A lightning strike 1/2 mile from Emigrant CIC plant initiated a small wild land fire. This location is within the POO boundary of the Emigrant mine site. As this fire was small, the location was quite inaccessible and before the MERT members that had been dispatched to Emigrant for an unrelated event could access the fire an air attack plane extinguished the fire.

While swinging hammer, employee felt a sharp pain in middle to lower back.
Ee climbing ladder on pipe handler and struck head on pipe. Seen at ER diagnosed with head injury, neck/lumbar strain. Released to full duty. On follow up on 07/15/19 ee placed on restricted duty making this reportable.
Employee was lifting materials out of service truck when employee felt a strain in lower back
A maintenance welder was lying down on side to weld a hose bracket on a 994 loader. The employee felt pain in low back when employee pulled self up after completing the job around 9:00am (March 24th). The employee reported the incident the following shift (March 25th). The employee was placed off work on March 29th after a clinic appointment.
IP reported on 3/20 that IP had been experiencing discomfort in lower back for the last several weeks while performing routine tasks in the lab. IP was seen by provider, prescribed physical therapy and given a list of restrictions. Restrictions will not be accommodated.
While walking to hot change van, Employee slipped on ice striking head on ground.
Employee was clearing a lime screw feeder to prepare it for a maintenance inspection. The employee identified the feeder was plugged and was attempting to clear the blockage. Dry lime burped out of the feeder (swirl box) while the employee was standing in close proximity. The employee sustained chemical burns to the face, neck, chest, abdomen and eyes.
Ee was sliding filter plates across a beam and pulled a muscle. Ultimately had surgery on 11/07/19 making this lost time.
IP felt pop on left side of lower back while raising handle to filter press.
Employee report pain in their back when the bed of the haul truck came down after dumping a load of rock to the crusher.
While employee was washing glass bottles, a bottle slipped out of their hand breaking and causing a laceration to their left wrist.
Employee struck the back of left hand on a truck door causing an acute displaced facture in left hand.
Employee reported shoulder pain 2/15/2019. Employee went to the doctor for initial evaluation on 2/18/2019. Upon full medical review (3/26/2019) the event was determined to be occupational thus making the event reportable. The employee went to 6 physical therapy appointments.
IP was in the process of closing the service cage at the 4450 station. In that process, IP's left ring finger came into contact between the handle and latch. The injury resulted in a laceration which required sutures to close. IP is currently on restrictions.
Employee was riding in the back of a tractor. The tractor hit a hole and the employee bounced up and back down striking the bottom of their foot on a hose that was on the floor. Their foot flexed up against the metatarsal boot causing a fracture.
Operator fell while dismounting equipment and fractured right ankle.
Employee was working overhead tightening bolts with impact gun. When moving to the next bolt the locking mechanism failed allowing the socket to detach from the impact gun. The socket then fell from the bolt striking the employee on the bottom lip.
The employee was removing scale from an autoclave. During the demolition, a piece of scale broke away and fell onto employee's forearm area.
While removing a liner in the mill with an overhead crane the liner swung free hitting the employee on the back of the head causing a laceration to the scalp.
EE was using an electric hand drill to drill a hole. The drill became stuck in the hole causing the drill to rotate in the EE hand. This caused a dislocation of EE right hand middle finger. The EE sought medical treatment (Physical Therapy) on 8/13/2019 making this a reportable injury
6/9/2019 temporary laborer was reaching over to lift an assay lab sample bag when they felt a pain in the back/shoulders. On 6/13/2019 they sought further medical treatment and was then placed on light duty by the physician
Employee placed hand on boom of liner handler. Handler retracted pulling employee's finger into pinch point resulting in a pinched finger
EE was in the process of transporting piping for leach operations when they caught their left 2nd finger between pipe and back of loader. Initial clinic visit revealed no fracture and EE returned to full duties. On a follow-up visit on 5/1/19 the PA prescribed 2 physical therapy sessions when EE reported finger felt stiff.
EE was working to replace a leaking gasket on a coolant line when residual pressure caused the line to separate resulting in some coolant entering the EE's eyes. EE requested a medical evaluation during which time was prescribed an antibiotic to prevent possible onset of infection.
Employee fell backward due to muddy conditions on the ground, and landed on left arm.
CAT 16m motor grader made contact with coach bus resulting in bus passenger sustaining neck strain. Company was notified on 03/19/2019 that the injured employee had attended physical therapy that was recommended by the Dr. on 2/27/19 making this reportable.
Ee was driving the haul truck and hit a deep pothole. EE struck head on the window of the haul truck. EE was transported to the ER with neck pain. Diagnosed with a cervical strain and put on restricted duty for one day.
IP was conducting a post blast inspection in the 4200-212. IP was standing on top of the muckpile checking for misfires. As IP was walking down the muckpile, the IP fell forward resulting in laceration to the left wrist. One suture was needed to close the wound.
IP reported experiencing right wrist pain. IP sought medical treatment and was put on restricted duty, and was diagnosed with Ulnar Carpal Instability.
The KK employee was removing refractory from an autoclave while on a work platform. As the employee stepped down from the platform onto the floor of the equipment, the employee mistepped and sprained an ankle.
