Over-exertion in wielding or throwing objects

Employee was using a 3.5 ft long steel rod to hammer on the side of the pelletized feed hopper in an attempt to free up the feed stored inside. While swinging the rod the employee felt a sting in his lower abdomen. Doctor determined he had a hernia which will be medically repaired.

Absorption of radiations, caustics, toxic and noxious substances

Employee was removing a piece of rock from the railroad track area between Plant 5 and Plant 13. Picked up rock, copperhead snake was underneath the rock, employee was bitten on the right thumb. Taken to hospital for treatment.

Struck against a moving object

Employee was operating mechanical scaler on 820 ft level. Attempting to remove loose rock, placed tip into rock, tip slipped out of the crack, causing boom to move forward, rocking the scaler cab back and forth. Sudden movement caused strain in back and shoulder area.

Employee was stepping off rock breaker track to ground, foot slipped which caused him to twist, pulling muscle in lower back

Employee was opening lid on customer tanker trailer, pulling up on cam on lid, felt pop in right wrist.

Fall from machine

Employee was climbing down ladder on rear of truck trailer, slipped on third step from bottom, fell three feet landing on platform on rear of trailer. Bruise to upper chest.

Employee was walking on ground near Mill #54, slipped on the ground, sprained left ankle

Struck by rolling or sliding object

Employee was standing between pillars R-31 and R-29 on the 600 ft level, talking with two other miners and propped left foot on top of rock lying on the mine floor. Rock rolled over onto right foot, bruising top of foot. Employee was wearing steel toed shoes at the time of the accident.

Employee was using a cordless drill to tighten a screw. Drill twisted in his hand , cutting his middle finger on his right hand against a fan housing. Taken to ER and received 7 stitches. No work restrictions.

Struck by flying object

Employee was leaning over to place a pallet on a bagging station and dust blew into his right eye. He was wearing safety glasses at the time. First aid was administered, but he still required eye washing at the local ER.

Contact with hot objects or substances

Employee was beginning to work on bearing on tube mill #3, relieved water pressure initially, started to relieve water pressure again, water line came loose and left forearm was burned by hot water.

Fall from machine

Over road truck driver picking up a load at plant fell off his flatbed truck while securing a tarp over the load. Not an Imerys employee or contractor. Reported incident to Geneva, NY field office and advised by Bill Korbel to submit accident report. Driver treated at emergency room for possible concussion and abrasions.
