Unclassified, insufficient data

Employee never reported an accident or injury. He had stomach pain, went to personal doctor, doctor determined employee had a hernia and informed employee it happened at work. Accident date listed above is date of doctor visit. Currently working with work restrictions.

Caught in, under or between running or meshing objects

Employee removed an inspection plate cover just below the airlock feeder on the cyclone located on Silo #3. The airlock feeder was running; employee's right hand contacted the airlock rotor, cutting off tips of the index, middle and ring fingers. Employee was treated at local ER.

Struck against a moving object

Miner driving R-40 Euclid Haul truck into south portal of Mine #4, bed was still partially up, bed struck roof of mine which is approximately 100 feet inside portal opening, casuing damage to front of truck bed. Minor back strain to employee.

Climbing down off tanker truck, stepped on gravel, twisted ankle which led to twisting of lower back.

Struck by falling object

Employee was assisting with installation of a punch plate in a screen deck. While attempting to place the plate into position, the plate dropped pinching the ends of the two middle fingers on left hand. Employee was taken to local ER for treatment, no broken bones.

Employee was holding up winch on Plant 5 reject elevator, while another employee bolted it into place. Strained lower back.

Employee was getting off forklift and twisted left knee.

Employee was backing out of door on R25 haul truck, struck right elbow on door. Minor bruise.

Employee was bending down to enter man lift basket, felt pop in right knee. Reported injury and went to doctor on 3/8. Placed on restricted duty activities.

Over-exertion in lifting objects

Employee was removing a 50# bag off conveyor belt leading to palletizer located in plant 4. Felt pain in lower back area. Treated and released from local ER. Follow up with company doctor on 3/5/07, issued work restrictions.

Struck against stationary object

Employee was stepping down off truck at Plant 13 loading area, slipped in loose gravel and twisted right ankle.

Over-exertion in lifting objects

QC lab employee attempted to pick up 5 gallon bucket filled with lab samples, felt pull/pop in lower back. Has history of back injuries. Bucket weighed 47 lbs.

Caught in, under or between a moving and a stationary object

Employee was attempting to clear a rock hung up in the crusher feeder with a steel pry bar. The bar slipped and caught his left thumb between the bar and the edge of the crusher box. Cut to end of thumb and chip to bone in the last joint. Thumb was x-rayed, bandaged and placed in splint.

Struck against stationary object

Employee was attempting to open a window in the electrical shop. He pushed on the window frame and his left hand broke a pane which cut his left palm. Six stitches required for the cut.

Employee was loading a bulk truck, struck above left eye with unknown object. Cut to skin just above eye requiring 9 stitches.
