Accident type, without injuries

While attempting to dump his load, a haul truck operator of a 100 class haul truck backed thru the safety berm causing the unit to settle in an upright position. As a result, the operator was unable to safely exit the truck and was removed with a man-basket. The berm was not of adequate height for this model/type unit.

Due to a wild fire starting on BLM property and burning onto Goldstrike property, the Meikle/Rodeo underground was evacuated as a precaution due to power bumps from electrical lines in the path of the fire. This was just precautionary in case the hoists were to go down from lack of power due to the oncoming fire.

Goldstrike Mine rescue crews started fighting a wild land fire that BLM had been fighting for several days previously. Goldstrike did not engage the fire on Barrick property until this time.

A brush fire was reported between the old Newmont Mill 4 area and the Barrick AA Taildam facility. The fire was in an open field area and impacted a non energized electrical pole line, as well as a company data cable. No injuries occurred as a result of the fire. Fire was contained and extinguished.

A fire occurred on the surface area of the rodeo mine in an unoccupied maintenance building. Damage was limited to this building. Determined to be an electrical short between the walls of the building. No injuries resulted.

Employee was raising a scissor deck and crushed his right thumb between the back and the hand guard on the scissor deck.

Flash burns (electric)

While testing a nip on a 1000 volt Dosco, arc flash occurred causing flash burns to the employees hands and minor burns to the face. He was seen at the ER, given silvadene and dressing and referred to another doc. He was seen in Reno and told to remain off work until 02/19/11, no physical therapy required.

Absorption of radiations, caustics, toxic and noxious substances

EE was sitting at the computer and felt burning on his left leg. He notified his co-worker. They called rescue one who then responded to the gate house. He then became nauseous. Rescue one arrived and they called appropriate Barrick staff who determined to transfer EE to the local hospital.

Fall to the walkway or working surface

IW stepped off bolter, planted left foot and turned. Left knee popped and knee gave out, causing IW to fall to ground. Seen at ER, released to restricted duty. Previous non work-related knee problems investigated, MRI revealed new ACL full tear, claim accepted by workers' comp on 07/12/11. Surgery scheduled.

Over-exertion in pulling or pushing objects

The employee was pulling on a pry bar throughout his shift.

EE was cutting plastic tubing with a razor knife, the blade cut through the tubing and contacted the back part of the ee's left thumb, cutting down to the tendon. The ee required surgery to repair the tendon.

Over-exertion in lifting objects

Employee was removing rear differential from equipment and the opposite end was hoisted up, forcing the end the employee was holding down, and the employee strained his back trying to hold his end of the differential. Seen at ER, released to restricted duty.

Struck against stationary object

After getting off equipment and starting to get ready, the helper was walking beside the equipment, stepped on a rock and twisted his right ankle.

Worker was placing a piece of casing into the back of a tractor. As he slid the casing into the back, it rotated and rolled out of his hands. His index and middle finger was smashed between th e casing and rack causing the index finger to be amputated just below the finger tip. The middle finger had a broken bone and laceration.

Employee was walking through a cut in the berm when she stepped on a rock with her left foot. She lost her balance, catching herself wiht her right leg and heard and felt her right knee pop. She was seen at the ER and referred to an orthopedist. She was seen by the orthopedist, given a brace and put on sedentary duty until 09/06/2011.

IW was changing a bit on a cubex. His lanyard hit the controls, moving the skid plate, pinching his finger. Fractured finger. Released to restricted duty.

Employee was tripping out approximately 150' of HQ rods when he momentarily lost control of a rod while unscrewing it. While attempting to regain control of the rod he reached with his right arm for the rod and then felt a sharp pain in his shoulder.

Employee stepped out of service truck, caught foot on grating and twisted his ankle. He was seen at the ER, given crutches and a boot and two days restricted duty.

Miner was in process of cleaning shotecrete sprayer; when he released the quick-vic, material under pressure struck miner in face. Miner sustained corneal burn to right eye.

Struck by rolling or sliding object

While unloading split tube tray stand from the pickup, the tray slid sideways and struck the employee in the face causing a puncture wound to the left side of the nose. Employee received 7 stitches to the side of the nose and 3 stitches to the upper lip.

Employee was cutting tie wrap with a pocket knife and cut his left wrist. Seen at ER where cut was closed with stitches. Released to full duty.

While opening the latch on piece of equipment, the employee cut the outside of his right hand. Received stitches and released to full duty.

Employee was pre tripping a semi truck. Climbed into the cab and felt a spasm in the back. Employee climbed out and stretched with some relief. Climbed back in cab and drove and spasm got worse.

IW opened shovel door to discard trash. He threw the trash and caught his left pinky finger on the bracket for the blinds, cutting his finger. Seven stitches required, released to full duty.

Absorption of radiations, caustics, toxic and noxious substances

IW unhooked a hose from a fire extinguisher. Dry chemical discharged, hitting him in the face and eyes. Diagnosed with burn to left eye, prescribed eye drops, released to full duty.

Struck by falling object

Employees were removing diamond plate covering to get into work area. While removing the final plate, employee lost control of plate and plate fell approximately twelve inches on top of his right foot, resulting in a broken metatarsal bone.

The EE was aligning a pipe next to an existing pipe that was set in a 45 deg angle. Existing pipe had a support towards the bottom but was being held at top by a victaulic clamp only. The EE pushed the bottom of the pipe at the support section to put the other pipe in place, as he did this the victaulic clamp failed causing the pipe to swing & hit the EE on the back of his head.
