Accident type, without injuries

The service cage was being brought to surface with a paste pipe rack loaded on the second deck. The rack contacted the sub-collar and damaged the divider between the first and second decks on the service cage. There was no damage to the shaft.

At approximately 10:00 p.m., during the post blast inspection of the 1330-705 heading, a fall of ground was identified. The fall of ground ranged from 7 to 13 feet high and worked back from the face approximately 50 feet. There were no injuries or damage to equipment as a result of this event.

At approximately 2308 on Wednesday, October 5, CAT793D HT953 right rear inside tire caught fire. HT953 was traveling loaded South on the North/South Haul Road just north of the Oasis water stand.

Hoistman identified that was having issues with the service cage positioning on the hoist monitor. The empty service cage was then sent in manual to the 4450 station, a known location, to verify the accuracy of the position indicator. In the process the service cage traveled past the station and contacted the shaft bottom damaging a guide wheel and chair linkage.

Contract elevator operator let employees off at level 8 and as EE began descending, the operator felt a jolt. The elevator came to a rest and was between levels 7 and 8. Employee was unable to exit the elevator as it was stuck between levels. Contract elevator company was called in and able to move the elevator, according to their SOP's, for safe exit by the employee.

A fall of ground was discovered at the 4200-201 intersection. At the time of discovery there was no work at or beyond the affected area. No equipment was involved and there were no injuries as a result of this event.

Fall to the walkway or working surface

Employee tripped and fell while walking; falling to the ground. Employee sought medical treatment and was taken off work for one shift.

Struck by falling object

An employee was working in a manbasket of a powder truck loading a development round in the 4470 North Hanging Wall. The employees right forearm was resting on the rail when material fell from the face. The material fell onto the forearm causing a laceration and later identified a slight fracture.

While removing a piece of metal from a conveyor, the employee lost their grip on the piece of metal. The metal that the employee lost their grip on, contacted and rolled off of the employees lower leg.

Mechanic was removing the right rear tire/wheel assembly from a mucker. After removing all of the lug nuts the tire slid off the spindle, tipped over and fell onto the mechanic resulting in a minimal, nondisplaced fracture to the pelvis.

Employee was removing a bucket and shovel from a tractor when left leg went out from under ee. The employee was diagnosed with internal derangement of the left knee and taken off work. There were no witnesses to the event.

Struck by flying object

Employee was trying to loosen up a bucket tooth on LD466 (992 Caterpillar front end loader) when a piece of metal 3/8 inch by 1/8 inch came off Snap-on steel faced hammer and struck employee near right collar bone. Transported to medical facility where piece of metal was surgically removed.

Struck against a moving object

Haul truck 930 traveling from the 45 stockpile, section 3 area towards the North/South haul road went through the water bar and slowing down for a stop sign, the operator and trainer heard a loud bang which startled the Operator. The Operator reported some pain in lower left back after the event had occurred. Operator was seen by a Nurse Practitioner and placed EE off work.

Employee had been mounting/dismounting a tractor throughout the shift. Towards the end of the shift, the employee reported pain in the left knee. The employee sought medical treatment and was unable to make the next scheduled shift.

Operator on TD856 called and reported that they had pulled a muscle in arm. Employee was lifting a 70 lb drill bit to place on drill rod. This injury became reportable on May 24, 2016 when employee received a restricted duty release during a follow up Dr. visit.

Struck against stationary object

Employee was dismounting a Caterpillar Jammer in the 4490 and while stepping down, stepped on a rock. The rock shifted as well as the employee resulting in a severe sprain to the foot and mild sprain to the ankle. No broken bones.

Over-exertion in pulling or pushing objects

Employee unloading 1200 lb bags of Aluminum from trailer. It had tipped over in truck which made it hard to unload. Employee used a pallet jack to move bags. The employee reported hurting shoulder, on way home was sent to clinic, then Occupational Doctor following day received full release. Went back on 03/14/2016 MRI and Therapy scheduled. Became lost time on 3/15/2016

Fall to the walkway or working surface

EE was in the process of depressurizing a discharge line on a portable pump when a reducer on the line failed. Discharge line contacted the ee and the ee was knocked to the ground at which time some material entered ee's right eye. This resulted in a corneal abrasion and a subsequent sensitivity to light which did not allow the employee to return to work.

Absorption of radiations, caustics, toxic and noxious substances

Employee sat down to close the bottoms of EE's trailers in water that was contaminated with lime. Causing a chemical burn.

Technician was repairing air pipe in mill. Due to weakness in shoulder, when residual air from line came out, arm dislocated from the socket. This is the 2nd time this has happened in 2 months. Technician was taken to hospital to have arm reduced into place.

Fall from machine

Our driver was unloading Hydrochloric Acid when EE went up trailer ladder (approx. 4-5 feet) to disconnect air supply hose. EE disconnected the hose, vapor and some product came out and EE dropped the hose to the ground. Product continued to leak out of the hose and EE hurried down the ladder to stop the leak. EE missed the last rung and fell to the ground, fracturing right heel.

While getting out of a service truck the employee felt pain in the right knee when the right foot made contact with the (flat) ground. The employee went to the doctor and was put on restricted duty.

Over-exertion in lifting objects

While an employee was installing a drive-line in a rubber tire dozer, EE felt a pop in EE's left bicep and experienced pain in the same arm. The diagnosis is a complete tendon tear.

Maint techs were changing out sub adapter as retainer was broken at the hex head. Welder cut old sub adapter off and techs proceeded to remove pin with pin punch and sledge hammer. The tech held the pin punch with left hand on the end of handle and right hand on the handle near the punch. The sledge hammer glanced off of the punch striking employee's right index finger and knuckle.

Employee twisted leg and resulted in EE falling and hitting EE's knee on a rock. Causing contusion and bruising to knee.

Employee was returning from the scale house to the truck, which was parked on the scale. As EE turned to change directions, EE twisted right knee, injuring it.

Operator was coming down from HT and EE's right foot slipped off the rung of the flexible ladder and EE fell to the ground, landing on EE's right foot.

While tripping pipe onto the Rolagon, employee was on the Rolagon sliding the 10 foot RC rods into place. When the employee reached down and pulled on the pipe ee felt a pull / strain in right shoulder.

Employee was investigating a needed repair to a headlight on a tractor when the engine cover shifted causing a laceration to left index finger requiring sutures to close.

Employee was in the process of dismounting a light vehicle tire off of a wheel, using tire bars when employee felt a pop in right wrist. Employee went to the doctor and was placed on restricted duty.

Shovel operator felt a pain in back after stepping on a piece of gravel.

Employee was helping pulling equipment out of DW 69 when hand came into contact between the power tongs and the column pipe, smashing two fingers on left hand

Employee was lifting an argon bottle and felt pain in their back. EE was taken to the doctor and given a restricted duty work release.

Employee was leaning over a rail to reconnect a 4"" flange and while lifting on the pipe to line up EE felt a pain in back. EE was put on restricted duty on 6/20/16.

While climbing a ladder the employee felt a pull in EE's groin.

Employee was kneeling down clearing an obstruction when EE stood up EE's left foot slipped on a rock EE heard EE's knee pop and felt a sharp pain. Employee was taken to the doctor where EE was given a restricted duty release.

While exiting EE's cab a driller felt a pop in EE's knee when EE's foot hit the ground.

Employee was working in engine compartment of 793D Haul truck. While attempting to remove the intake boot the Employee momentarily lost balance. In an attempt to catch self, employee caught left hand pinky on unknown object on the engine.

Struck by rolling or sliding object

Employee was scaling a rock out of the back when a rock began to slide along the scaling bar. The employee attempted to deflect the rock and received a laceration to EE's left hand which required sutures to close.

Injured employee was cutting collar pipe when the knife became stuck. In the process of freeing the knife the employee cut left forearm resulting in a laceration requiring 3 sutures to close.

Struck by flying object

Employee was using a wire wheel to clean some gasket material off a pipe flange. Some of the wires separated from the wheel and one of them stuck in right arm. A doctor had to remove the wire piece and employee received 3 sutures.

Employee was working a cubex drill. EE placed one hand on some hydraulic hoses then activated one of the controls. When the hoses moved from the pressure EE felt a prick to left hand in the webbing between the index finger and the thumb. The next evening the employee reported the incident and was taken to the hospital as a precaution for infection.

Installing pipe in mill. Employee holding pipe overhead, stepped forward to adjust strop, right shoulder popped out of joint.

Pulling off an old swellex bolt and as employee pulled the bolt took a step back to get balance and slipped and fell backwards.

An employee was using a mill bar to clean out a chute. As the employee was bringing the bar back, EE struck EE's hand on a cross member bar, causing a laceration to EE's right hand. The employee received six sutures to close the wound.

LD-237 (Caterpillar 992) loader was going down for mechanical reasons, LD-245 (Caterpillar 994) loader was called to support Sage mill feed. While tramming with a full bucket, the operator drove up on one of the stockpiles which resulted in the loader tipping over and coming to rest on its right hand side. Following Work Care's recommendations operator was released back to work

Struck by flying object

Employee was struck in the face with a 3 pound hammer head that came off its handle while another employee was driving a shaft through the planetary gear.

Employee's right wrist was cut by a copper plate that penetrated EE's work glove.

A shaft crew employee was attempting to clear an obstruction from the belt magnet. The belt caught the scaling bar the worker was using and pinched the employee's fingers between it and the support structure holding up the belt causing a finger laceration.

Struck by falling object

Employee was grabbing a wire off the stack that was leaned up against the rib. Employee took a step back and the whole pile fell on top of employee knocking employee down and landing on top of employee.

Welder was blend grinding a repair on the hard nose of LD245 when the grinding wheel of the 41/2"" Dewalt grinder grabbed and ran up the steel plate contacting the employees left inside forearm causing a laceration. Employee was wearing all proper PPE and all guarding was in place on tooling at time of accident.

While Shotcrete Sprayer was ready for tramming and boom folded back, operator forgot to bleed the system of air and water. The nozzle was pointed in ee's direction and blew accelerator, water and dirt to the side and back of the head at 90 psi.

While deburring a flange in the lathe with a hand deburring tool, the tool slipped off of the flange and the right thumb contacted the other side of the flange, cutting EE's thumb.

The employee was struck above the left eye by a wrench while trying to get a stuck steel out of the hole.

Employee was replacing a hoist brake actuator when EE's left hand got caught between EE's wrench and a piece of the brake assembly. After removing EE's glove the employee noticed a laceration on the webbing between EE's thumb and index finger. The laceration required sutures to close.

Employee pinched EE's left index finger between a 2 inch airline and the rib causing a laceration to the tip of the finger requiring sutures to close.

Struck by falling object

Employee was stocking a heading with bolting supplies when EE lifted EE's hardhat above EE's head to wipe the sweat off EE's forehead. At the same time a bolt that was leaning up against the rib tipped and struck the employee on the head resulting in a laceration requiring sutures to close.

Employee was loading a piece of drill steel into the rod basket and EE's finger was caught in between the drill steel and the basket.

Fall onto or against objects

Employee had a 16 penny nail in EE's right hand when EE heard EE's name called. When EE turned to see who had called EE, EE tripped over an air line and fell to the floor. The nail cut EE's left wrist requiring 7 stitches to close. Employee was released with no restrictions and returned to work.

Employee was installing a yolk on a transmission when the socket slipped off the yolk nut and struck the employee in the chin resulting in a laceration requiring sutures to close.

Mechanic was tightening cutting edges bolts on RT022 with a 1"" impact. As EE was tightening a bolt, the impact slipped off of the nut and the side handle of the impact hit EE in the mouth, chipping EE's left front tooth

The employee was using a 4"" grinder with a 4"" thin cut blade to cut a piece of 4"" pipe. The pipe was approximately 26"" off of the ground. The wheel got pinched in the pipe and the grinder kicked back and out of hands. It struck lower right leg causing a 1-1/2"" laceration. The employee received two inner stitches and 12 outer stitches.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was driving up the ramp and began to feel light-headed and dizzy. EE stopped truck and called supervisor and got out of truck until help could arrive

Employee slipped on ice and dislocated EE's left thumb.
