While night shift was beginning their shift, a SMD contract employee discovered one of SMD's haul trucks burning at the 4510-519 stope. Attempts were made to extinguish the flames, but were unsuccessful so the mine was evacuated. All personnel were evacuated with no injuries and the brass board was cleared at 7:45 pm. Several mine rescue teams were utilized to put the fire out.

Employee was assisting in the removal of a grader scrapping edge. Employee was picking up bolts after they had been knocked out by co-worker. When the last bolt was knocked out the employee went to pick it up and the unbolted edge fell onto his hand causing injury.
Employee was struck in the head and neck with a piece of falling mud while changing a tire.
Employee felt a pop in his wrist while trying to straighten a step bracket.
Driver was opening 4"" product valve on pheunmatic trailer to off load bulk crushed lime. Valve was stuck and he jerked on valve handle and said he felt pain in right sholder.Doctor said he had pulled muscles in right sholder and restricted him to no pulling or pushing of 20 pounds or more
The employee was replacing the liners in the Mill. The employee went to replace a loose liner when the liner fell striking his right shin and foot area. Causing a fractured metatarsal bone in the right foot.
Employee was lying under a Kubota tractor on his back removing a drain plug. The plug was tight, so the employee decided to brace himself with his left leg against the tire to get more leverage. As he turned the wrench he felt a crunch in his lower back.
Employee was removing rusty bolts from a discharge flange, using a 1 7/8 inch wrench.
Employee was riding out of the mine on a lube truck when he felt some discomfort in his abdomen. He did have a pre-existing abdominal hernia, however it was determined by three different physicians he had an abdominal muscle strain which was the issue, not the hernia. The determination of the muscle strain was determined on 11/3/2010.
Employee sought medical attention on 9/23/2010 for some respiratory distress he was experiencing. He had mucked in a recently blasted stope for a period of time the day before with a strong odor of ammonia. Employee was taken off work for one shift and returned full duty on 9/27/2010.
Employee was cutting existing fill from inside Cell 1 of the Maggie Creek cooling tower. Fill media 3-4' behind employee collapsed pulling the vertical column post which employee was using as a tie off point causing employee to drop approximately 22' to the basin. Multiple fractures.
While attempting to install a tire on an underground loader, employee was guiding the tire in place when the tire fell back on top of him fracturing his tibia just above his right ankle. Employee is currently off work.
Employee was struck in the calf by the pin of a shackle resulting in a bruise to the leg. Originally diagnosed as a non occupationial due to personal medical condition on 12/15/10 it was determined to be occupational.
Employee was removing frozen ore from semi bed, chunk of ore released from bed and struck employee on left foot pinning foot and causing employee to fall backwards resulting in fracture to left ankle.
Mechanics were removing the impeller from a cyclone feed pump. A mechanic placed his hand on the belt and his right thumb got caught between the drive belt and the motor sheave resulting in an open frature.

While washing down at the 4315 belts, employee slipped and fell injuring his right knee. Employee was placed on restricted duty.
Employee was walking from Midway building to the ready line, where his truck was parked. In between the South side of the midway and the ready line the employee slipped on a patch of snow covered ice causing him to fall on his left shoulder. Employee was placed on restricted duty.
Employee strained his back while lifting five drill rods back up from the ground and place against the rib. They weigh approxiamately 60 pounds each.
When employee stepped down off chippy hoist at 4450 station the ground was uneven, the skip was about 8"" higher that the station floor. He felt a pain in his left foot and burning sensation.
Lifting tool boxes in tractor and felt pain in abdomen. Did not become reportable until surgery on 11/29/10 to repair hernia.
Picking up 1 inch hoses and throwing them into the backend of a truck.
Employee stated that he was going up the steps leading to the mill control room. The employee stated that he had three point contact when his foot slipped on the first step causing his right foot to come into contact with the side of the stair case itself.
Employee was installing ground support with a jackleg. He was drilling in the springline collaring a hole. He bent down to reposition his leg and when he looked up a rock rattled loose and slid down the drill steel striking him on the right shoulder.
Miner was drilling with a stope drill and was struck in the palm of his right hand by a piece of metal that broke off of the string of drill rods.
While accessing engine compartment of haul truck to perform repairs, mechanic slipped. He felt pain in his arm after he caught himself from falling.
Employee was returning from taking his belongings to the crew bus when he tripped over his feet. The area he was walking in had no holes or rocks, it was flat. He fell down hitting his hand and face on the ground. He was taken to town for treatment and was found to have a fracture in his hand.
While swinging a hammer, employee missed and strained his shoulder. He was released to work with restrictions.
While lowering the cage from one deck to another deck, the hoist tripped out and dropped approximately 2-3 feet, this caused the employee to lose his balance and fall on his butt.
While setting casing the employee's hand was pinched between the driver sub and hammer bit.
Jib line caught on top head causing claim shell to elevate up and catch onto the employee's harness lifting employee up and swinging him into side of derrick.
While bolting up heading in 6-5530- material fell from face hitting EE on left side of his head and shoulder.
Employee was exiting his loader, when he went to open the door his hand slipped off of the door allowing it to close on his right knee.
While lifting hood on loader EE felt pain in his back between his shoulders resulting in restricted duty.
The miner received a thermal burn to the right forearm when the packer broke spraying him with hot water. (Over 130 degrees). The miner was tighting the valve when the packer broke sending hot water up the sleeve of his slicker jacket and under the rubber glove.
While using battering ram to remove Sag Mill liner bolts from the mill liners, on the swing back the mechanic on the non-impact end had his hand impacted between the battering ram and a suppot I-beam for the battering ram. Smashing his right hand giving him contusions.
While picking up a pemco box, employee strained left shoulder. He was originally given a full duty release, however the shoulder was still sore so he was placed on restricted duty on 7/22/2010.
Employee was installing pin on strut and was using short 4' step ladder. While descending ladder and standing sideways on lower step, he spun to left and with his right foot stepped off ladder and onto a 20 lb. sledge hammer that was standing with handle in vertical position and slightly to the left of the ladder. The employee had been using the hammer and placed it on the ground.
While attempting to adjust the forks on a forklift, employee lost his grip with his right hand and the fork dropped pinching his left ring finger between the fork and the forklift frame. Employee received several sutures to repair the wound and was placed on restricted duty.
Employee was struck in the lip with a tool while reassembling a brake. He required 5 stitches.
Operator completed a walk-around inspection on the dozer then moved crew van to parking area. While walking back to the dozer the operator tripped and fell to the ground and on his right hand palm causing a cut on the heal of his hand. Employee was taken to a medical facility and received two sutures for cut.
The Employee was pulling electrical wire, the wire flipped over pinching the right wrist of the employee between a motor and the wire he was pulling, causing a contusion of the right wrist.
EMPLOYEE WAS OPERATING SCISSOR LIFT #8, HE NEEDED TO ADJUST HIS SEAT. WHEN HE PULLED THE LEVER TO ADJUST HIS SEAT TO MOVE IT BACKWARDS HIS RING FINGER ON HIS LEFT HAND GOT PINCHED IN BETWEEN THE LEVER AND THE FRAME RAIL OF THE SEAT. IT CUT HIS FINGER BAD ENOUGH TO REQUIRE STITCHES
Employee lifted welding hood to inspect weld and retreive another welding rod. During this time a piece of slag or metal shaving entered his eye.
WHILE REMOVING HYDRAULIC OIL COOLER WRENCH SLIPPED CAUSING LACERATION ON KNUCKLES
While fueling tractor @ fuel bay, EE was walking around to the back of the tractor and tripped on containment berm. He fell into the bumper of the tractor causing a cut on his right forearm.
The Mechanic hit his right ring finger with a hammer while installing a Tie Rod end on PU 117.
While Bolting the jack-leg was turned on and the mat hit his arm cutting it. Two staples.
Employee was exiting the cab of a shovel and caught his left middle finger in between the door and door jam which resulted in a fracture.
While employee was removing bolts from a panel, the panel sprung open and struck the employee in the nose, resulting in a minor laceration on the bridge of the nose which required two stitches.
Employee was disconnecting camlock coupling which was tight, when it broke free it struck his face above right eye.
While employee was welding on a bucket a clamp that was used fell striking the employee in the lt. forearm causing injury
While pulling rods, emplyee dropped a rod and pinched his finger between the rod and the centralizer. The tip of the left pinky finger was fractured.
While placing boards in slot on back of drill rod trailer, board got stuck, employee applied more force. When board freed, employees hand contacted drill rod end and resulted in laceration.
while using a portable hydraulic press a piece came loose and fell on the employee's left foot. employee did not seek treatment and returned to work. He sought treatment on 8-31-10 as his foot was still a little sore. He was found to have a fracture of his left pinkie toe. He became reportable as of 8-31-10.
While repairing damaged phone cable employee recieved cut that required stitches.
While using a utility knife cutting a 2 inch plug out of a piece of rubber mat the employee's right hand slipped cutting his left index finger which required stitches.
While cutting string with a utility knife, employee cut his left pinky finger. The finger required sutures.
While removing a piece of scale from the quench tank, the employee's left thumb was caught between the scale material and the tank, fracturing his left thumb.
Employee lost his footing stepping over a containment wall, reached out to steady himself and received a puncture wound to his left hand from sharp corner of angle iron. Employee received sutures.
employee was cleaning up his work area after welding. dirt and dust was blowing around and something got into his eye. he was taken for treatment and debris was removed. he returned to work.
Employee was bolting the back with a jackleg drill when a small rock fell through the wire and struck his left upper arm. The wound required several sutures and the employee was released to full duty.
Two employees were removing a light and conduit,one employee was cutting the conduit,the other employee was pushing the light away when it gave way pining his hand and fingers against the existing conduit.causing the right ring finger to fracture the tip of the bone.
The employee involved was working at the Process Tanks to remove a 12"" spool pipe on slurry cooler #3 at phase II. When tapping the spool piece out the Vic coupling failed to hold Vic 741 in place. The valve popped out of its groove releasing acidic slurry into the work area. The employee received 1st and 2nd degree burns to the wrist, neck area and right elbow.
The employee was moving a hose hanger when he dropped the hanger. One of the hooks on the hanger punctured his right lower forearm.
Employee was grinding bed liners and at some point during the day, strong winds blew something in his right eye, behind his safety glasses. A metal sliver was removed the next day by the doctor.
Employee was loading bucket tooth onto service truck when right ring finger was pinched between tooth and tail gate of service truck.
Drill crew was removing water swivel for survey, drill pipe with swivel was laid down on top of head for disassembly, the driller had placed a 36"" pipe wrench on the swivel for removal, the wrench slipped off and the water swivel and driller let go of the wrench and placed his hand on the transmission motor to keep his balance and the loose wrench struck him on the left thumb.
While working on a 2 yrd loader, employee bumped the engine cover knocking the engine cover rod out of position, which allowed the cover to fall forcing his head down and he split his lip. Several sutures were needed to repair the laceration.
Employee was using a different methodology to install rubber liners in ball mill due to the fact that the liners would not fit using usual methods. A porta power was utilized to spread the liners on the shell of the mill to install the last liner. The porta power released unexpectedly and struck the employee above the eye.
employee had a sore thumb for some time and was originally checked at the health clinic in march (non-reportable at that time). On 6-25-10 the doctor reclassified him as a Reportable Illness and he had an outpatient surgery to treat his thumb, also received some sutures.
While cutting open a box with a knife, employee's hand slipped and cut the palm of his other hand. Sutures were required to close the wound and he was released for full duty.
An employee was carrying a computer, and while walking to the building, she stepped onto grating with her left foot and with her right foot, stepped into a small ditch at the end of the grating.
In process of moving the drill back on line the helper stepped forward to get a better look and placed his finger between the dewee dag plate and the footclamp plate on the drill. At the moment the helper did this the drill moved again pinching his finger between the dewee dag plate and the footclamp plate.
Employee bumped his head getting into the van after his shift was over. Several sutures were used to close the wound. He returned to work with a full duty release.
The employee was attempting to hitch a light plant to a pick up truck. The jack broke dropping the tongue of the light plant on top of the employee's right foot. The employee was evaluated by a physician and returned to his normal job duties. On April 14, 2010 after a follow up MRI it was determined that one of his metatarsals had a fracture.
Employee was cutting a part on the shape cutter. He had finished the cut and the part did not drop. He used a hammer to knock the part out of the plate; the hammer bounced off the plate, hit the side of the shape cutter bridge and drove the hammer and hand into the corner of the plate, cutting his right hand second finger knuckle.
Employee was holding a pin driver with a pair of channel lock pliers so another employee could drive out a pin from a pin boss. The hammer glanced off of the frame and hit the other employee driving his left thumb into a sharpe corner causing a cut on his left thumb. Requiring stitches.,
An employee was doing some grinding work, when he felt some debris and dust hit his chest and bounce up under his face shield and safety glasses.
employee was climbing down from a haul truck and felt a pop in her wrist. she was examined and sent for a follow up to see if it was sprained. she was placed in a splint for immobilization.
Employee slipped and fell landing on his cap lamp creating a large bruise. He was seen by a doctor several days later and released full duty, however he had to return on 2/4/2010 and have the area drained of fluid. he was release back to work for full duty.
A pulley fell out of it's holder and pinched employee's finger. He received 3 sutures to his right middle finger.
Employee was walking to the admin building and slipped on the ice. He fell and injured his rear and back, fracturing a rib.
While walking across the parking lot outside the main gate to the mine, the employee was struck in the right arm by a light vehicle. Employee required medical attention and was diagnosed with fracture to a bone spur. Employee returned to work full duty the same shift.
While operating ULD052, the bucket of the loader struck the left rib causing the employee to hit his head on the door window resulting in a laceration to the left eyebrow. Several sutures were needed to repair the injury and he was returned to full duty.
The employee was installing a belt on a pump and got his right ring finger pinched between the belt and the sheave.
Employee was working on a pickup truck tie rod. When he pulled on it, it came free and smashed his finger between the frame of the truck and the tie rod end. His left pinky finger required 5 sutures.
Employee is a Mechanic Foreman who reported having pain in his hands, elbows, and shoulders. He was seen by a Physician and diagnosed as having Carpal Tunnel Syndrome as of 12/15/2010.
Employee states that while tripping out rods, a pipe fell and entrapped his left middle finger between the pipe and bucket.
