While investigating decreased air flow in the 4540, a rib failure was discovered at the top of twin ventilation raises on the 4675. The raises are located behind a permanent bulkhead which can only be accessed when a sealed inspection cover is removed. Although the condition was in an inactive area as a courtesy was reported to MSHA. As a result a 103K order was issued.

In the 4510 Level North, the right rib came in. The affected area was estimated to be 17 feet x 12 feet x 5-10 feet deep. The affected area did not extend into the back nor did it impede passage or affect ventilation.
A fall of ground occurred in the 4100 Station East drift. An 8'x8'x3' deep section of back fell into the access as contract miners were working to relieve the basket. The miners were able to safely pass by the affected area where ground support was still fully functional. The affected area did not go beyond anchorage, affect ventilation or impede passage.
During the service cage tests at the beginning of shift it was noted that a recently replaced brake cylinder was not disengaging smoothly. It was decided to take the service cage out of service and conduct additional maintenance on the hoist braking system. No personnel were trapped and the Chippy remained in service throughout the repair.
Employee was replacing a loose liner with two other employees when it shifted and pinched his fingers between the liner and the assembly.
Employee was changing an air conditioner filter in the ceiling. The employee was on a step-ladder when he lost his balance and fell injuring his left shoulder.*Miner was given a permanent partial disability rating of 13% from his treating physician for partial loss of use of his left shoulder. Miner was given separation paperwork on 7/7/15.
Employee was working with another mechanic and was lining up a safety pin for the tailgate of a haul truck and his index finger was pinched between the end of the pin and the under side of the haul truck bed crushing the tip of the finger.
Employee was lifting a grease pump off the top of an empty barrel. The pump slipped out of his hands and when he caught it, the employee felt a pop in his right elbow. The employee continued to work normal duties until six working days later and re-aggravated the injury. The employee sought medical treatment on 1/6/15 and was classified as restricted duty.
The mechanic was using a pry bar to remove a trip lever when the bar slipped and the mechanic fell to the ground onto his left shoulder.
Employee was diagnosed with a concussion and taken off work after a gate latch impacted the left side of his face.
Employee was cutting free a closed tarp from a dumped, side dump trailer. When the tarp was cut free the tarp bar sprung upward striking the employee in the nose. The employee sustained a broken nose.
Employee was repositioning a 4 inch hose and felt pain in his lower back. He was subsequently seen by a doctor and taken off work.
Employee reported soreness to the back and shoulder after operating a haul truck.
Employee was walking on drill pattern when he stepped on an area of ground that consisted of uneven frozen mud causing him to roll his ankle.
While driving a 789 haul truck the seat belt contacted the seat recline control causing the seat to unexpectedly recline.
Employee turned to exit his cab and felt pain in his right knee, employee had surgery 05/07/14.
While riding on crew bus from time shack to location of equipment in pit, the bus hit a bump.
2 Employees were installing hydraulic fittings and hoses on some valves for dump leach. The hose came out of the hands of Employee and hit him in the mouth. Employee was seen by the dentist.
While trying to shotcrete the Southeast Decline, operator had lost air pressure and had his nozzle plug up. Sprayer operator shut off the air and started disconnecting the air and accelerator valve at the end of the boom when the energy was released it sprayed the operator in the face.
While lifting tool box on to service truck employee felt a pop and then pain in his left shoulder.
While changing filters on WT029 the mech. stepped off the boarding ladder and felt a pain in his left knee. Employee worked full duty until 3/23/15. At which time he received restrictions.
Mechanics were separating a gear box from an agitator shaft flange. The Victaulic coupler holding the shaft failed causing the shaft to slip through the hugger approximately 3.5 feet. During the agitator's fall an impact gun resting on a mechanic's leg was forced into the top of the mechanic's knee causing a severe fracture to his lower leg.
Employee was moving wire in preparation to start bolting when he felt a pop in the inside of his right arm near his bicep. After being evaluated the miner was given a restricted duty release.
Employee was struck by a piece of concrete remnant released from the wall.
Mechanic was lifting a tire jack onto the flat bed of a truck. He felt a pain in his left shoulder and returned to work. He did not report the injury until the following day. He was seen by a doctor on 7-31-14 and was given a full duty release. On 9-5-14 he was given a light duty release in response to an MRI indicating a torn rotator cuff.
Employee was traveling up the decline when his batch truck hit a pothole which resulted in the employee hitting his head on the cab ceiling. Medical evaluation determined the employee experienced a cervical thoracic lumbar strain and was placed on restricted duty.
The crew was placing concrete using a suspended concrete bucket. As the bucket approached its intended position, IP placed his hands on the gate handle with the intent to pull down handle to dispense cement. The IP had started to pull down on the handle when his hands slipped off the handle. The handle sprung back with force striking the IP in the lower jaw and chin.
Employee felt pain in his shoulder when swinging a hammer. He was taken to the doctor 11/24/14 and given restricted duty.
An employee was using a hand drill when it caught and twisted his right hand. He returned to work but on 12-1-14 reported that it was still hurt. An X-ray on 12-2-14 showed he had broken a bone in his hand.
While attempting to bend a wire mat, the wire mat sprung free, his left arm went straight suddenly, felt pop in left arm, began to feel pain in left arm.
Shaft plumb bob assembly being pulled up via it's winch. Cable crosby clamp caught on bottom deck, as employee went to investigate it suddenly came loose and snapped up and stuck employee on right side of face.
Employee was descending stairs while talking to a fellow employee and missed the bottom two steps falling on her knees.
While swinging a 3 pound hammer employee sprained his left hand. Employee's hand did not come in contact with anything.
When changing drill tooling on Track Drill 32, the drill operator placed the bushing fork into the flats of the 2 foot sub. While changing the down the hole hammer the bushing fork fell off the steel and hit the driller on top of his hand.
Employee was going to move a small box weighing about 5 pounds from the floor into on of the shipping boxes and when she bent over and started to move the box she felt a pop in her left shoulder and felt pain.
EE were loading a flange & valve into the back of a p/u tk. Emp. stood on tailgate & moved the valve so tailgate would close. Valve was positioned between the side of the truck & flange. As he moved it his finger was pinched between valve & flange resulting in a cut finger. Dr took x-ray, the tip of finger was fractured & 15 sutures required. EE placed on restricted duty.
Employee stepped into a drill hole. This did not become reportable until employee recieved a restricted duty release from the doctor 11/20/14.
Employee stepped off a concrete step onto an uneven concrete pad and sprained his left ankle.
Tech was placing hot cupels on board and the cupels started to tip. The tech reached out with left hand to stop them and was not wearing gloves. Tech received a minor burn to the palm of the left hand.
Employee was using a drill press when the table vice slipped, causing the employee to pinch his thumb between the vice and the drill press bench. This resulted in a laceration requiring sutures to close.
Employee was using an air lance to unplug the bottom of a tank that had acid solution in it. Received second degree burns.
Employee exited his mucker and in the process rolled his right ankle resulting in a sprain. The employee was placed on restricted duty.
The employee fell five feet from scaffold during erection. The deceleration device on his personal fall protection was deployed.
Employee was loosening a bolt with a crescent wrench on discharge boot on CIL #1. While loosening bolt employee felt a pop in his lower back which turned into a sharp pain that went through his lower back, hips and legs. Employee was taken to the Clinic and put on restricted duty.
Left hand was pinched between the man basket rail and the forklift.
Employee was stripping the inner layer of cable when the stripping knife slipped and lacerated his left hand just below the little finger. The laceration required sutures to close.
Employee missjudged the single step along a walkway and fell which resulted in a laceration to his right palm and required sutures to close.
Employee bent over to pull a bearing housing off of a hand cart when he felt pain in his lower back. Employee was seen by a doctor on 1/23/2014 and was given a restricted duty release.
Employee was working outside troubleshooting a pump. The wind was blowing dirt and snow. The employee got something in his eye. Employee was wearing safety glasses.
Employee was repairing a damaged metal door. When he hit the door with a double jack, foreign material from above went behind his safety glasses and into his right eye. Although the employee immediately rinsed his eye, the following day the remaining foreign material was removed by an optometrist.
When transferring some UNR829 Rudy inhaled some vapors.
When stepping out of a pickup the employee stepped on a rock, turning his ankle.
Employee was conducting normal sampling duties and felt pain in his shoulder. He went to the doctor 07/17/14 and was prescribed physical therapy.
Loader 52 was loading Haul Truck 105 when the operator of Loader 52 bumped the hoist lever causing the boom of loader 52 to fall slightly. The operator caught the lever and stopped which caused loader 52 to rock. The bucket of loader 52 struck the bed rail of Haul Truck 105. This employee was prescribed physical therapy when he was evaluated at medical facility at a later date.
Employee was opening the 4450 shaft station cage door when his left ring finger was caught in the door latch. This resulted in a laceration requiring sutures to close.
At around 8 am worker got shotcrete in the eye while trying to unplug a concrete hose. The worker was taking off the clamp of the hose for inspection, the hose still had some pressure in it causing foreign debris to get into his eyes. (Both Eyes)
As an employee got up to exit the cab he was turning to the right to walk around the pilot control shut off lever, he felt a sharp pain in the back of his knee.
Employee felt debris in left eye while riding in a truck.
Employee received a diagnosis of Carpal Tunnel and went in for surgery on November 24, 2014. Notification was made to HSLP on December 9, 2014.
