There was a ground fall at the Deep Post mine, Deep Star side of the mine on the 4570 level. This ground fall impeded passage to the work area. MSHA mine inspector was notified and went to the area.

At approximatly 12:30 pm a ground fall was discovered at the Deep Post mine in the Deep Star Decline. It was located 75 feet above the second set of steel sets going up the ramp. The demensions were 9 high 12 wide 14 feet long. This was reported to MSHA. Rehab of the area has begun per MSHA approval.
POWER OUTAGE AT THE MINE SITE WITH MEN IN THE SHAFT. TWO FUSES WERE BLOWN ON THE EMERGENCY GENERATOR DURING STARTUP. THE MEN WERE SAFELY EVACUATED AFTER REPAIR.
At 7:30 PM the Gold Quarry North Dump failed, approximately 10 million tons of waste rock and clay materail were displaced. Approximately 1.2 million tons will need to be removed from Nevada State Route 766 to open the road again. Incident was reported on 2-05-05 and we were instructed not to file a report at this time on 2/06/05.
Employee states while attempting to slam tube down barrel his glove became entrapped between tube nut and barrel. Partial amputation and fracture right index finger.
Employee was driving a pickup, bounced up and came down with his elbow on his lunch box that was sitting on the seat, pulling his shoulder. Medical treatment, was sought on 12-29-2005, he was released back to full duty. His shoulder continued to bother him and he underwent surgery on 3-23-06, for a torn rotator cuff.
There was an accident involving two haul trucks. Employee sought medical attention and was taken off of work.
Operating a Case farm tractor & disk, disking clag. Turned out of the clay raising the disk, turned in the seat to look back at the disk & drove over a rough area causing him to turn & twist in the seat. *Note: Seatbelt was being used.
Employee was carrying a core rod and stepped on a hose twisting his knee. Employee had knee surgery on February 16, 2006.
While washing SH108 with water cannon, employee fell on deck jarring shoulder. Employee sought medical attention and had surjury on 12/1/05 on his shoulder.
Employee was desending a ladder when he slipped and fell. Medical treatment was sought and he had several sutures placed to close some lacerations on his wrist and elbow. he also was placed on restricted work.
Picking up a compressor motor - strained low back.
Employee was loading a face round when the rib/back fell in. The rock struck him on the head/hard hat. He was taken to the hospital the next day and the Dr. placed him in a neck collar for precautionary reasons.
OPERATING SCRAPER. HIT SOFT SPOT IN HAUL ROAD. TOSSED AROUND INSIDE OF SCRAPER, HITTING HEAD ON RIGHT WINDOW (CONCUSSION) AND HURTING LOWER BACK.
Employee was assembling shoring. He removed a form from upright staged bundle. Bundle slid forward striking employee and pushing him into a deep hole. He landed on his feet and buttocks fracturing his pelvis.
Employee cut his right hand when the wrench he was using to tighten steel bolts slipped. He did first aid and continued to work. The cut later became infected requiring treatment on 5/8/05.
EE WAS WORKING WITH COWORKER IN THE SUB-COLLAR OF THE UCAT SHAFT. THEY WERE WORKING NEAR A MANWAY THAT HAD BEEN CUT OUT IN THE DECK TO ENABLE ACCESS TO PIPE THAT NEEDED TO BE INSTALLED. A RETRACTABLE FALL RESTRAINT WAS OVER THE HOLE AND SIGNS SHOWED OPEN HOLE. EE FELL APPROX. 12' (CONCUSSION).
Employee stepped on the PVC pipe when working on the leach pad and fell onto his right shoulder. He was given a full duty release at this time and sought medical treatment on a later date and was recommended for surgery for a rotator cuff tear 6/28/05. *Original on-line EDoc # 211279*
Employee was hanging ventilation bag in his work area, while lifting a section of vent bag with one arm, the miner felt acute pain in his right shoulder.
EE WAS STRUCK BY CHOCK BLOCK THAT HAD NOT BEEN REMOVED COMPLETELY FROM REAR DUALS ON HAUL TRUCK.
Employee was getting off tractor and rolled his left ankle. Employee had surgery on 4/4/06 for ankle reconstruction.
Employee was a member of a team cleaning out scrubber and cone return tank. They removed two pipe caps and a clean out. When scale was removed from the clean out, hot water exited into the scrubber cone filling the base. The area was hot and filled with steam and the employee had to leave the area and in doing so contacted the hot water in the base of cone.
Employee received grout burns to both eyes when the grout return line came out of the agitator tank and sprayed him in the face. This accident occurred during shaft grouting operations at the Leville Vent shaft.
Employee was pulverizing samples and was turning the bowl over to dump out the samples and the bowl slipped out of her hand. She reached for the bowl to keep it from rolling off the table and in the process twisted her wrist. She was placed on restricted duty.
Walked out of batch plant and slipped on ice and fell.
Employee was loading some 2""X 1/4""X10' angle iron onto his work truck when he felt a sharp pain in his left groin area.
Employee had placed a Duff Norton Jack under a haul truck and jacked it up. He then had a stand placed under the truck. When he pushed the lever to lower the jack, it popped loose hitting him around the mouth cutting his face. Employee also broke his ankle, but unsure how. He was taken to a medical facility and received stitches.
Employee was pushing 10' drukk rods on to bed of truck. Experienced pain in upper left arm.
Employee was climbing off a loader when he felt a pain in his right knee. He said the knee had been hurting from a prior, unreported incident that occurred while getting out of a van. Employee was taken to a medical facility, given pain meds. & placed on restricted duty. Follow up appointment scheduled.
LIFTING 4 X 8 X 3/4"" SHEET OF PLYWOOD. AT 11:30 AM FELT ILL AND FOUND BULGE IN ABDOMEN. DIAGNOSED AS HERNIA ON 5/11/05.
WORKER EXITED HAUL TRUCK TO PERFORM AN ""AFTER LUNCH"" EQUIPMENT INSPECTION. WHILE PERFORMING THE INSPECTION, HE STEPPED ON A SMALL ROCK,""TWISTING"" RIGHT ANKLE. HE REPORTED THE INJURY TO HIS IMMEDIATE SUPERVISOR, BUT CHOSE NOT TO SEEK IMMEDIATE TREATMENT. APPROX 10 DAYS LATER, HE CHOSE TO SEEK TREATMENT FOR THE INJURY RESULTING IN THE APPLICATION OF A SPLINT.
EE ATTENDING A Co. SPONSERED BBQ, HAD BEEN SITTING ON SOME BLEACHERS. WHEN THE EE GOT UP TO LEAVE, INSTEAD OF STEPPING DOWN ON THE NEXT STEP OF THE BLEACHERS HE STEPPED DIRECTLY TO THE FLOOR. THIS DID NOT BECOME REPORTABLE UNTIL THE EE HAD S SCOPE ON 4/18/05 WHICH SHOWED A TEAR & WAS TAKEN OFF WORK AT THAT TIME PENDING SURGERY FOR REPAIR. PLEASE SEE THE ATTACHED EXPLANATION
Employee was walking by a rib and a rock fell out of the rib and struck his right shoulder. He went to the Emergency room on 3-22-05 and got a full duty release. His follow up visit with a orthopedic physician on 3-25-05 where he received a restricted duty release. That made this accident a medical reportable.
Employee was working on lifting pump and finger got caught in chain of pump which caught his glove and pulled it in and ripped finger.
Employee was pushing a hydralic jack through the mud to position it under the rear of a haul truck. Employee slipped and felt a strain in his lower back after falling. Medical treatment was sought on 1/31/05 and he taken off work.
Employee twisted his left ankle on1-15-05 and on 1-16-05 he was seen at the ER and had a tore his acchellies tendon in his left ankle. This resulted in surgery and restricted work. As of 1-17-05 this was reportable.
Two employees were lifting wheelbarrow to dump contents into a dumpster when one of them felt a sharp pain in his lower back. Employee was taken to a medical facility and placed on light duty.
He was holding a part that was being removed when the part fell hitting his hand
Was prying with a bar, a rock that was wedged between wheel and frame when he strained his back.
EMPLOYEE STEPPED IN SOME SOFT DIRT WHILE WALKING TO HIS VEHICLE CAUSING HIS ANKLE TO TWIST. HE SOUGHT MEDICAL ATTN.& WAS GIVEN LIGHT DUTY FOR SEVEN DAYS. HE RETURNED TO WORK WITH A FULL DUTY RELEASE ON 12/03/05.
WHILE USING T-POST DRIVER TO DRIVE IN GROUND ROD, EMPLOYEE LIFTED THE DRIVER MISSED THE ROD AND THE TPOST SLIPPED OUT OF HIS HAND & LANDED ON TOP OF HIS FOOT.
Changing hose on bolter when he tripped and caught himself with his right arm and strain his shoulder. The accident happened on 11/14/05 but he did not get medical attention until 11/29/05 when he was placed on restricted duty.
Employee stepped out of tire service truck into a rut causing his ankle to twist.
Employee was operating compactor on the new tailings pond. The compactor shifted into neutral causing it to roll about 200 ft. Compactor stayed in upright position but the machine was jarred by the movement and rough ground conditions causing some bruising to the employee.
Employee was walking down walkway and looking off to the side when his boot got caught on a step and he hyperextended his knee. He sought medical attention on November 21 and was scheduled for a MRI. He was put on restricted duty on November 25 and is scheduled for surgery on the 15th of December.
A HAUL TRUCK OPERATOR INJURED HIS LEFT KNEE WHEN HE STEPPED FROM THE LEDGE OF THE HAUL TRUCK HE WAS OPERATING, TO THE GROUND.
Employee felt a pop in his back while bending over to pick up locks for shovel teeth. Employee complained of pain and was taken to a medical facility. He was placed on light duty.
EMPLOYEE REPORTED HIS ELBOW WAS HURTING. HE STATED IT ONLY HURT WHEN HE PULLS BACK ON THE JOY STICK IN THE SHOVEL.
PLACING 18"" ROCK INTO DITCH, ATTEMPTED TO BREAK ROCK WITH BUCKET OF 345 TRACKHOE BY HITTING ROCK, THIS JARRING THE EXCAVATOR RESULTING IN STRAINED NECK AND BACK. *original EDoc # 266042*
Employee was using a come-a-long and a hook to reconnect a mast chain for a drill. While tightening the come-a-long, the other hook started straightening and the come-a-long hook slipped off hitting the employee in the collar-bone. He was taken to a medical facility and diagnosed with a broken collar-bone.
Employee originally hurt his hand on the 10th of October, employee was compensating for hand injury and hurt his shoulder.

While installing a blind flange on a valve, the flange slipped shifting weight all on employee's left arm. He heard a pop and felt throb in arm.
Employee was stepping over an air filter unit and stepped on a grab handle twisting his ankle. Medical treatment was sought and he was placed on restricted work activity.
Employee started the tractor from the side it jumped forward pinning him between the stairwell and the tractor.
Employee was organizing laydown/storage area for demobilization of project, including stacking lumber forms and discarding unusable pieces of wooden formwork. The employee stepped on a nail that was protruding from a 2x4 piece of lumber that penetrated his foot.
Crew was changing a drill bit and employee's left pinky finger was pinched between drill bit and drill table. Emlpoyee received a laceration to the tip of his finger that required stitches.
Employee was attempting to exit cab of lube truck, facing out. He had ahold of door handle and handrail on exhaust stack, his foot slipped and caught the step on top of fuel tank. He hit his right hip on a concrete wall corner. He was taken to a medical facility and placed on light duty.
Employee and two coworkers were changing the impeller of a pump. They were in the process of re-rigging a sling around the impeller when it tipped over against the employee's leg, pinning him against a pipe. This became reportable on 09/07/05 when he was put on restricted duty.
EE was putting the orifice plate back on pump using air impact. He was holding the nut with his left hand, activated air impact. The nut had a burr on it and when impact was activated the burr cut thru employee's glove into the palm of hand causing laceration.
Employee states while walking from drill deck to control panel his knee gave out.
EMPLOYEE REPORTED BEING BIT BY A SPIDER ON THE LOWER LEG WHILE OPERATING A D9R DOZER. ON 11 AUG, HE SOUGHT MEDICAL ATTENTION FOR A SWOLLEN AND DISCOLORED LEFT BIT TOE. HE WAS TREATED AND PLACED ON RESTRICTED DUTY.
EE was traveling across bench going thru rut from haul truck, service truck suspension bottomed out jarring lower back.
EE STATES HIS SHOULDER POPPED AS HE WAS EMPTYING A 55 GALLON TRASH BARREL INTO A DUMPSTER. NO IMMEDIATE PAIN BUT THROUGHOUT THE DAY EE STARTED TO FEEL PAIN THEN NUMBNESS DOWN HIS ARM INTO HIS HAND AND FINGERS.
Using an air hammer to help removal of keeper pins on teeth for lip protector on a loader. Pin punch became wedged so employee tried to use air hammer on other side. When he pulled the trigger, his left little finger became wedged between the hammer and the next tooth.
Employee was stepping down off of the bus that was delivering him from town. He stepped on some uneven ground spraining his right ankle. At this time he was not a medical reportable. on July 15th he went for a follow up and was placed in a walking boot and placed on light duty.
Cleaning rocks and debris in tunnel. He reached into a moving conveyor belt to remove a rock and the rock shifted and pinched his hand between the rock and a bolt. He was taken to the doctor and was placed on restricted duty.
Employee was installing a wheel cover while lifting and twisting to get bolt holes lined up, when he felt a slight burning sensation in his left abdomen. Medical treatment was sought on July 6, 05 and he was diagnosed with hernia and released to full duty. Surjury to repair the hernia was conducted on 9/22/05 and he was placed on restricted duty.
EMPLOYEE STEPPED ON A SMALL ROCK AND TWISTED HIS ANKLE
Employee was getting into mucker when his light cord got caught on the door handle which snapped his neck back and hit his hard hat on the ROPS.
Employee was cleaning out a lifter when a rock fell from the face and struck him on the left back, fracturing three ribs.
Crew were pulling 5' tube. Employee grabbed the tube with left hand and released the overshot with right hand. When overshot release employee lost balance. Regained balance and pulled teh tube out of drill string. Later complained of pain in lower back.
Employee was stepping down onto a hydralic tank and stepped on a picking eye twisting his ankle. Medical treatment was sought and he placed on modified duty.
Employee lost footing off explosives truck and twisted his ankle. (Fell off truck while loading a round.)
While dismounting a haul truck, employee stepped in a hole causing his foot to roll. This resulted in a cracked bone in the right foot.
Employee removed a pump from back of truck by reaching over side and lifting with one arm causing pain in shoulder. He reported incident to supervisor, but did not want medical attention. He went on regular days off, returned to normal shift on 4/25/05, and asked for medical attention due to pain in shoulder. He was taken to a medical facility, and was placed on restricted duty.
EE was attempting to close a pressurized sulfuric acid line valve. When the valve wouldn't close, EE removed the bolts on the valve packing. This caused the acid to spray out through the packing onto the EE. EE was not restricted until his 4th follow-up visit with a doctor different from the original.
While climbing down the ladder of track drill (equipment #TD656), with one foot on the ground, and still hanging onto the handrail, the employee felt a pain in his right elbow. He was taken to a medical facility and placed on light duty.
While running a jammer employee hit some backfill and some dust got in eyes. He went to the ER and got a pescription eye medication and two restricted days which made this medical reportable.
Employee felt a twinge in his right shoulder while pulling a sample through the deck of the drill. Employee went home after shift, but the pain became worse, so he called his supervisor. He was then taken to a medical facility and placed on light duty.
Employee struck in lower back with overshot when driller pulled the wireline control instead of rotation control. Overshot pulled up to the pulley at top of mast and wireline broke. Overshot fell on deck and bounced striking employee in back.
EMPLOYEE WAS CARRYING TWO BAGS OF CONCRETE ACROSS THE COLLAR DOORS & STEPPED OFF OF A 14"" ELEVATION. EE INJURED HIS RT. KNEE, EE WAS RELEASED TO LIGHT DUTY & TOLD TO WEAR A BRACE THAT HE HAD FROM A PREVIOUS INJURY.
EE was driving an underground truck and due to EE inattention, tipped the truck on its side resulting in a right shoulder injury. The emergency room Dr gave EE a restricted work release that resulted in this medical reportable accident.
Employee was lifting timbers under the drill mast while the driller was positioning the mast to set up a 45 degree drill. A deck timber slid when the mast was being moved.
He picked up box of boosters, turned and walked to truck, then complained of pain in lower back. At the time of the incident, he did not wish to receive medical attention. The following day, the employee wanted to seek medical attention. He was taken to the doctor and placed on restricted duty.
WHILE PLACING CONCRETE, EE WAS KNOCKED OFF OF THE FORM WALL BY PUMP TRUCK HOSE, CAUSING SPRAINED KNEE.
Employee was hanging utilities in a mucker bucket, once the bucket was lowered, he forgot he was tied off. He hit the end of the lanyard and lost his footing. He landed on his left elbow on the lip of the mucker bucket and received a laceration.
Employee was getting on his truck at the truck shop after repairs, while using a 4' ladder the ladder kicked out causing the employee to fall to the ground and sustaining fracture injurys to his leftside ribs and scraping his left shin, employee was transported to a medical facility for treatment and put on light duty.
Employee was putting in a hydraulic pump by hand when he felt a tear in his right shoulder.

Operator was getting off haul truck at end of shift, he stepped onto some uneven ground frozen mud and sprained his ankle. requiring a splint on his ankle.
Employee was climbing down the ladder on the the side of the pipe truck. He slipped and fell from bottom step. He strained his back muscle when he fell. Employee worked the remainder of shift and 3 more days. Went to doctor on 4th day because soreness was not getting any better.
The employee was removing scale in a tank. A piece of scale fell from the wall of the tank causing the employee to lose his balance and fall back. He grabbed the agitator to catch his balance, injuring his right wrist.
EE was climbing steps to get into operators cab of dozer, slipped on first step and in the process of trying to catch himself from slipping, felt something pull in his shoulder. he did not wish to seek medical attention the day of the injury but due to continued discomfort was treated on 1-24-05 and placed on restricted duty.
EMPLOYEE WAS AIDING IN THE REPAIR OF A CLARKSON VALVE ON SLURRY COOLER PUMP. AFTER LOOSENING A VIC COUPLER, SLURRY STARTED RUNNING OUT OF FLANGE AND THE SPRAY FROM THE SLURRY CAUSED BURNS ON THE EMPLOYEE.
EE BENT OVER TO PICK UP SOME PRIMA DET CORD, STEPPED ON A CLUMB OF FROZEN MUD AND TWISTED BACK
Employee was using a die grinder to cut off a exhaust pipe when the grinder kicked back and cut his wrist. Medical treatment was sought and sutures were used to close the wound.
Employee was driving his haul truck when the louver for his heater fell into vent. As he retrieved it his finger came into contact with the heater fan blade. Medical treatment was sought and sutures were used to close the laceration.
Employee was installing center section of driveline for LD025 when he lost his balance and driveline landed on his hand. Medical treatment was sought and sutures were used to close the wound.
Employee was underneath a haul truck changing a hydraulic hose, when the wrench slipped down and hit employee in upper lip and broke a tooth.
Employee was stripping a 3/0 XHHW wire, when the blade slipped off of the wire, allegedly cutting through a leather glove and cutting his left index finger above the knuckle. The laceration required 5-sutures to close. Employee was wearing leather gloves instead of a cut-resistant material such as Kevlar (as required).
While cleaning, she overused muscles that were not in adequate shape. Muscles were strained and her elbow was dislocated. Drug screen results negative. Possibly not a valid claim.
EMPLOYEE WAS IN CAB OF DOZER USING HAND SIGNALS TO DIRECT TRUCK TRAFFIC. HE HIT THE XM RADIO MOUNT WHICH HAD A ROUGH EDGE CUTTING THE END OF HIS FINGER ON IT. THE CUT REQUIRED 2 STITCHES.
Employee was working on the bed of a underground haul truck and ran his hand down the sharp side rail of the bed resulting in a laceration of the palm of his right hand. The laceration required 6 sutures. He was released full duty.
Employee got his foot caught under a hitch while hooking up trailer hoses. Medical attention was sought and found to have fractured the tip of the second toe on the right foot. Employee was given a full duty release.
Employee was using a grinder and as he rotated the object he was using he came in contact with the grinder surface cauing a laceration on his hand. Medical treatment was sought and sutures were used to close the wound.
Employee was walking and slipped on a patch of ice, landing on the right hip.
Employee knelt on floor to help position forks on a pallet lift. Later in the morning the employee noticed blood on the knee of his pants and found he had a laceration. Employee did not know what he might have knelt on. The laceration did require stitches.
Employee was loading a 12 ft drill steel into the back of his haul truck. When he raised his arm to load the steel, his left shoulder dislocated.
An employee was tacking a metal plate in place when a piece of slag landed in his ear. After a few consultations with a physician, he was eventually refered to a surgeon to repair a hole in his ear drum that was apparently caused by the slag. He had surgery on 2/24/06, to repair the hole and returned to work for his next scheduled shift.
EMPLOYEE WAS LIFTING A HAMMER DRILL ONTO THE BACK OF A FLATBED TRUCK. THE LITTLE FINGER ON HIS RT. HAND WAS SMASHED BETWEEN THE HAMMER DRILL & THE END OF THE TRUCK BED.
EMPLOYEE WAS USING POCKET KNIFE TO CUT THE WRAPS. THE KNIFE FOLDED CUTTING EE'S FINGER. THE LACERATION REQUIRED A STITCH TO CLOSE THE WOUND. EE RETURNED TO HIS REGULAR SHIFT & DUTIES THE SAME DAY OF TREATMENT.
While sitting in dozer employee leaned forward and struck his head on the radio. Medical treatment was sought and sutures were placed to close the laceration.
Employee was crawling across a steel beam when he twisted the right knee.
Employee tightened an anchor bolt and the nut got tight. The employee pushed harder on the wrench to snug the nut down. There was a welding BB in the threads of the bolt and when the BB broke loose the employee's hand struck the tank.
Lifting 50 lb sample bags repeatedly possible copycat injury report because the injured thought the work was too hard. laid off effective 10-21-05.
Employee was lining up a pipe support to weld it when he felt a pop int he right shoulder.
Employee was dismounting uniloader and slipped on a bolt that was in the bottom of the bucket. He fell hitting his face and cut his right eyebrow with the edge of the safety glasses. Employee was taken to a medical facility and received stitches. He was released to full duty.
RIDING PASSENGER VAN TO HAUL TRUCK LOCATED IN PIT AT START OF SHIFT. VAN HIT A BUMP CAUSING EMPLOYEE'S HEAD TO HIT THE ROOF OF THE VAN.
Employee was changing cutting edges. While holding bolt it spun, cutting his finger. Medical treatment was sought and sutures were used to close the wound.
EE HAD BUILT A FRAME OUT OF ANGLE IRON WITH WHEELS. HE PICKED FRAME UP TO PAINT BOTTOM OF FRAME AND IT SLIPPED, CATCHNG HIS FINGER BETWEEN FRAME & STEEL TABLE.
EE WAS LOOKING UPWARD WHILE FITTING PIPE WHEN A 1/2"" COUPLER FELL STRIKING THE FRONT TOOTH
Employee was using a hammer to remove lip protector when a piece of steel flew up and stuck into his leg. He was taken to a medical facility where the metal was removed, and the employee received 2 stitches. He was released back to full duty.
Unloading a bin, the gate released and another bin came down and smashed her fingers. Drug results pending. Doctor told her no work until 8/29/05. (Letter from Newmont regarding validity of claim attached to original report).
Employee struck on back of hand right by 4"" x 5"" x 1/2"" sharp rock causing a nasty laceration.
Employee changed the bit on the drill & was in the process of lowering the steel back down on the new bit. The steel was not going to line up properly so the operator stepped out to straighten out the bit & caught his right upper palm area on a sharp edge of the drill steel. Employee sustained a laceration that required five sutures (full duty release).
The employee was entering the elevator and tripped over the door which she did not open all the way. It was up about four inches. The employee fell to the floor on her right knee. She began to experience some discomfort in her knee and developed a small bruise. On 9/2/05 she went to the hospital to have fluid drained off of her knee.
EE, along with another employee, was replacing a broken Dixon Cam Lock fitting. They had trouble getting the new fitting into the hose and during the process, the cam lock let loose, hitting the employee in the mouth piece of respirator. The employee scheduled a dental appt for 08/16/05, at which time a chip was discovered and capped.
Employee was working on gang forms when he slipped and fell approx 6 feet with his fall protection. Hard hat fell off and employee's head struck a form tie causing laceration to scalp.
While cleaning for a drill hole with a Fin Hoe a rock rolled down and he hit his finger on the rock. Cutting his middle finger requiring 4 sutures.
Climbing down a wooden job ladder. Brushed arm against rail and picked up a sliver.
EE WAS INSTALLING A SPLIT SET WHEN A ROCK CAME DOWN ON THE WIRE FALLING BACKWARD & HITTING HIM ON THE HARD HAT. THE ROCK KNOCKED OFF HIS SAFETY GLASSES & SCRATCHED HIS LEFT EYE.
EE was filling acid digestion tanks. An acid line valve failed, spraying acid onto the employee. He was taken to the ER and given a full-duty release and a prescription for some eyedrops.
Employee was holding the door on a van for exiting coworkers. Another employee, not knowing EE's hand was in the way, closed the door and smashed EE's pinky. Eight sutures were applied.
While performing regular maintenance work the employee was swinging a hammer. His hand slipped and he struck his left index finger with the hammer. X-rays showed a fracture in the tip of the finger.
Employee was operating loader, he went over a bump and hit his head on the ROPS.
Driving haul truck. Drove through a soft spot in the road, causing the seat of the haul truck to bottom out - strained EE's back. EE was examined by physician and released to full duties. Reportable as of 8/5/05 when EE sought medical attention from a chiropractor. Released full duty.
Employee was assisting in changing out tooth on loader bucket when another employee struck the tooth with a hammer. A piece of metal flew off the tooth and embedded in the employee's forearm. Metal sliver had to be removed and a suture was used to close the wound.
EMPLOYEE WAS TIGHTENING A FILTER HOUSING WITH A CRESCENT WRENCH WHEN IT SLIPPED. HE BROKE AND CUT HIS FINGER.
Employee was bolting when he woke up next to the bolter. Employee was unaware of what happened.
While breaking the seal on a 5 gallon bucket with a box knife, the knife slipped and resulted in a laceration to his left ring finger and palm. Stitches required.
Ee was cutting with utility knife when he slipped and cut his knee.
Employee was installing a T-iron when he reached around the bottom of the brace. His thumb got caught between the iron when the crane bumped up a bit.
Employee was moving steel around when he felt a strain.
Operating haul truck and his eyes started burning, next day right eye was irritated. Employee received prescription antibiotic.
Haul Truck #1 staged to pull under loader (bed facing loader); Haul Truck #1 began backing to the loader, and contacted the bed of Haul Truck #2. No damage to the haul truck; minor injury to the driver of Haul Truck #1 (two stitches to left side of scalp).
Employee was starting a drill hole. His hand was on the pull down cable. A burr on the cable caught his glove and pulled his left pinkie finger into the sheave. Laceration and fracture to the tip of his finger
The employee noticed irritation in his right eye on May 11th. He does not know the exact time that the eye started to bother him. About 5:00pm that day he reported it. He asked to seek medical attention on May 13th.On the 13th the Doctor Removed a tiny piece of metal from his eye and gave him a prescription for an antibiotic.
While bolting, a bolt mis-loaded. EE went to look and the bolt slid out, hitting him in the lip.
Employee was positioning a pipe into place when the pipe shifted it pinched his right thumb between the pipe and the pump casing causing a laceration that required several sutures to repair. Employee was released back to work full duty.
Employee altercation resulting in cut above eye on one of the the employees involved.
EMPLOYEE WAS REMOVING BEAT SEAT BAND ON TIRE WHEN PULLING FLANGE OFF OF WHEEL, IT SLID TO RIGHT SIDE PINCHING HAND BETWEEN FLANGE & TIRE HANDLER PAD.
While working on a leaking line, solution sprayed under employee's glasses and got into his eyes. He was taken to a medical facility and released to full duty. At follow up appt. on 4/25/2005, he was given a prescription for antibiotic eyedrops and released to full duty. Seen again on 5/3/05 with no problems.
EE was installing resin bolts in back, pushing resin bolt up into hole with jack leg, spinner came off of nut. 10' resin bolt fell out of back, struck EE on right wrist. It hurt, it hurt bad.
Employee was using a razor knife to cut a plastic zip tie, when the knife slipped and cut his left thumb. Medical treatment was sought and sutures were placed to close the wound.
Employee's right ring finger was pinched between forks on forklift and resting plate.
Employee was working on the Galloway at the vent shaft cleaning concrete out of a water ring. With approximately 220.000 CFM of air coming up the shaft, it blew a piece of concrete or rock into his right eye. The object came up under his safety glasses.
EE CUT THE TOP OF RIGHT THUMB WILE PULLING A HARD DRIVE OUT OF A COMPUTER. EE SOUGHT MEDICAL ATTENTION TO GET STITCHES TO CLOSE UP THE CUT
This employee had a gang box lid fall and it caused a laceration. on 3/4/05. The Employee did not report this to the company, and went on days off. When he returned to work we were told by him that he went and got stitches. This made this accident reportable. We were not notifed for 4 days and there was a break down in reporting that is the reason for this being subbmitted late
Disassembling axle hub for brake repair, putting a 5 1/4"" socket on impact wrench, he accidentally touched trigger. Socket came off. His finger came between axle hub's sharp edge and the heavy socket.
Employee's left little finger was caught in tongs resulting in laceration/fracture
WATER SWIVEL HOSE CAUGHT ROOF SUPPORT BRACE (BRACE WAS UNSECURED), CAUSING THE SUPPORT BRACE TO HIT EMPLOYEE'S FINGER.
Employee was pumping from a acid pond when a hose broke spraying solution on to his back. Some solution managed to get into his eyes and medical treatment was sought. He was given a prescription for his eyes.
Employee was unloading bags of prell when he felt a strain in his left shoulder.
While walking down stairs, employee rolled her left ankle on a mat at the bottom of the stairs causing a strain/sprain on her left ankle and knee.
Tripping rods in hole. put water swivel on last rod. Swivel hung on head, popped up, hit helper in mouth, chipped tooth.
EE GOT INTO THE CAB OF LOADER 29 AND REACHED TO CLOSE THE DOOR. THE DOOR BEGAN TO CLOSE UNDER ITS OWN WEIGHT. THE TIP OF EE'S RIGHT MIDDLE FINGER WAS CUT BETWEEN THE DOOR AND THE CAB, REQUIRING 6 STITCHES TO CLOSE.
Employee was replacing the lid back on strainer of strip vessel. EE struck the lid with a hammer to seat the lid and a small piece of dried scale broke off and went into ee's eye. ee was wearing ppe including safety glasses.
Employee was stripping cable and hurt his wrist. (This is repetetive motion carpal tunnel syndrome)
EMPLOYEE INFORMED SUPERVISOR ON 11/10/05 HIS SHOULDER WAS HURTING AND SUPERVISOR REMOVED EMPLOYEE FROM FRONT END LOADER AND DIRECTED HIM TO OTHER DUTIES. EMPLOYEE WENT TO ER AT NORTHEASTERN NEVADA REGIONAL HOSPITAL ON 11/18/05 ON HIS OWN WITHOUT NOTIFYING EMPLOYER.
