A 15 foot drop raise was drilled and shot. When it was blasted there was damage to the nose pillar, which in turn eroded into the back of the intersection beyond the anchorage of the ground support, which was noticed on 11/27/12, which was 3 days after the blasting of the drop raise. Upon noticing the failure it was reported to an MSHA inspector on site for a regular inspection.

Operator of (ULD59) was remote mucking a floor pull in the 4430-141. The mucker was 100 feet from the operator when the left rib began to work, trapping ULD59. MSHA 800 line was called. EE of MSHA placed a (k)order on potentially affected areas yet said we over reported. MSHA supervisor EE advised that a 7000-1 still needed to be filed.
At approximately 03:00am a fall of ground occurred near the face at the right spring line of the 4455-909 BC stope access which was being shotcreted in preparation for bolting. The boom of USM006 was covered in muck. At the same time, the left rib on the 4490-909 MC stope access subsided. The left rib was in the process of being bolted. No miners were injured.
During the weekly inspection of the secondary hoist a hydraulic valve, which supplies hydraulics for the raise and lower of the hoist became stuck rendering the hoist inoperable. The valve was repaired and the hoist was returned to operation.
B-32 blade cat 16 h caught on fire while it was parked on the readyline. This happened between shifts. Extensive fire damage to the cab and engine compartment. No personnel were injured in this event.
A fatal accident occurred at the Exodus underground mine on Friday August 31st at approximately 13:30. The employee was preparing the top cut of the 4410-315 long hole stope, when he fell into the stope.
While unloading the shaft liner plate from the carrier, the form that was tied off to a tugger raised unexpectedly and pinched the employees thumb between the liner plate and the cross beam on the Galloway.
Changing a tire on haul truck, employee lowered jack and caught his finger between tire and truck frame. He lost the tip of his middle finger of his left hand.
While shoveling Snow the employee reported back pain. The employee did not reported the injury to his supervisor until 12/27/12 he received medical treatment 12/28/12 in which it became a lost time.
See Attached Was assisting other EE's in grinding on end of a spool. Co-EE heard a popping noise and the end of the pipe he was working on shifted. When he looked to the IEE he saw EE's head pinned to the concrete by the end of the spool (approx 2,700 lbs.) Co-EE's used forklift to hoist the spool off of the EE. 911 called rescue team transported EE to hospital.
Employee was jostled in the cab of the equipment, straining his back. On 9-5-12, he was treated and returned full duty. On 9-17-2012, he sought further treatment and was classified as a lost time.
Employee was lifting a portable tool box when he felt a pain in his lower abdomen.
This 7000-1 is being filed to terminate citation #8692733. Haul truck operator reported feeling some pain in his lower abdomen after boarding his haul truck and that he had a similar pain the previous shift while exiting his haul truck. The employee had surgery to repair a right side inguinal hernia with obstruction on 7/12/2012.
The employee was setting the grizzly beams in ore bin #2. His knee was rested on the toe kick. He felt his knee cap shift and pop. This resulted in a torn meniscus and strep infection. The medical paperwork was received by Atkinson on 7-19-12. Atkinson questions whether this was a result of the incident at work.
Worker was fueling a CAT 777D Haul Truck The wiggins kicked off indicating the tank was full. The worker went to remove the fuel hose and was sprayed with diesel fuel. He was wearing full wrap safety glasses, but the fuel made contact with his eyes and clothing. The sup. flushed his eyes with saline solution.
While walking through the line out room the employee(custodian)went to go down the steps in which she missed the steps and landed on the ground and fractured her ankle.
Employee was sorting steel grinding balls when her hand felt like it was cramping. She let it relax but still could not pick up balls. She states her hand starting hurting, her fingers started to tingle, and her hand and wrist swelled up.
While walking on a steep grade of the descending ramp,the employee pulled a muscle in his left calf.
Employee was in the process of loading explosives and lifted a 50 lb bag of ANFO and felt something pop in his right lower back.
Employee exited the back of the administration building to board the bus to town. He slipped on a thin sheet of ice that had formed in a small puddle on the cement pad outside the building and fell on his right side injuring his right knee.
employee was walking on the deck of a drill and tripped, twisting his knee. employee sought further medical treatment several days later and was classified as restricted duty on 6-4-12.
Haul truck XXR-611 was descending the Reona pad loaded. XXR-611 breached the left side berm on the second 90 degree corner. The truck continued to moved across the flat and came to rest against the berm of another haul road.
Mechanic slipped while checking maintenance issue with haul truck engine. He caught himself with his left arm causing a strain to his shoulder. Employee didn't seek medical attention at time of injury but later requested to be seen by doctor and was diagnosed on 3-21-12 with a torn rotator cuff.
Employee was working on a barge at the tailings pond and a pipe fell on his left hand causing a contusion at the knuckles.
Employee assigned to the powder crew was in the process of preparing to load a round when his partner inadvertently lowered a stabilizer jack onto the employee's left foot, resulting in a hairline fracture of the second metatarsal.
Employee was dismounting from the back of the water truck and stepped on a small rock in the road and rolled his right ankle causing swelling and a bad sprain.
While stepping down off of the crane the employee missed a step and started to fall striking the tracks and then fell to the ground.
employee was taking the left track off on a D10N and had removed the master pad and bolt. he grabbed a hammer with his right hand and put his left hand on a track pad above the master link and when it came apart the track pad came together pinching his left index finger,causeing the employee to received 3 sutures
Employee was bolting with a jack leg when a rock came down from about 5' and struck him on the top of his left foot. At first was thought to be just a bruise and he was given a full release, but after the x-rays were examined by a radiologist 2 days later it was determined there was a minimally displaced fracture of the navicular and a restricted duty release was issued.
Employee was opening the station door when his right thumb was caught between the ring and the latch when the door was opened, causing a laceration and fracture just below the thumbnail.
While exiting truck hand was on door frame when door closed fracturing hand.
Exit truck stepped on rock twisted knee. Injury became reportable on 10/10/12 when he had surgery on knee.
Employee was stepping up to bolter cab when he felt a pop and experienced some pain in his right knee. Over the next several weeks pain did not improve. MRI revealed a small meniscus cartilidge tear. This was surgically repaired on 09/19/2012.
Employee was torque turning final drive mounting bolts on a 793D with a rad gun. Pulled trigger on gun and got finger pinched between reactionary arm and bolt.
While moving jackleg it fell pinching his thumb between machine and leg.
Employee arrived at Mine Site by Bus. Stepped down of the bus onto a rock and rolled his left ankle. Sent to hospital for X-rays. Employee has hairline Fracture.
Employee was fueling equipment and tripped over a 2"" fuel hose. He fell and caught himself with his right arm and heard a pop in the shoulder area. ER physician diagnosed with muscle strain and the arm was placed in a sling. He is to follow up with his physician the week of 6/4/12. He is on modified duty until he sees another physician.
Employee was repairing hard line when he lost his balance and contacted the rib with his left forearm and elbow.
When employee stepped on loader to check for hyd leak he twisted knee. He was placed on restricted duty on 5/9/12
The employee was cleaning the release handles on a CAT Uni-loader (Skidsteer) with the bucket hanging down and elevated approximately 1 foot off the ground. When the handles released, the bucket rotated forward and dropped off of the attachment plate bouncing off of his right foot and breaking several bones in the top of his right foot.
Employee was on the converter taking samples. He went to move to the side and his left leg went through a gap between the converter and the grating, and he hit his left side ribs (back area) on a ledge of the door. This resulted in fractured ribs.
Operator noticed smoke/flames coming from his haul truck. He activated the fire suppression system and attempted to leave the truck. There were some flames near as he exited the cab of the truck so he attempted to remove the fire extinguisher but changed his mind and exited the truck without the extinguisher. He got some burns on his arms and a small spot on the side of his face.
While loosing a pipe string, pipe wrench spun around striking right arm.
Employee says he hurt his back while shoveling on Saturday on rig LK-40, there was no specific incident his back just became sore throughout the day.
Employee was coming out of a Jumbo Drill when the employee's right knee gave out when he stepped on the ground.
While filling hydraulic oil tank, lid fell closed on employee's finger cutting and fracturing it.
Employee was attempting to unload Drisco pipe from the rack of a tractor. He had his rap wrench on the end of the pipe and the wrench slipped off the pipe and he lost his footing and tried to brace the fall with his left hand resulting in a slight fracture on the distal end of the radius.
Pulled on hose-reel at lub-bay and strained abdomen
Employee's hand was pinched between two panels while installing one of them. The employee broke & lacerated his left little finger when his hand was caught between one of the hanging forms and the grout plug on a form that was previously installed on the rib.
Employee was drilling blast holes when a rock came loose and bounced down the wall and hit employees foot at edge of metatarsal guard causing injury to left foot.
While jackleg drilling, air hose came loose and hit him in the face and eye.
Employee was pulling jack leg out of a hole and felt a pop in his left elbow.
EE and co-worker were lifting a reel of 3/4"" hose off of the hose rack. EE felt a pop in his back and when he rose up he felt a pain going down his right leg and up the right side of his back. EE was seen at Northeastern Nevada Regional Hospital in Elko Nevada.
Employee exited his truck and stepped on a rock rolling his right ankle, causing loss of balance. Employee fell to the ground with right arm outstretched. Sprained ankle and fracture to right scapula (shoulder blade).
Employee was accessing a haul truck when he slipped and fell to the ground spraining his left wrist.
Employee was taking an electrical cable out of a cab hook which attached to the rib of the drift. The cab hook spun resulting in a laceration between the index finger and the middle finger on his left hand. It required about 7 sutures to close.
Employee was cutting the insulation off of an electrical cable when the razor knife slipped causing a laceration of left hand between his thumb and index finger. The laceration required 8 sutures.
Employee attempted to open a sandwich with a razor knife resulting in a laceration on the left thumb requiring sutures to close.
Employee was unhooking a light plant off the pintle hitch of a pick up and in the process lifting the light plant the employee pinched the tip of the right thumb between the pintle hitch of the pick up and the hitch on the light plant. Resulting in a fracture and sutures on the tip of the bone on the right thumb.
Mechanics were doing an outer feed end liner change out. They started knocking out liners & lifter from the outside through the bolt hole. After the liners & lifters were unbolted they used the steel rod to knock out stuck liner. While knocking out bottom liner with steel rod the center liner came loose & fell on steel rod, bent it & rod struck employee on nose causing a laceration
While employee was tightening bolts on dozer heel plates the employee pinched his right thumb. The employee became dizzy and stumble in too a ladder striking his tooth on a step causing it to chip.
Employee pinched his right index finger between the haul truck bed and the centering bed bracket resulting in a laceration to the tip of the finger which required sutures to close.
Moving jack-stand caught finger on burr on jack-stand cut finger received sutures.
While EE was cutting round holes on the liner he slipped with his right hand cutting his left thumb.
Employee's hand was pinched in a hinge point on a scissor deck wing, causing a laceration to his right hand that required 6 sutures.
Removed clamp on air line without bleeding off pressure.
Employee was cleaning up an underground laydown when he pulled a band out from under a bundle of rock bolts. The end of the band struck his right forearm resulting in a laceration which required 7 (seven) sutures.
While manually handling a rock, employee pinched his right index finger between the rock and structural steel. Although the employee was wearing impact gloves, the contact resulted in open fracture of the tuft of distal phalynx, which required 8 sutures.
Technician was pounding in a wheel stud when head of hammer came off handle while swinging down allowing his hand to contact wheel assembly.
The Employee was swinging a hammer and the end of his Right little finger hit the edge of a metal flange, causing a laceration to the end of the little finger.
While enlarging a hole with a burr bit, the bit and grinder kicked out and struck the left forearm, causing a puncture wound.
While holding onto a cable tray in a man basket, the employee's hand was pinched between the basket and cable tray, causing a laceration to the left hand.
Employee was opening a can of pineapples when his right thumb came into contact with the edge of the lid causing a laceration. The laceration required sutures to close.
Employee was putting drill rods into the back of a tractor. While putting the drill rods in, the employee's right middle finger was caught between two rods pinching the finger.
Was moving railroad ties and cutting blades. About 45 minutes later employee was operating a backhoe and experienced extreme pain in his abdomen and upper leg.
Employee was at the 4450 station at the end of shift. There was an employee smoking a cigarette. Employee was standing next to this gentleman. When he turned and an ember from the employee's cigarette went into employee's left eye. Employee was wearing his safety glasses. He felt a burning for a few seconds.
Employee was positioning a porta power and pinched his fingers between the tooling and a haul truck frame resulting in lacerations to two fingers.
Employee was placing a drill bit in a bit basket when his hand came in contact with the casing causing a laceration on his finger.
Drill assistant put the breakout wrench on the table to adjust it. When he did this, the driller went out with the breakout wrench and pinched the driller assistant finger between the handle of the wrench and the table.
The employee was walking and the wind blew some debris into the left eye causing a corneal abrasion.
Employee was descending right side ladder from boom truck. He slipped from step area as he was stepping down to the ground. Employee stumbled as he turned around and contacted this head on the outrigger cylinder housing, causing a laceration that required 4 sutures.
Employee was cleaning bolts in the shop with a rag and noticed irritation in his eye. He was taken to the hospital for evaluation an a piece of metal was found in his eye.
Employee was removing a hose from a loader when his hand slipped and struck against the loader frame, resulting in a cut to his thumb that required 4 sutures.
The employee was setting the last liner plate in the ring. He could not get the plates to line up. The employee was using a pry bar to wedge the liner behind the flange of the lower ring. In the process, the bar popped loose and struck the employee in his left cheek. The employee received stitches and a Zygoma fracture (chip) to his left cheek.
Employee was preparing for a batch of shotcrete when he attempted to lift the lid on the batch truck. The lid slipped and contacted the employee's right middle finger causing a laceration and fracturing the distal phalanges.
Employee was disconnecting a water hose that was hung on the rib of the drift by a cab hook. The cab hook spun in the palm of his hand creating a laceration which resulted in 9 sutures to close.
Employee was walking when he slipped falling to the ground causing injury to his shin which required 5 stitches.
Employee was attempting to loosen a hose fitting. When the fitting broke loose the employees hand went foreword and struck a sharp edge. This caused his left small finger to be cut in the middle of the finger nail. Employee received 2 stitches and the remaining fingernail was removed. Employee was wearing gloves at the time of the incident.
Leach pad Employee was moving to get out of way of reversing ATV (Drip Line installer on leach pad) and fell onto pad skid lacerating his left shin requiring 5 stitches.
Employee smashed his left thumb with 3 pound sledge hammer while driving a bushing into scam on differential. Employee did not leave work - first aid was provided. Emp. called supervisor at approx. 7:30pm and stated that his thumb didn't look good and wanted to go to the dr. Emp. was met at ER. Diagnosis was small fracture. Emp. was given RX and released to full duty.
Sup.was completing daily paperwork inside the site office trailer when he believes that he was bitten by a mosquito.He did not report the incident that day. On 4/30,he arrived at work, the lt.side of his face was noticeably swollen, an area appeared to be infected.He ind.that he tried to""lance""that area with his pocketknife while at home.EE seen a phys. who admin.IV antibiotics.
Cutting a jug in the warehouse and states the knife slipped and cut his left wrist.
Employee got his right thumb caught in the door of a 4-yd loader. His thumb nail was torn off and he received a laceration, which required 2 sutures.
Employee was shoveling ore off the ground outside of mill 5. He was near the building and as he was shoveling he hit the side of the building with his hand and received a cut to his left index finger. The cut did require stitches.
Employee was working on TD850 with co-worker, they were tightening a bulk head fitting which required 2 people. While kneeling down to hold the bottom of the bulk head fitting his co-worker was tightening the top the crescent wrench slipped off the bulk head and chipped employees top front left tooth. Dentist capped damaged tooth.
Employee was doing some clean up in his area at the assay lab. As he was cleaning he lost his footing and slipped. As a result, he hit the corner of the waste vibrator slide, hitting the top left of his eye brow. Employee did receive a small cut that required stitches.
Employee was using a Sawzall and some of the shavings got in his right eye. Employee sought treatment 03/06/12.
The employee was moving pieces of steel and pinched the end of the left ringer between the steel causing a fracture to the tip of the left ring finger.
Employee went to hit a center punch with a hammer, he missed the center punch and hit his left middle finger with the hammer. Employee was seen by the Dr at the clinic. XRay showed small chip on tip of finger, dr cauterized nail, tetanus was given & antibiotic shot. Employee was given a full release to return to work.
Installing cylinder in Bolter when it jammed and a piece fly off hitting him in the wrist.
Employee was at the surface shop getting his loader ready to go underground. He realized the seat needed to be adjusted and when he lifted the adjustment handle the seat slid back and pinched his left middle finger in the track causing a small laceration and slight tuft fracture of the end of the finger.
Employee was removing the spring steel off of an underground drill when his arm brushed across a metal burr on a guard causing a small laceration on his left wrist that required 5 sutures.
Employee was changing out tooling on a track drill when the tooling shifted and pinched his fingers between the tooling and the mast.
Employee was working on the relining of the mill and taking out bolts. His right hand came in contact with the impact drill, which resulted in stitches to the right thumb.
Employee put his hand in a pinch point when trying to release slack in chain rigging. This resulted in 7 sutures.
While using a swing hammer, mechanic pinched his right pinky finger between the tool and the head bolt on the mantle. Tissue injury required three stitches.
Emp was cleaning filter parts with grinder w/wire brush attachment. Someone called him he looked away. Grinder jumped causing the wire brush to travel across his left hand causing abrasion & punture to his left thumb knuckle. Emp examined by Dr Agbasi @ Golden Valley. Emp received Tetanus shot & couple sutures. Emp given instruction on keeping wound clean. Full duty release given.
Employee diagnosed with carpal tunnel syndrome and had surgery 05/09/2012.
EE was bringing loads of concrete from the Newmont slick line to bin #2. At some point during the first load concrete made its way in to EE's left boot. At no point did he take his boot off to inspect his foot or see if there was anything in his boot. At the end of shift, he took his boot off and discovered what appeared to be a possible burn.
