Caught in, under or between running or meshing objects

Utility operator was found non-responsive with EE's clothing entangled in the drill steel of the roof bolter.

Struck against a moving object

Employee sat the wagon down on the track, the motor revved up and took off uncontrollably and hit the car. Shuttle wagon maintenance repair, arrived on site and did an inspection/functions test and was unable to find anything wrong with the wagon.

Struck by flying object

One employee shot another employee. Investigation pending.

Employee was driving man lift across track 3 crossing; man lift bounced EE up causing EE to strike EE's leg on the man lift tray under the controls. Employee suffered a break to tibia, fibula and bone was jammed up under kneecap.

Miner was working in man lift when EE accidently switched the lift into high gear. The lift hit a large, hidden rock and jolted several times. The miner's lanyard shortened with each jolt and EE could not dislodge EE's foot from the dead man's switch. EE sprained EE's right ankle.

Struck against a moving object

773 haul truck went over the edge of a 20' high stockpile. The truck driver suffered a minor laceration to the head and was taken to the hospital where EE was administered 6 staples to close the wound.

Over-exertion in lifting objects

Employee was removing a boom pin from a scaler and felt discomfort in EE's left bicep.

Caught in, under or between a moving and a stationary object

Pinched finger between rope and handrail, pulling finger apart requiring 11 stitches in finger. Return to work next day on light duty at office.
