GROUND FAILURE. APPROXIMATELY 400 TONS OF MATERIAL FELL OUT OF THE 5230 INTERSECTION COVERING THE FRONT END OF UFR-004. WHILE THE UNIT WAS BEING PULLED OUT, THE BOOM OF THE UNIT WAS TORN AWAY. TOTAL FAILURE - APPROXIMATELY 700 TONS.

DURING THE MUCKING OPERATIONS AT THE VENTILATION SHAFT THE #2 BUCKET(EMPTY) WAS BEING LOWERED INTO SHAFT. AT 291' FROM COLLAR THE BUCKET HIT THE A(#1) PUMP. #2 BUCKET WAS SHUT DOWN. THE SHAFT, BUCKET & PUMP WERE INSPECTED USING THE #1 BUCKET. NO DAMAGE TO THE SHAFT, ROPE OR PUMP BUT THE LOWER RING OF THE CROSS HEAD WAS BENT. RING WAS REPAIRED. 2 METHODS OF EGRESS REMAINED

HOISTMAN WAS LOWERING A DRILL BASKET WITH THE #2 BUCKET & RAISING MUCK WITH THE #1 BUCKET. THE TOPLANDER WAS CONTROLLING THE DUMP CHUTES FROM THE COLLAR SWITCH BOARD. TOPLANDER CLOSED THE #2 DUMP CHUTE, INSTEAD OF THE #1 CHUTE. THE #2 DUMP CHUTE CLOSED ON THE WIRE ROPE CABLE WHILE THE DRILL BASKET WAS BEING DELIVERED TO THE BOTTOM OF THE SHAFT. CHUTE PINCHED CABLE INTO HEADFRAME

A SUBSIDENCE OCCURRED AT THE 4020 DRIFT A SMALL HOLE OPENED UP IN THE FLOOR. THE HOLE WAS THREE FEET WIDE, FIVE FEET LONG AND FIVE FEET DEEP. THE AREA WAS FLAGGED OFF AND THE APPROPRIATE AGENC IES WERE NOTIFIED AND WE WERE TOLD IT WAS OK TO FILL IT WITH MUCK AND MOVE ON. NO OTHER PROBLEMS WERE ENCOUNTERED.

Employee reached down to move his tool tote tray and felt a popping sensation in his lower back. Medical treatment was sought on 12/26/04 and he was taken off work.

Employee was working on a belly pan with a pry bar and overstressed his right shoulder. He was given a full duty release to work. On going pain caused him to see another doctor. and On april 22, 2005 he was seen by a Doctor who gave him restricted duty release and made this accident a medical reportable as of 4-22-05

EMPLOYEE WAS CLIMBING A LADDER TO CHECK THE JOHNSON SCREEN & HIT HIS HEAD ON A VENT PIPE ABOVE THE LADDER- JAMMING HIS NECK & CHEST AREA. EE WAS PLACED ON RESTRICTED DUTY.

WHILE WORKING ON WATER LINES FOR SCRUBBER, EMPLOYEE STEPPED WRONG ON A PIECE OF SHALE CAUSING HIS KNEE TO BEND IN THE WRONG DIRECTION WHICH IN TURN CAUSED A DISLOCATION.

EMPLOYEE LOST FOOTING WHILE DESCENDING STAIRS, CAUSING HER TO FALL. HER LEG WAS CAUGHT UNDER HER CAUSING THE INJURY.

Employee was installing a fan vent tube. When trying to adjust tube physically to secure with bracket, he pushed too hard & felt a rib snap. He thought he was fine but then took a step & felt a strong pain in the back rib area.

EE WAS WORKING ON A TRUCK AND LOST HIS BALANCE AND FELL. HIS KNEES WERE INJURED; INJURY RESULTED IN LOST WORK DAYS.

Employee bent over and was picking up a piece of pipe, when he slipped. He caught himself before he actually fell. Medical treatment was sought on 2/12/04 and he was released to full duty. A follow up visit on 7/20/04 resulted in the employee being taken off work.

EE SLIPPED ON THE ICE AND FELL ON HIS RIGHT KNEE. EE MISSED ONE DAY OF WORK AND WAS RELEASED TO FULL DUTY BY A PHYSICIAN.

EE WAS CLEANING HIS AREA AT THE START OF SHIFT AND SLIPPED ON THE ICE. ON 5/5/04 EE HAD SURGERY ON HIS KNEE AND TURNED THE INJURY INTO A LOST-TIME ACCIDENT.

DURING THE POURING OF THE FINAL CONCRETE LINER OUR MINERS ARE EXPOSED TO CONCRETE. DURING THIS PROCESS, EE GOT CONCRETE IN IS RIGHT GLOVE, HE CLAIMS THAT HE DID WASH OFF AFTER POUR, AND SHOWER ED AFTER THE SHIFT, BUT HE FAILED TO REPLACE HIS RIGHT GLOVE DURING THE SHIFT. WHEN HE CAME BACK TO WORK THE NEXT DAY, HE COMPLAINED OF THE BURNS ON HIS RIGHT WRIST & WAS TAKEN TO HOSPITAL FO

The crew of the production shaft were getting into the bucket for transportation to surface at the end of their shift. The last miner in the bucket entered facing inward, with his left arm rested on the rim of the bucket. The bucket belled up and before he could move his arm and his sleeve caught on the guard rail pulling his arm into a space between the rail and bucket.

Fall from ladders

EE was unloading a blowpipe from the muck bucket. The pipe was caught up in his jacket causing him to fall off the ladder backwards and sideways a total of four feet onto the ground. He stuck out his arm and fell onto it, pinching it in between the rail, himself and the blowpipe.

Employee stepped off from a 2 1/2 ft. flat bed trailer when his righ knee collapsed. This resulted in an ACL tear to his right knee. He was placed in a knee brace and given a light duty work release.

EE, AN ELECTRICIAN, WAS CLIMBING FROM THE 2ND LEVEL ON THE GALLOWAY TO THE BOTTOM LEVEL WHEN HE SLIPPED OFF THE LADDER AND FELL APPROX. 4 FEET TO THE BOTTOM DECK.

EE WAS ATTACHING THE WISHBONE ASSEMBLY ON DOZER. NOT NOTING SLACK IN RIGGING, EE REACHED UNDER THE WISHBONE TO LOCATE THE PIN RETAINER. AS HE REACHED, THE WISHBONE DROPPED APPROX. 10 INCHES, CRUSHING HIS HAND AGAINST SOLE PLATE OF DOZER.

EE WAS CLEANING PHASE II LIME CHUTE WITH A SHOVEL IN A LATERAL POSITION WHEN HE FELT A ""POP"" IN HIS NECK. HE WAS TREATED DAY OF ACCIDENT AND RELEASED WITH A FULL DUTY RELEASE. HE CONTINUED TO FEEL PAIN AND WAS FURTHER TREATED ON 8/6/04 AND TAKEN OFF WORK.

Employee was lifting a five gallon water bottle and as he placed it on his shoulder he heard a popping sound. Medical treatment was sought on 07/03/04 and he was relaesed back to work. A follow up on 07/21/04 resulted on him being placed on restricted work.

Employee was in the process of handling a 54lb. box of blasting boosters. As the box was falling off the side of the truck, employee attempted to catch the box which resulted in torn bicep muscle tendon. He did not seek medical attention until 05/24/04 and is scheduled for surgery on 05/28/04. He will become a lost time beginning 06/01/04.

EE WAS TURNING A TRACK ROLLER, DURING THIS OPERATION EE TRIED TO REMOVE METAL SHAVING FROM CUTTING TOOL. SHAVING WAS VERY SHARP AND CUT THRU EE'S GLOVE LACERATING FINGER.

EMPLOYEE HAD SLIPPED ON THE ICE AND STRAINED HIS BACK EARLIER (PRIOR TO SHIFT). AFTER SHIFT BEGAN, HE WAS HELPING ANOTHER OPERATOR LIFT TRASH SCREEN OUT OF THE END OF THE LOADED CARBON SCREEN AND FELT A SHARP PAIN IN LOWER BACK. EMPLOYEE WAS TREATED AND PLACED ON RESTRICTED DUTY.

WHILE EE WAS DOING HER ROUTINE TASK SHE GOT OUT OF SECURITY VEHICLE AND PROCEEDED TO WALK TOWARD SECURITY CENTER. SHE SLIPPED ON ICE, CAUGHT HERSELF WITH RIGHT HAND AND DISLOCATED HER RIGHT EL BOW. SHE MISSED 7 SCHEDULED DAYS AND WILL BE ON LIGHT DUTY FOR APPRX. 1 MONTH.

Employee was packing a pump using a mill bar to force the packing follower into place. The mill bar slipped and as he got his balance he rolled his ankle. He was instructed to wear a walking boot for about a week. I orginally tried to file this electronically on 12/28/04 but apparently failed because when I tried to close it out could not find accident.

While taking out an old liner a piece fell on his right foot. The employee did not want to seek medical attention at that time. After going home and seeing that his foot was swollen, he was taken to the doctor and was placed on restricted duty for one day.

Struck by flying object

Crew was tripping back into the hole after a bit change. As a piece of drill pipe was being picked up the metal on the clamshell broke. When the clamshell broke the tension on the cable caused it to kick back and hit employee in the left cheek, causing laceration to edge of nose. Stitches required.

EMPLOYEE FELT PAIN IN SHOULDER WHEN HE OPENED VALVE ON FIRE HYDRANT DURING FIRE FIGHTER TRAINING. HE REPORTED BUT DID NOT WISH TO SEEK MEDICAL ATTENTION AT TIME OF ACCIDENT. ON 12/20/04 DUE TO CONTINUED DISCOMFORT HE WAS TREATED AND PLACED ON RESTRICTED DUTY MAKING INJURY REPORTABLE.

Employee was tripping a drill hole and strained lower back muscle while handling drill pipe.

A MINER WAS WORKING ON THE BOTTOM DECK AND ANOTHER MINER WAS WORKING ON THE THIRD DECK OF THE GALLOWAY IN THE VENT SHAFT. THE JUMBOS AND CRYDERMAN HAD BEEN REMOVED. THE MINER ABOVE WAS COVERING THE HOLE LEFT BY #1 JUMBO WHEN HE PULLED ON A HOSE TO MOVE IT. THE HOSE WAS RAPPED AROUND A 5-GALLON BUCKET OF OIL WHICH WAS PULLED INTO THE #2 JUMBO WELL. IT HIT THE VICTIM ON THE HARD HAT.

Employee was carring an old impingement plate down a flight of stair, when he slipped straining his grion area. Medical treatment was sought on 10/12/04 and he was placed on restricted duty.

Employee was on the deck of the drill performing routine maintenance -greasing the traveling shieves. His right foot slipped and he fell backwards and to the right and landed on his right hand.

Employee stepped down off of a dozer and missed the bottom step because his bifocal lenses appeared to offset the step. While breaking his fall with his right arm he strained his shoulder. He sought medical attention on the morning of 10/04/04 and was given a shoulder sling and given one day light duty.

While conducting lighting PM's employee was desending down a fixed ladder when he slipped and fell, striking he lower back on an handrail. Medical treatment was sought on 10/04/04 and he was placed on restricted work activity.

This employee stepped on a rock while getting out of loader and twisted his left ankle. It resulted in a small tear in his ligament in his ankle. This resulted in one restricted work day.

Using a hammer to align coupling halves in tight quarters. He swung at the coupling and hit a come-along chain with the handle of hammer, missed the mark and struck left wrist with hammer head. He was taken to the doctor and treated for a bruised bone and placed on restricted duty.

EE came off of the acid tank because of flow problems. Went to have mask placed on face when the air was turned back on a small flake of acid went into his eye.

Employee was pouring samples from a sample mug into a sample bag. She raised her hand and hit the edge of a plexiglass.

EE brought a wheel barrel from third floor to the first floor to dump refractory into the dumpster. While unloading the wheelbarrel it tipped over and landed on the side of his left foot.

Employee stepped on a roll of duct tape twisting his left ankle. Medical treatment was sought on 8/04/04 and he was placed on restricted duty.

Struck by falling object

The machined edge of the oil seal retaining ring fell from a temporary 2"" x 4"" wood support while operator was replacing oil seal. Operator's right forearm was cut and required 12 stitches.

Employee was tripping pipe and doing helper job and for several days had progressively increasing pain in his left groin. Went to doctor on 7/29/04 to get pain checked out. Diagnosis was left groin strain

As the employee was placing a drill sub in the bed of a pick-up, he pinched his finger between two bits resulting in a chip fracture of the right index finger. He was placed in a splint and received light duty.

On 6/16/04 the employee was bending over from his drill to pick up a drill sample when he felt a pain in his lower back. He was taken to the hospital, treated and released to full duty. On 6/24/04 he was taken to the doctor for a follow-up visit and at that time he was placed on restricted duty.

Employee was using a scaling bar and it slipped and hit him in the mouth and he fell to the ground. This resulted in stitches to the mouth and hand.

Employee was going under a loader and hit his head on the loader bucket causing a strained neck. Restricted work days were the result.

Employee was removing hand wrench from drill pipe and was going to lay the wrench on derrick table. However a noise distracted him and the wrench slid off the table. The weight of the wrench on his arm and shoulder was not anticipated, resulting in a strained right shoulder.

EE WAS ASSISTING IN MOVING SOME METAL TABLES IN THE LAB. THE NEXT MORNING HE AWOKE WITH STIFFNESS IN HIS NECK AND SHOULDER. HE WAS TREATED AND RELEASED. DUE TO CONTINUED DISCOMFORT, HE HAD A FOLLOW UP VISIT ON 6/17/04 AND WAS GIVEN RESTRICTIONS THAT MAKE THE INJURY REPORTABLE.

EE SPRAINED ANKLE WHILE WALKING TO WORK AREA, HE STEPPED ON A SMALL BOLT CAUSING ANKLE TO TURN UNDER.

Fall from machine

Employee was climbing into pipe truck and his hand slipped off of handle resulting in a strained wrist. We question the validity of this accident since there were no witnesses, but Dr prescribed light duty.

Was throwing away trash bags and back started to bother her.

Employee was running out pipe on pipe truck while tripping from an exploration drill hole. As he was pulling pipe to front of truck, he strained a muscle in his lower back.

EE WAS STEPPING DOWN OFF A DOZER AND STEPPED ON A ROCK TWISTING HIS RIGHT ANKLE. HE WAS GIVEN A WALKING BOOT FOR IMMOBILIZATION AND 2 DAYS LIGHT DUTY. HE WAS INSTRUCTED TO WEAR THE WALKING BOOT FOR THE 2 DAYS OF LIGHT DUTY.

EMPLOYEE WAS HELPING LOWER A 100+ POUND TEE BAR WITH A CHAIN HOIST WHEN THE HOIST MALFUNCTIONED. EMPLOYEE TRIED TO CATCH TEE BAR TO STOP IT FROM HITTING ANOTHER EMPLOYEE AND IN THE PROCESS HIS THUMB WAS HYPEREXTENDED.

EE WAS INSTALLING SCREEN PANELS. THE SCREEN DECK MOVED CAUSING HIM TO FALL FORWARD. HE EXTENDED HIS ARM OUT TO CATCH HIMSELF AND FELT A POP IN HIS WRIST.

EMPLOYEE WAS SHOVELING SOME MUD OFF A LINER, FOR A POND TO REPAIR. HE SLIPPED AND EXTENDED HIS RIGHT HAND OUT TO CATCH HIMSELF.

Operator was cleaning the quench tank when he fell fracturing his wrist.

Ee was walking through a door connecting the mcc room & the hoist room & failed to step down & fell. he complained of chest pain & was taken to the hospital for examination. X-rays revealed bruised rib but he was given full release to come back to work as hoistman. He worked all his shifts till he returned to Dr on 4/12/04 & complained of lack of sleep.

Employee dislocated his right shoulder while loostening a c-clamp on the mud tub. He was not aware of any previous damage to his shoulder.

EE WAS PULLING ON A 4"" BULL HOSE AND STRAINED A MUSCLE IN HIS LWOER BACK RESULTING 4 RESTRICTED WORK SHIFTS.

EE WAS GETTING OFF THE CREW BUS AND SLIPPED AND FELL ON HIS RIGHT SHOULDER. THIS RESULTED IN TWORESTRICTED WORK SHIFTS.

EE started pouring the first load of concrete down the slick line at the production shaft when he realized he had not set the brake on the slick line tugger. Concerned he may have made a mistake he ran toward the tugger not ducking to clear the winch cable. He ran face first into the cable cutting his upper lip and bruising his nose; he also hit his head on the cement when he fell.

Employee was blowing the bottom of the production shaft. He was located near the center of the shaft under the cryderman but in view of the operator. He had blown a small quantity of material to the location where he was standing and then shut off the blow pipe. As he was walking away from the muck pile he was struck on the left leg/ankle with the cryerman bucket.

Employee lifted his welding hood to chip a weld when a piece of slag hit him in the face and flew up under his prescription safety glasses.

Miner was closing the trap door on the vertical ladder in the galloway of the production shaft when a weld broke on the handrail post and landed on his foot. He was taken to hospital and was given full duty, two days later he returned to hospital and x-rays revealed a fractured toe.

Employee was grinding and some of the dust came off of the grinder shield and hard hat and entered employee's eye. He did not report the incident until 12/6/04 when he was taken to the doctor and a foreign body was removed from his left eye. He was released to full duty.

Employee had been balancing and mounting light vehicle tires through out the day and began experiencing pain in his lower back and leg at the end of the day. He seen a chiropractor on 11/03/2004 and was given a full duty release.

Ee was conducting training session on operation of emergency generator system. The generator idled for awhile and ee switched to run mode. Explosion occurred, ee was either knocked back by explosion or stepped back and lost footing. He fell and was knocked unconscious. Taken for medical evaluation and released with no restrictions.

HELPER WAS TRYING TO UNHOOK HOSES WHEN INJURED EE BENT OVER TO HELP THE HOSES CAME APART STRIKING INJURED EE IN THE HEAD BETWEEN SAFETY GLASSES & HARD HAT.

Putting out signs and cones to alert of heavy equipment in area and slipped through cattle guard causing a laceration on left shin. Employee was taken to medical facility and received stitches.

Employee was pouring grout when he noticed an irritation to the right eye.

Employee was dismounting a haul truck when the flex step under the bumper swung back and struck him in the left shin. He recieved two small lacerations which became infected. He was treated by a physician for the infection.

Employee was leaving the property, when he felt something in his eye. He sought medical treatment after repeated attempts to remove the irratant. He was given prescription eye drop medication for his eye.

Miner was ramping the 5305-614. he had his hand outside of the cab of uldoo5 and pinched his left middle finger wheile making a tight corner.

Employee was retrieving wood lathe out of the back of light vehicle #EX18 and in doing so his foot slipped causing him to fall onto the tailgate fracturing two ribs on has left side. Employee sought medical treatment on 9/29/04 and was released full duty.

Employee was standing on a three foot step ladder using a handled piano wire to cut the windshield loose from the frame of a truck. The wire broke and the employee fell backwards lacerating his arm on the rear view mirror frame. The laceration required five internal and twelve external sutures.

While welding, ee felt something in eye. at time felt it was some dirt but due to continued discomfort had eye treated following day and a piece of metal was found and removed.

Employee was bolting and a rock fell from the back and struck his hard hat. Staples were used to close the wound.

Employee put on his work coveralls at the start of shift and a piece of wire wheel must have been caught in the clothing from previous work day, as the employee sat down in a chair the metal wire piece imbedded into his back resulting in a physician removing the object and applying one suture.

Employee was pushing a portable cart when it hit a small crack in the floor. This caused the cart to move suddenly and caught her finger between the cart and a sample bin. The cut she recieved resulted in 4 sutures.

A gust of wind blew through the open shop door, employee thought he got the particle out of his eye. He returned to work the next day with irritation and sought medical attention. The doctor removed a piece of plastic from under the eye lid that was not imbedded. He was given a prescription for antibiotics.

WHILE WALKING ACROSS THE WELDING SHOP FLOOR, EE HOOKED HIS FOOT IN A LOOP HOLE OF A WELDING LEAD WIRE AND FELL DOWN. HE WAS CARRYING A CLAMP AND WHEN FELL HE STRUCK HIS KNEE ON THE CLAMP. HE W AS RELEASED FULL DUTY BUT THE DOCTOR APPLIED AN IMMOBILIZATION.

THE TOP LANDER WAS MOVING FROM TUGGER TO THE BELL WHEN HE DUCKED TO GET UNDER THE GALLOWAY WENCH CABLE. HE RAISED UP TOO SOON HITTING THE CABLE WITH HIS NOSE AND UPPER LIP. WHEN HE JERKED AWAY FROM CABLE EITHER HIS FACIAL HAIR CAUGHT ON CABLE & HE RIPPED HIS UPPER LIP OR THE IMPACT WITH THE CABLE CUT HIS LIP. HE WAS TAKEN TO ELKO HOSPITAL WHERE THE CUT WAS CLEANED & SUTURED.

Employee started feeling nausea, headache and tingling up and down her arm. She was transported to the University of Utah Hospital where she was operated on for 2 brain aneurysms.

On 12/15/04 employee was lifting small lifters into place in the sag mill. He feels he hurt his back at this time. On 12/29/04 he informed his supervisor of the incident. At this time we took him to the doctor and he was placed on restricted duty.
