EE WAS GETTING OFF ROOF INTO MAN BASKET AND UNHOOKED SAFETY HARNESS BEFORE HOOKING INTO MAN BASKET. EE WAS ON UNSTABLE ROOF SHEETING IN DANGER TAPE AREA. ROOF SHEETING GAVE WAY AND EE FELL TO GROUND, APPROX. 18'8"".

Fall from scaffolds, walkways, platforms

Employee was shoveling product off a catwalk area, stepped backwards and fell through an opening in the expanded metal flooring. Fell 11 feet to the ground below, spraining his right shoulder

EMPLOYEE WAS HOLDING A SMALL AIR LINE, TWISTED HIS UPPER BODY AND FELT PAIN IN HIS LOWER RIGHT BACK AND RIGHT LEG. HE HAD PREVIOUS BACK SURGERY IN 1993.

Over-exertion (Not Elsewhere Classified)

Employee was operating a Getman scaler. Developed back strain (spasms) during his shift. Requested medical treatment on 10/07 (next day).

Fall to the walkway or working surface

Employee stepped of concrete ramp at a height of 18 inches to ground below, feet slipped and he fell on his lower back and hip. Continued to work with no problems until requesting medical treatment on 7/21. Visited doctor on 7/23, was given work restrictions and released back to work.

Contact with hot objects or substances

Employee was draining oil from Raymond Mill #53 at Plant 5, hot oil burned his right index finger

Absorption of radiations, caustics, toxic and noxious substances

Employee developed an infection in both ear canals, aggrevated by wearing ear plugs. Employee will wear ear muffs in the future.

Fall onto or against objects

Employee was walking down steps at the crusher, slipped on bottom step, reached out for handrail, cut finger on metal door frame

Contact with hot objects or substances

Strong & gusty winds caused small piece of metal being cut to burn through pant leg & fell into boot, causing burn on right foot.

Employee was using a hammer and punch to remove a bolt. Hit punch, piece of bolt broke off and stuck into left hand. Bolt fragment was removed at local Emergency Room. Employee returned to work same day with no restrictions.

EMPLOYEE IS CLAIMING WORKERS COMP. DUE TO STRESS/NERVES SO BAD THAT SHE CAN NOT WORK & ANXIETY ATTACKS. UNKNOWN AS TO WHEN THIS PROBLEM BEGAN OR WHEN HER FIRST DR.'S VISIT WAS. SHE REPORTED THIS AS A WORKERS COMP. ON 10/13/04 TO THE SAFETY MANAGER.

Fall from machine

Supervisor were traveling thru the plant 4 parking lot area. they noticed adult male lying on the grount near a flat bed truck. he stated he had fall off his truck to the ground.he climbed his truck cab to sit in the air conditioning and thought he would feel better 30 min later request medical treatment have pain back. ambulance picked him up. he is not member of property.
