Fall to the walkway or working surface

Employee was walking outside towards the breakroom building to clock out, slipped on the snow and ice, fell on right side injuring his side and ribs

Rock being crushed by Jaw entered drive pulley, fragmented rock, fragment hit employee on left hand.

Over-exertion (Not Elsewhere Classified)

Climbing down a four step ladder to access the top of a tanker truck, right foot slipped off bottom step, striking the back of his leg and twisting the right knee. Injury (strained right knee) was treated at local ER and company physician.

Employee was walking on catwalk near Dust collector 42 at Plant 4, twisted left ankle resulting in minor strain.

Over-exertion in lifting objects

Employee was attempting to lift a 50# bag off the reject line and strained his lower back. Treated at local clinic and placed on restricted duty.

Employee was atempting to move a bulkhead inside a railcar and strained lower calf muscle in left leg.

Employee was walking on catwalk behind off white silo, stepped down off end of catwalk to ground approx 6 inches below, twisted right knee. Treated at local ER, follow up with company doctor on 4/4. MRI being scheduled.

Fall down stairs

Employee took one step down stairs inside Plant 3, woke up on the stair landing, does not remember falling or how he got to landing. Had small contusion on left elbow. Was treated at local ER for elbow and tests to determine why he fainted.

Employee walking down steps at end of plant 2, stepped onto ground, twisted right ankle. Treated at local ER, mild strain.

Struck by... (Not Elsewhere Classified)

Two maintenance employees were attempting to tighten a pressure gage, both were using wrenches. One wrench slipped and struck the injured employee over the right eye, resulting in small cut. Injury was treated at local ER, 3 stitches were required.

Struck against stationary object

Employee was getting into bobcat and cut left elbow on bolt that was sticking through the cage frame. Minor cut, treated at local ER, 3 stitches required.

Fall from ladders

Descending ladder from screens to roof of pelletizing plant, stepped on door left in wrong place (partially in ice/snow). Lost balance, fell forward on both knees and chest, bruised right knee. Reported accident to management on March 19, 2008. Received medical treatment by doctor on 4/9/2008. Continued normal job duties.
