While remote mucking the 4400-304 open stope, employee was struck on the hard hat, back of left shoulder, back of left thigh, and the top of his right foot. Employee's right foot was fractured and he has been taken off work.
While EE was unloading quick lime pneumatically out of a trailer the product hose split spraying lime out. He was hit in the face with the lime. He was wearing safety glasses. Lime went under the glasses into his eye. The cornea of his eyes was scratched and he was required to use eye drops and keep his eyes covered for 48 hours to insure healing.
Worker was pulling samples in assay lab when she felt shoulder pop resulting in pain in shoulder and numbness in fingers. Because of additional doctors visits and evaluation this was not classified as reportable until 8/30/07.
Employee was changing track rollers on a back hoe. Employee's finger got caught in pinch point causing injury.
Employee was walking past a haul truck which had stopped in the drift. The haul truck driver thought the miner was clear & began to move forward. As he did the employee's right foot was run over by the back tire & when he pulled it out from under the tire he felt like his belt twisted him around causing the left femur to fracture.
Injured employee walked away from half empty coffee cup. After working for a short time employee returned to his coffee and drank a couple of ""gulps"". Employee noticed ""chemical"" taste in coffee and began to vomit. It was discovered that a co-worker had put a liquid chemical de-icer into the coffee causing a possible poisoning situation.
EMployee was picking up a barrel for pump when he felt a strain on his left shoulder. Employee was originally given full duty release, but shoulder has not gotten any better. He had surgery on 1/14/2008 and is currently off work.
Employee was washing a loader when the hose he was using got caught on the bucket, he gave the hose a yank and when it released he bumbed his elbow into the loader bucket. Originally he was given a full duty release, however the pain persisted and he was scheduled for surgery.
The employee states that while lifting a bag of mud out of a support vehicle at shoulder level, he felt a pain in his shoulder. He did not want to seek any medical attention at that time. He had scheduled days off and while on days off he called on 06/18/2007 and his shoulder still hurt so he went to get it looked at.
Employee states while lifting bag of mud, he experienced pain in lower abdominal area. Did not desire treatment at the time. Symptoms did not improve.
Employee slipped and fell landing on his left shoulder. He did not seek medical attention initially, but did eventually get treament and later had the injury repaired surgically.
Employee was working on a pickup truck when he experienced pain in his right shoulder. Employee did not seek medical attention at the time of the accident. Employee did have surgery to repair his injured shoulder on 02/07/08 making this a lost time accident.
Employee got his foot run over by a shotcrete shooter causing a fracture to a bone in his right foot.
While cutting foam product employee states the box cutter he was using ""slipped"" causing a laceration to rt. palm.
Employee states that while cleaning an auto splitter between loads the lid of the splitter fell onto her left hand/wrist causing a contusion.
Employee states he was pulling filter cloths and felt pain in his lower back/leg.
The employee slipped on ice, aggravating a non- occupational left shoulder injury. Her next scheduled work day after the accident was 2/18/07.
IW was scraping bed of truck when a chunk of Drycon fell from top of the bed striking her in the elbow.
Employee was walking along leach pad, stepped on a piece of ice and fell into a hole tearing the soft tissue in her left calf.
Employee slipped and fell into the rib, injuring his left shoulder and lower back. He was seen by a physican and placed on restricted duty on 12/25/2007.
While removing a bearing the puller failed causing part of tool to strike hand.
Worker scraped finger on rough surface while working inside of process tank, causing infection.
Employee was removing scale chunks out of sump. He felt a sharp pain when he tossed scale outdoors with a shovel. Post accident taken 8-8-07 results negative.

Employee was removing the nuts on a wheel group on a loader when he he pulled a muscle in his back. Injury required surgery at a later date making the incident reportable.
While lifting a garbage can with the help of another electrician, into a large trash barrel, employee felt a pain in his right shoulder. He had surgery on 05/08/07 to repair shoulder and was taken off work.
The employee slipped in the mud while walking toward a piece of mobile equipment, the location was 4150 E. The employee had a follow up doctors visit on 1/17/2007, at that time per part 50 reporting this became a medical reportable.
Employee was walking on a pond liner when he slipped and fell. He was carrying a lifting device which landed on his right hand when he fell fracturing his right ring and pinky fingers.
Helper was running an impact wrench to tighten a dog collar on pipe. Driller attemped to flip a toggle switch to turn on air compressor, but hit the rotation lever by mistake. The pipe rotated and pinched helper's right ring finger between impact wrench and wrench stop on the drill table. Fracture and laceration to tip of finger below the last knuckle.
Employee was sharpening a chain on a chain saw using a hand file with his left hand. He pushed the file and it caught, the saw twisted, fell off of the table and pivoted the blade upward catching the employee on the wrist causing a laceration.
Employee was performing maintenance on the circle driver of a grader. He was using an impact wrench to remove the lower shaft support from the main housing. He was removing bolts when a piece of the cast housing broke off and cut employee on the top of his left hand behind the index finger. The laceration required sutures and surgery.
Employee was stepping down from his truck when he slipped on the step twisting his right knee. He sought medical attention and was released full duty with a knee brace.
Employee states he tried to free up a drill spindle with a wrench and as soon as he tried, the rod dropped due to slack in the mainline smashing his finger between the pipe wrench and the foot clamp handle.
Employee felt his knee pop while trying to position a Duff Norton Jack under a loader.
While conducting repairs on a lube truck, the employee stepped off the lube truck onto a ladder and lost his balance falling approximately 3 feet to the floor. Caused a laceration above his eye and bruising with swelling to his knee.
Employee was bent over starting a pump when an LHD entered the area. The operator did not see the injured employee and struck him with the mucker bucket. The employee did not suffer any MSHA reportable injuries but was placed on restricted duty.
Employee was doing maintenance on a pump when he got his finger caught between sheave and drive belt. This resulted in an open fracture requiring sutures.
An air hose on an air gun was pulled tight when JLG that air gun was on moved. Employee tried to knock the air hose loose when the air gun attached to the air hose moved catching his finger in a pinchpoint.
While changing foam filled tire on UG Jumbo, injured was attempting to push bottom of tire in to align lugs for installation, back began to tighten and radiate pain down leg.
While tightening a fitting above the front differential of a loader, employee slipped catching his right arm between the front driveline and some hoses, hyperextending his right elbow and straining his right shoulder.
While stepping down from bolter, employee stepped on a rock causing his left ankle to twist. Employee is currently on restricted duty.
While attempting to slide the liner to attach a lifting stap on the liner, employee felt his lower back pop. He was taken to a medical facility and placed on lift duty.
The employee was using a pry bar to remove a rock from in front of the wheel on a pallet jack. The load shifted on the pallet jack, which put pressure on the bar and smashed the employee's left index finger.
While pushing on the drifter of a rock bolter during repair his knee failed.
Employee was working on a derrick screen boot and standing on some grating next to screen. The narrow grating buckled causing the employee to loose his balance and twist his body which strained his back.
While replacing piping, employee stepped off of ladder contacting end of pipe that was just removed, puncturing foot.
Compacting fill material around pipe culverts with ""jumping jack"" type compactor. Compactor struck foot behind steel toe and fractured left fifth toe. Occurred on 8/3/07, denied medical treatment until 8/15/07.
Employee was climbing a ladder when he strained his right shoulder.
Employee was opening radiator cap on equipment to check coolant level. Coolant sprayed out from under the cap and burned his fingers.
Employee was leaning against hand rail to disconnect an air hose to a chicago fitting. Employee heard a pop while applying pressure on the hose fitting and later felt pain in rib area on chest.
The employee hit a high pressure water line with a mucker and broke a valve off. He got off the loader to shut off the water and the line whipped around and struck him in the lower legs. His left calf was bruised and his right ankle sprained.
Employee was using a sledge hammer to remove a concrete pedestal. During this process he struck his foot with sledge hammer.
Employee had completed drilling out his round when he got off the jumbo to clean out muck from his lifters. While in the process of doing this a rock fell out of the face and struck him on the top of his right foot. The x-ray was negative for a fracture but he had contusions that placed him on resticted duty.
Employee stepped on a rock while picking up a stake on the drill pattern causing his ankle to turn to the side.
Employee was mounting a motor onto a sump pump frame when it slipped and landed on the pinky finger of his left hand.
Employee was hanging up the chock block on his haul truck and missed the hook on the bumper. The chock block fell and pulled his arm down and strain/sprained his elbow.
Re-assembling hoses on shotcrete spray truck. While pushing against the hose his knee buckled.
Employee was loading a barrel of oil onto a lube truck with a co-worker who was operating a forklift. While using the forklift to lift the drum with a barrel clamp, the employee's finger got caught between the clamp and the barrel lid. He was taken to a medical facility for evaluation.
Employee was getting out of his haul truck at the UG batch plant when he stepped on a rock and rolled his right ankle. This resulted in a sprain. He originally went to medical attention on 4/2/07 but then returned to the Dr. on 4/4/07 and was placed on restricted duty.
Employee was stepping up into his underground haul truck when his foot slipped off of the step. This resulted in a muscle strain of the upper left leg/hip. He was taken off work on 03/23/07
Employee was drilling bolt holes in a flange with a drill press. The drill bit caught in the flange causing the flange to spin. The employee's hand got caught in between the flange and the height adjustment pipe. Employee was taken to BM General hospital for evaluation.
While climbing off of the top head after doing maintenance, helper slipped and fell four feet to the ground. He instintively extended his arm to break the fall and fractured his right wrist
Employee strained his left shoulder while putting hoses onto a loader. The injury required surgery in January 2008 and became a restricted duty case on 1/19/08.
Moving a piece of 8"" channel 12' long that was in a travelway. One EE dropped his end causing the other EE to drop his end on his left foot.
Employee was lifting a haul truck chock block when she strained her back.
Driller was making a connection, and unscrewed top-head from drill pipe. Helper put his hand over pipe to keep cuttings from falling down the air side. Top-head bounced down, pinching his finger between top-head and drill pipe. Fracture and laceration to left index finger.
Employee was checking a hydraulic fitting on a jumbo when the electrical plug shorted out and made a ""pop"". This startled him and when he rose up he hit his head on the boom of the machine. The impact resulted in a neck strain with 2 days restricted duty.
EE was participating in Mine Rescue Team practice when his neck and shoulder area started to hurt.
In the course of repairing a launder a Kambalda screen wiper blade struck the employee's foot.
Employee was trying to get into the back of a mantrip when slipped and fell on ice landing on his tailbone. He sought medical attention and was released to full duty. Sought medical attention again and received a second full duty release. Then On 2/9/07 he went back to the doctor and received restricted duty.
The employee just finish filling the hydraulic tank on a 6 yard loader to take to the shop for a hose repair, the driver of the loader started the loader up and drove backward not knowing the mechanic foot was between the bucket and the rib. the loader had been articulated inward toward the rib. the mechanic foot was bruised.
Employee was fueling equipment when she slipped on some ice and fell bruising her right knee and hip. She was given restricted duty for 3 days.
While operating the D6 wide-pad dozer on the dump, employee backed over a rock, shaking the cab, resulting in pain to his lower groin area. Employee thought that he may have a hernia and sought medical attention on 1-9-07. No evidence of a hernia was found but the employee was placed on restricted duty.
Employee felt a pop in his knee while descending down a ladder.
While using hammer worker struck his finger causing injury.
The employee was using a grinder to cut a handrail. The grinder stuck and kicked back. The employee lost control of the grinder and it spun back, striking the left thumb.
While removing a pilot hose under the cab of shovel, the wrench that he was using slipped off the hose fitting of the hose he was changing allowing his index finger on his right hand to come in contact with a bolt head cutting his knuckle. Employee was taken to a medical facility and received stitches. He was released to full duty.
Employee was struck on the hard hat and knocked to the ground by a rock which peeled from the face while loading a round in the 4080. Employee was released full duty, but then had a follow up with a Chiroprator.
After cooling the truck with a water hose, he threw the hose over the engine compartment and the end hit the radiator cap causing it to loosen and spray hot cooling fluid hitting him in the chest and shoulder.
While using hand operated winch, handle struck worker in face.
While loading steel into jackleg, employee's thumb was got in a pinch point when the nipper closed the chucktender, pinching his thumb between the acorn nut and the head of the machine. Employee was treated at medical facility and released full duty.

Employee was working on Track Drill when a piece of debris fell into his eye. Debris became inbedded in his eye and had to be removed.
Employee was changing a bit when the bit slipped smashing his right index finger resulting in lacerations requiring sutures.

Mechanic was using a pocket knife to remove a zip tie from a mirror bracket. While kneeling down the mechanic pulled the knife towards himself and the knife slipped causing a small laceration to the right thigh. Employee was taken to a medical facility, recieved sutures, and was released to full duty.
While tramming L.H.D. down main decline employee felt something in his eye.
Employee was climbing down a ladder when his arm caught on a sharp piece of metal on a pipe support bracket. The laceration required sutures.
While cutting hose with hacksaw the blade hit finger.
Employee was working on a pump in shotctrete, filled water with a hole in his boot. He did not immediately rinse the area and several hours later received a burn to his ankle.
Employee was using a bar to break down a tire. The bar slipped off of the lock ring and sprung back hitting the employee in the mouth.
Employee was breaking a band on a bundle of rock bolts in preparation to begin bolting, when the band struck his eye. He was given a prescription for treatment of the eye.
While employee was on a break, employee was cutting an apple with a pocket knife. The knife slipped cutting the palm of his left hand. This action resulted in sutures.
Employee was doing maintenance in the ball mill, when a ball exploded and a piece of the shattered ball struck him behind the right ear, requiring sutures.
While preparing to bolt, employee leaned a wire panel on the rib and as he turned to get another panel the panel leaning on the rib fell striking his face causing a laceration which required sutures.
He has been complaining that his wrist hurts. He fell while pulling liner out of ditch.-while walking tripped and fell.
Employee was looking up for phone cable when a drop of liquid fell into his right eye.
The employee was walking by a scrubber return tank. The tank overflowed with hot liquid and some of it fell onto the employee who was walking by.
Employee was washing off his boots with a high pressure washer and accidently washed above the boot line causing a second degree burn on the inside calf of his left leg. He did not seek medical attention until 06/6/07. He was treated for infection of the burn.
Employee was attempting to make a hole in a clipboard larger with a pocket knife. The knife closed on his right thumb causing a laceration.
Employee was cutting a tie strap with a folding pocket knife. The knife slipped and folded with his finger inside the blade causing a laceration which required 8 sutures to close.
Employee picked up the end of a pallet to move it out of the way and did not see a nail in the bottom. When the nail came in contact with his finger it resulted in a laceration which required 5 sutures to close.
Employee was in the process of shifting the forks on a forklift when the fork slipped out of his left hand. When it slipped it caught his right pinky finger between the back of the fork and the mast frame of the forklift. The injury was defined as a de-glovinig of the tip of the right pinky finger. The injury resulted in a partial amputation of the tip of the finger.
Employee passed out due to dehydration and fell down causing laceration to chin. Received 8 sutures to close the laceration.
Dismounting tire from wheel, Tire Iron slipped from wheel and struck injured on tooth. Chipping upper tooth.
After being terminated on 6/20/07 the employee said he was moving rock bolting wire and sprained thumb on 5/1/07.
The employee was cleaning out a cooling fan when dust particles got into both of his eyes. He recieved a prescription medication for his eyes.
IW alleges drivers side door wasn't working so she climbed over the radio console and caught her back. She twisted her back and neck.
Employee was unloading some old screen panels from the back of a pick-up. He slipped and fell on the screen panels and cut his left forearm. He received 4 sutures to close the wound, making this reportable.
In reviewing our records we found employee was given a prescription which makes this reportable. While hanging overhead cables, employee got something in his left eye. He was taken to a medical facility and received prescription medication.
While using a drill press employee got his finger caught between the chain that holds the chuck key and the rotating chuck. This dislocated his finger. He was given a splint to wear on his right middle finger.
Employee was servicing a 5 LB fire extinguisher. He set it down near the edge of his work bench and the extinguisher started to fall. As he reached over to catch it there was a metal tag that was bent out on the side of the extinguisher. It cut his wrist which required 4 sutures to close the laceration.
On March 23rd employee was using a magnetic drill and was going to move it to a new location. While lifting it he pulled a muscle in the groin area. He reported it to his supervisor and after it would not heal he went to the Dr. who diagnosed it as a hernia and did surgery on April 15th at which time it became reportable.
The employee was using an air operated die grinder when it kicked back and struck his hand causing a laceration that required sutures.
Operating a scraper, hit a bump and popped his neck.
While rolling welding cable off an electrical reel a large piece of mud came off the top section of the reel striking the employee on the right side of the face and head. The impact of the mud knocked off the employee's hard hat and safety glasses and mud entered employee's right eye resulting in a minor abrasion.
Employee was bolting in the face of the 5494 level with a jack leg and was having to use a bolting dolly on a two foot steel to reach the back. While coming off a bolt the dolly came off the steel and fell about three feet and struck him above the safety glasses on the Eyebrow/forehead. Employee sustained a laceration that required six stitches.
The employee was attempting to replace the bolts in a Blader when he strained his right elbow. The employee did not report the incident until March 8, 2007.
Employee was changing a tail pulley. The slide bar dropped and pinched his finger. This took 10 sutures to close the wound. Making this a reportable injury.
While unspooling the last wrap of 2"" poly pipe off a spool, the end of the pipe sprang back and hit the employee on the left ring finger resulting in a fracture to the tip of the finger.

Injured employee and another employee were carying a steel I beam. In the prossess of setting it down there was a misscomunication. The other employee dropped his end early causing the end the injured was holding to fall out of his hand. The beam fell on top of his right foot fracturing his second metatarsal bone.
