Rodeo #2 side hoist has small kink in rope on top by bonnet. we reported to MSHA. Rodeo hoist is down at this time until repair to rope can be made.

Leadman inspected work area & discovered west rib of 4445 Ramp had sloughed in. Immediately barricaded access to the area & reported to supervisor. The affected area was approximately 50 ft in length by 4 to 5 ft in depth. Initial inspection found the ground support, coated swellex bolts had corroded. This area did not impede ventilation and/or foot traffic.
A faulty ammonia sensor alarmed causing an evacuation of our Underground operation and MSHA was notified as a courtesy. MSHA cited the Meikle Mine for not notifying them about a possibility of an unplanned gas inundation near the Betze Portal within 15 minutes.
Odor of smoke was smelled near 1225 PM bay. Location investigated (1225 MCC); Mine Emergency pronounced to dump stench, evacuate mine. Exhaust fans went down to due to power outage. MSHA notified of Incident via telephone and called once again to allow Barrick to restart the exhaust fans. MSHA agreed for Barrick to restart fans and monitor any gas levels.
Initial investigation indicates that a 2 ft diameter pipe that carries aggregate to the mine backfill plant broke loose and struck the hoist cage.

Employee was working on an emulsion truck. The air acuator was stuck and he hit it with a wrench, causing it to move and pinch his right glove and index finger. He was seen at the ER and referred to an orthopedist. A portion of the bone was removed at the ER, and then more in surgery. He was released to full duty at his next shift on 10/14/2010.
Employee tripped on metal platform stair on 5th floor of autoclave. Was taken to the ER, x-rays positive for left tibia plateau fracture requiring surgical repair.
Employee in hot change bus that hit berm. Diagnosed with concussion, right ankle sprain, abrasion, contusion. Put off work for 3 days.
Employee was thrown around in bus when hot change bus hit berm. She suffered a broken nose and whiplash injuries to the neck. She was put off work pending surgical repair to the nose and subsequent recovery period.
Employee in bus that hit berm. Diagnosed with left face contusion, neck strain, upper back strain, right shoulder bruise. Off work 3 days.

Employee thrown around when hot change bus hit berm. Cut to inside of lower lip and chin requiring stitches. Put off work pending follow-up on 09/13/10.
Employee in bus when bus hit a berm. Diagnosed with concussive head injury, forehead hematoma, and cervical and thoracic strain. Put off work pending follow up by personal physician. He was seen on Monday Sept 13 and kept off work pending follow up on Sept 16. On Sept 16 he was released to full duty and told to attend physical therapy.
Employee in hot change bus that hit berm. diagnosed with bruise to chest, neck and upper back strain. Put off work for three days.
Employee was engaged in sandblasting operation. Hose slipped and abrasive material struck and penetrated protective hood the employee was wearing. Abrasive material struck employee on right side of face and lacerated lower jaw.
Employee was servicing water truck at the end of shift. While holding up the engine cover and reaching for a container of oil, the heavy cover started to come down, pushing the employee off balance backwards. The employee jumped down awkwardly away from the engine cover, landing on his feet, from a height of approximately five feet. Fractured left ankle and right heel.
Shovel hit haul truck, jarring employee. Initially released to full duty after ER visit. Was working on 06/18/10 and unable to lift chock blocks. Returned to doctor and put off work. Lost time began 06/19/10. Currently attending physical therapy.
Emp haul truck getting loaded. Huge rock fell from shovel bucket, hit headache rack of truck, jostling emp in cab of truck. Emp was thrown up in seat, caught by seat belt, and slammed back down. Seen at ER, CT scan showed compression fracture of L1, referred to orthopedist who placed emp off work.
Pulled overshot from rod, unclipped overshot. His left hand was pulled suddenly to the left and his hand was caught between the overshot and the drill frame or the sheave wheel.
Employee was washing the windows on a 988G front end-loader. He stepped down to a lower level on the deck to enter the cab and twisted his left knee. The step was approximately seven inches.
Employee stepped off bolter, placed foot on uneven ground, turning left ankle. Xrays negative, diagnosed with strain, released to light duty with a walking boot.
Employee was lifting a part. When he put the part down he felt sharp pain in back.
Employee was checking CIL tanks and stepped down step from top of tank, twisting his right ankle. He was seen at the ER, xrays were negative for fracture, and the employee was given crutches and a boot and released to restricted duty. He returned to his next scheduled shift on restrictions.
The employee was dismounting a 773 water truck when he felt a pop in his shoulder. It was determined the employee had a pre-existing condition that caused the shoulder injury. The employee received medical treatment and was placed on restricted duty.
Employee was using a wrench on a jumbo. Wrench slipped, emp hit rt elbow on a steel beam with force. He was seen at the ER, xrays negative for fracture, released to full duty. Went back to dr. on 06/07/2010, prescribed physical therapy and put on restricted duty effective 06/08/2010.
Employee was holding a 5 point safety card in the same hand as his glasses. When he went to put his glasses on, he poked himself in the eye with his 5 point card. He was seen at the ER, diagnosed with a corneal abrasion and given prescription eye drops and released to sedentary duty for two days.
Emp was dumping load and the bed of the truck fell, jarring his neck. He was seen at the ER, diagnosed with a cervical strain and released to light duty for two shifts.
Employee was moving an auger and pinched his finger between the chute and the auger. He was seen at the ER where stitches were required to close the wound. Released to full duty.
While walking on the drill the employee slipped and fell, landing on his left buttock and straining his lower back, buttock area. He was evaluated at the clinic, diagnosed with a strain, released to full duty. He began physical therapy 01/03/2011 making this reportable for treatment.
Employee was checking fire extinguisher and didn't realize it was charged. It exploded, spraying dry chemical into his eyes. He was seen at the eye doctor and given antibiotic eye drops as a precautionary measure. This is reportable for the drops.
Employee was pulling metal banding from under a pallet and cut the palm of his right hand. 5 stitches were required to close the wound. Released to full duty.
While tightening a bolt, the socket broke and the employee's hand hit a sharp piece of metal. Taken to NNRH where he required stitches to close the wound. The employee was released to full duty.
Employee was needle scaling muck off a 3 yard Wagner loader when a clump of dirt fell on his face and got behind his safety glasses causing irritation to the right eye. He was seen at the ER, and given prescription eye drops for the eye. Released to full duty.
Employee was cutting a zip tie and cut his left middle finger with a razor blade. 4 stitches required to close the wound. Released to full duty with instructions to keep the wound clean and dry.
Standard threshold shift in hearing identified through hearing conservation program. Confirmed to be work related on 11/17/2010.
The employee was working in a hot area hanging/repairing a vent line. The employee appeared to get overheated and was sent to town for further observation.
