Tripped climbing onto bulk truck. Fell striking EE's right knee on ladder of truck resulting in laceration & stitches
Turned in chair to get up, upon standing felt burning in back of heel. Damage to achilles tendon.
IE CHANGING FILTER CLOTH-PULLING ON EMBEDDED ROPES OF FILTER CLOTH. SPRAIN TO SHOULDER.
Employee exited scaler to stretch legs and stepped onto a rock twisting ankle.

Supervisor was descending stairs and hung left heel on the last step causing supervisor to fall.
EE was shoveling upstairs in Plant 4 & felt a burning sensation in right shoulder. Requested to have a medical evaluation. Employee was taken to the ER, received X-rays, a prescription for an anti-inflammatory, and was medically cleared to return to work on EE's next scheduled shift with no restrictions. *EE received 2 days restricted duty on follow up visit.
Employee was exiting the porta-john at the crusher area and stepped on to a rock rolling ankle.
Employee was loading a Bulk Truck at Plant 4 Bulk, railside, and stepped on a piece of angle iron causing ee to twist and sprain ankle.
Employee was performing some demolition work at Plant 2. While using a reciprocating saw, the employees thumb slipped up and over the rest into the reciprocating action of the saw, smashing finger. Employee received 6 stitches at the ER and returned to work the following shift.
Employee was using a shovel to clean up work area and felt a pain in elbow.
Employee was descending the stairs coming out of plant 3 Operator's Station and lost footing and slipped. Reported the event to supervisor and refused medical treatment. Returned the following shift with swelling and bruising in lower back.
