a laceration to the back of his head. Ee put off work.

Officer was exiting the porta-pottie, behind the Main Gate Entrance at Barrick Goldstrike. She tripped, lost her balance and fell to the ground. Officer landed on both knees and hands, jamming her right arm. She finished her shift and once in town, taken to hospital for further exam. She fractured her right elbow.

Employee was in a JLG man lift. Cables inside the boom failed, causing the boom to retract suddenly and knocking the employee into the basket handrail and floor.

Ee was working with 6"" pipe and his thumb was caught between the pipe and boom. He was seen at the clinic, diagnosed with a closed fracture and a laceration that required stitches. He was released to restricted duty.

Ee was loosening a nut on a dual brake can when the wrench slipped and his hand slid down the handle, cutting his hand. He received stitches to close the small laceration. Released to restricted duty.

Ee was getting off the bus and stepped onto uneven ground, twisting his left ankle. He was evaluated by a physician and placed in a walking boot and released to full duty. Use of the walking boot has restricted his activity and he is working modified duty.

EE had exited the air mover and after he was completely on the ground he turned to his left to move toward the rear of the vehicle. As he turned he felt a pain in his lower back. EE was wearing the required footwear and was not carrying anything when the injury occurred. EE sustained a muscle strain to his lower back and has been released to work with restrictions

Ee was loading items on the cage and pinched hand between cage door and cage. He was seen at the clinic, sutures were required, and he was released to restricted duty.

Ee turned off ramp and the truck did not come out of the turn, striking the rib, stopping the truck. The ee hyperextended his thumb. He was evaluated by a doctor and diagnosed with a strain to the thumb, given a brace and released to restricted duty.

Struck by powered moving object

Ee walking to buggy to leave work area; powder truck backed into ee, striking his back. Ee sought treatment 11/15/14. Diagnosed w/back strain. Seen again at ER on 11/18, diagnosed w/non-occupational gallstones. Surgery did not relieve back pain, seen on 11/25, put on restricted duty for back pain due to 11/12/14 incident.

Ee was sitting in a gator and a powder truck backed into it. Ee felt pain in neck and upper back, notified supervisor but did not wish to see doctor. Pain didn't go away, saw doctor on 12/12/14. Diagnosed with a whiplash type injury and referred for chiropractic therapy. Released to full duty.

While rotating sheave, employee pinched his right thumb between the sheave and the belt.

Ee was washing off powder truck, turned valve the wrong way and lost control of the hose which struck him in the head. He received staples to close the wound. Released to full duty.

Employee stated that while cleaning the windows of a dozer, slipped on track and strained back, did not indicate injury. Employee laid off that day, company recvd. diagnosis of disc herniation on 11/17/14.

While swing a chain ball over mid rail, employee crushed his left thumb. Employee used first aid measures and did not seek treatment with medical provider until 7/21/2014.

Ee was pulling on a switch bar to close a switch. It stuck, and when he pulled it, it let go and he was struck in the upper lip causing a laceration. He was seen by a physician and the wound was sutured. Released to full duty.

Employee was manually moving material from storage area to the tank. When he was passing the support threw the man-way when he cut his right palm on the edge of the support.

Employee was re-installing the bit and reamer and was having difficulty. The operator repositioned the core barrel in an attempt to make it easier. The employee attaching the bit moved his hands causing the injury to his left pinky and ring fingers. They were pinched between the bit and the set casing.

While employee was leaving gold room, dust particles flew in his left eye.

The employee's leg was struck by the metal foot pedal of a hydro blast pump resulting in a fracture. The hydro blast pedal housing kicked back and the shift in pressure caused the pedal to pop up.

While employee was off loading a pipe truck at the pipe laydown yard, a 4X4 had slid and struck employee's finger while employee was holding onto a 2X2. The pinch point smashed the employee's left hand ring finger.

Absorption of radiations, caustics, toxic and noxious substances

Ee saw doctor on 10/17/14 and was diagnosed with sensitivity to shotcrete mist. He did not have an active contact dermatitis but there was evidence of previous contact dermatitis. The doctor directed him to not work in an area where there was shotcrete mist.

Employee sustained a standard threshold shift in hearing. Confirmed reportable noise induced hearing loss. Confirmation received from AuD on 09/30/2014. Hearing conservation retraining completed and dB blockers ordered.

Contact with heat

Driller was found at the 4140 level underground and unresponsive by the leadman.

Ee was feeling bad, and when the feeling got worse, he went to the EMTs and the site nurse who toned out emergency transport. Transported to medical facility. Awaiting surgery for a non work-related condition.

Employee was an electrical/instrumentation technician the Autoclave. A reportable shift in hearing was identified in annual testing and confirmed by a doctor of audiology on 01/13/14. Written notification was given to the employee and retraining on the HCP has been completed as of 01/23/14.
