Front-End Loader fatality at Jeddo No 8 Preparation Plant (Jeddo Coal Company), PA, October 1, 2004

A fire occurred on the front end loader. The operator (victim) was dumping a bucket of refuse rock on a stockpile. A nearby truck driver noticed an oil leak on the loader and radioed the victim to shut down. The victim dumped the loaded bucket, backed up and drove the loader forward. The loader burst into flames. Fire was located between operator's cab and engine compartment.
