Employee was walking in area under silos through spillage when EE stepped off curb and felt lower back pain
Employee was attempting to remove ceiling tiles from a 8' ceiling in mining office. Employee did not have proper ladder for project and was working out of an office chair when the accident occurred. Employee twisted a knee and is awaiting an MRI on 02/13/2017. The x-ray's showed possible torn ligaments/tendons to be determined after MRI.
Employee was shoveling spillage when employee experienced pain in employees left shoulder.
A bulk loader fell from a tanker truck. Their harness arrested the fall and the swing fall carried them into the structural steel in the loading alley. No injury from the fall; Contusions to their legs from the impact of the swing fall.
Employee was operating a bagging machine when EE opened a machine guard without locking out the equipment and the employee was struck by automated arm that places bags on equipment to be filled with materials.
Employee was operating a Skid Steer Loader and noticed the machine temp gauge was running hot and steam coming from the radiator cap. The employee turned the machine off, exited the machine, and attempted to push the cap down. The cap came off and steam burned employee's right arm.
EMPLOYEE STEPPED DOWN FROM PLATFORM & TWISTED ANKLE.
Employee was getting into the Fuel Truck to exit the mine and employee's foot slipped on the step causing employee to fall backwards landing on a rock with employee's lower back
Employee was traveling from the 740 up the incline. EE stopped, and set the parking brake, to tell the Doosan Operator that the heading was ready for to scale, when the parking brake failed. The machine rolled back approx. 50 feet and hit the rib.
Employee was moving a 50lb bag from a waste bin to a waste pallet and felt a pain in forearm.
Employee was re-setting breaker in MCC room when arc blast occurred in electrical panel above area of switch EE was resetting.
Employee was working on a dryer discharge chute in the pelletize building. The employee's right hand middle finger came across some sharp metal on the chute area causing a laceration that required medical treatment and four stitches with prescription. The employee returned to work immediately with no lost time or restrictions.
Reportable hearing loss over years of exposure. This was not from a single event but has been gradual. No lost time nor restricted duty.
Recordable STS in Both Ears as per audiometric examination record.
