Groundfall at the 4080 level. MSHA contacted and inspected.
Fire lasting longer than 30 mins. after discovery burned grass, HDPE pipe, and power poles near CIC. No injuries, no environmental release, no cyanide issues & no outside resources needed. Fire was caused by arcing on a jumper insulator on a line pole in an area where vegetation is dry. 3 water trucks responded ( 1 put out fire, 1 on hot spots, and 1 on stand by).
A miner just pulled his Jumbo Drill out of the N.W. 5785-971 heading. The miner noticed some dribbling coming off the back in an intersection. The area was secured from unauthorized entry of personnel. The back fell above anchorage points. The fall of ground occured between shifts.
While in the process of bolting the 4020 west ""E"" R-8 heading a large boulder came out of the back. The boulder went beyond the anchor points of the support bolts. It was estimated to weigh between 8 and 10 tons. There were no injuries and minimal damage to the bolter.
At approximently 7:00 am on May 7, 2006 a Dynatec employee noticed a ground fall on the 4835 level of the Deep Star area of the Deep Post Underground mine. It was located in the B-101 heading. This area began working a couple of days earlier but at this time it was still in an inactive area. After it was secured the roof fall continued out into the active work area.
Groundfall above the roof bolt anchorage in intersection necessitating rehab.
Approximately 80' past the Deep Star cross cut wash bay, in the direction of the Deep Star Mine the left rib (east rib) springline area sloughed in. The ground fall was approximately 20' long and 18' high. Utilities were dropped to the sill with minor damage. The ground fall resulted in blockage of the Deep Post secondary escapeway, loss of ventilation, and loss of water.
Shaft was down 12 hours due to damage, inspection, repair, and subsequent investigation.
Employee states while walking and carrying pipe, he slipped and fell to the deck. Pipe fell onto his right ring finger.
Employee was in the process of helping another employee install a rock bolt when a rock fell out of the face and struck him on the right foot. The impact resulted in fractures of all five toes on his right foot.
EE was carrying a ladder across the parking area when he slipped on the ice, causing him to fall and twist his ankle. He went to have surgery on his ankle 4-11-07, this is the date lost time will start.
Prior to the start of his shift, employee was walking down ramp toward change room when he slipped twisting his knee. Employee did not seek medical attention initially, but later requested to see a doctor. His knee was drained of fluid and is currently off work.
EE was hosing down CIL containment for clean up work. The sump pump went down, causing the containment to back up with milk of lime solution. The solution was at or near the top of the EE's boots, dampening his pants.
Employee states while removing hydraulic hose from shovel, the hose fell onto finger and pinched finger between hose and metal on shovel. Injury resulted in laceration/open fracture of right little finger.
Employee was walking down stairs when she missed the last step and twisted rt. ankle, causing sprain.
Employee was fueling in a haul truck, as she removed the nozzle diesel fuel sprayed her in the face and upper body. She sought medical treatment on 9/04/06 and was taken off work.
The mining crew was completing a slip form pour in a vertical shaft. A foam ball was put into the slick line to clean it out, when the foam ball was blown through the slickline the hose whipped and struck EE knocking him into the rib and to the top of the form. As a result the EE sustained a head injury and was transported to the local hosp- ital he has a skull fracture.
Employee was getting off of a piece of equipment and simply twisted his left knee and felt it pop. The incident occurred on 8/24/06 but he did not seek medical attention until 8/29/06. At this time he was diagnosed with a tendon strain and was placed on light duty.
Employee was removing bolt from a suction tube, as he was pulling in his ratchet, he felt a sharp pain in his left shoulder. Medical treatment was sought on 8/05/06 and he was released back work full duty. A follow up and MRI confirmed a tear, employee had surgery on 10/06/06 to repair the shoulder and was taken off work.
Employee states as he was wiping tube, he grabbed the overshot, as tube kicked. His left hand was pinched.
Employee was operating a scraper and was told to go help dozer operator who was covering 4 inch drain pipe. The pipe pulled away from the 8 inch connector. They removed about 2 wheelbarrows of dirt to make the connection. Employee was digging with a truck cleaning shovel. A co-worker stated that he did not see employee pick up the 4 inch pipe.
LHD ran out of fuel and when loader engine stopped, loader ran down ramp.
Employee has developed continued pain in right elbow since early March. Employee sought medical on 05/23/06 and was given a full duty release. Employee went back to doctor on 08/25/06 and placed on restricted duty.
Employee pinched her arm between the arm rest and truck door. She didn't wish to seek medical attention at time of accident. It continued to bother her and in July was treated and released to full duty pending surgery date. On 9/12/06 employee had surgery to correct neuropathy and was taken off of work. It became reportable at this time.
Employee was stepping off ladder onto a concrete deck. The concrete floor had just been washed down and when he stepped onto the concrete his foot slipped causing his right knee to twist. Employee did not seek medical attention at the time, but later decided to seek medical attention and had surgical repair on 5/25/06. He is currently off work.
When sweeping, she started to turn red & itch. After awhile her throat swelled & breathing was difficult. Post accident drug screen results - Negative. Sweeping in ""dry"" area.
Stacking mill balls into a bucket, the bucket became top heavy and fell over pinning employee under the bucket. It took a crane to lift the bucket off of him. Drug test results pending.
Was shoveling bent over under a conveyor belt and woke up the next morning with a sore back. was notified today that he saw a dr. He missed one day of work. Drug screen results-negative-.
While walking down some stairs, the employee's right knee locked up causing him to fall forward and in the process of catching himself twisted his back.
While doing some repair work inside a mill a piece of the lining struck his right ankle.
Employee slipped on some slurry and injured his knee. He was taken to a medical facility, evaluated, and returned to work. After some continuing problems with his knee, he got an MRI. On 12-18-06 it was determined that employee would need surgery (to be scheduled) but could return to work.
EE was cutting a 2"" plastic pipe with a chainsaw. He didn't cut all the way through the pipe, which put the saw in a bind, stopping the saw. EE tried kicking the pipe and his left foot slipped off, twisting his right knee and falling to the ground.
Employee tripped over floor drain causing him to lose balance, he fell froward striking his shoulder on a bench. Initial diagnosis shoulder strain/contusion with full duty release. Re-evaluation showed a rotator cuff tear requiring surgery to repair. Surgery performed on 10/10/06.
Employee was pulling the slack out of a cable to keep it out from under the tires of haul equipment. The cable started to feed itself out and the employee was struck in the legs by the cable.
Pulling/lifting rods & rod fell on right hand.
Employee was walking accross the yard, stepped on a rock and rolled his right ankle. He sought medical treatment and was placed on restricted duty.
Employee and 2 contractors were pulling a sump pump to repair. Employee got a Come-a-long and was hooking it up to move the pump so it could be lifted out of the area. The lifting strap broke at that time and the pump fell hitting the employee in the shoulder area.
Employee was removing sample buckets from the back of pickup, stepped in a pot hole and twisted her ankle.
EMPLOYEE WAS INFORMED BY CONTRL RM OF A PLUGGED CHUTE ALARM. EMPLOYEE WAS REMOVING CHUTE COVER. WATER CONTACTED DRY LIME CAUSING A BURP THAT COVERED HIM WITH LIME ON FACE AND BODY.
Employee states while loading bags of drilling mud onto mud tank, he experienced popping in shoulder.
Employee was loading ore samples into bin on contractor's truck when he felt a ""pop"" in his shoulder. He was treated and released to Restricted Duty.
Employee heard a pop in his knee after getting a sample and returning to the drill from deck. He was taken to a medical facility and placed on light duty.
While doing some fabrication work, EE had to use the grinder and his thumb made contact with the grinding stone.
Employee fell off of a ladder. This resulted in a fractured wrist and laceration to lip which required four sutures.
Driller was in the process of breaking drill pipe loose from the top head when a pipe wrench slipped on the pipe and kicked back into his left hand.
The employee was stepping off of lube truck when he twisted his knee. The doctor put him in a knee brace making this reportable.
Employee was checking a loose fan belt on a forklift. He grabbed the belt, pulling upward twice, and felt something slice across his fingers. The employee caught his hand between the belt and pulling causing lacerations on his fingers.
Employee stepped off the drill, started walking around to put up drill stakes, stepped on a rock, and twisted his right ankle.
990F loader was loading 777D haul truck with rocky material containing very little fines. The first bucket contained a large rock. With no material in the bed or the truck to cushion the shock, the rock landed in the bed and ""shook"" the truck driver around. He sought medical treatment for a ""sore"" back on 10-18-06 and was diagnosed with a lumbar strain.
While entering a loader the employee became caught between cab and door of 3 1/2 yard loader. He was taken to medical attension and given a full duty release on the day of the event. On 10/16/06 employee requested to return to the doctor where he was placed on light duty pending results of a MRI.
Worker was walking alongside the backfill area for the overflow channel, when he stepped on a soft spot losing his balance and twisting his right ankle. The worker reported the injury to the Project Supt on 10/4/06 and chose to seek treatment the same day. The physician diagnosed a 2nd degree sprain, w/o restrictions. A 2nd opinion on 10/6/06 resulted in restrictions.
The EE was cutting wire while tying rebar, when the wire was cut, it whipped back and punctured the EE's eye. The EE was wearing safety glasses at the time of the injury.
Unloading drill steel off rack when several steels rolled down on top of foot. Resulting in restricted duty.
He was putting together the pump line on the shotcrete truck. While holding the pipe against his leg he was attemping to install a pipe clamp and felt his lower back pop. Resulting in restricted duly.
The Supervisor was helping load some trash wire on the forks of a TH-83 extendable boom forklift. He gave the proper signal to roll the forks back & did not realize that he had his hand in the pinch point until the injury occurred. The index, middle & ring fingers were cut & middle finger required 5 stitches & index 1. He was released on light duty.
Employee was bent over at the waist using an impact to remove bolts on the cutting edges on a rubber tire dozer.
Employee was removing his chock when he twisted his right knee. Medical treatment was sought and he was placed on restricted work.
Worker performing finish grade activities in order to prepare subgrade for liner crew. In the process of removing a rock from the prepared subgrade, the worker ""strained"" his back during the act of lifting (bending over at the waist). Worker reported the injury to the Project Supt. om 8/18/06 and chose to seek treatment the same day.
Employee states tube slipped as he was lowering same into rod. His hand was pinched between the rod and landing gear.
He was carrying 6"" plastic pipe on his shoulder, when the wind caught them, and he went to adjust his footing and stepped on a rock and rolled his ankle.
Employee was working on a scissor deck. While trying to install utilities the employee twisted wrong and felt a twinge in his back. Employee finished shift and went to the chiropractor next day.
Employee was cutting 3/4 chamfer with skill saw, saw bound and kicked back cutting ee's right knee.
Employee was driving haul truck and hit a berm. Employee sought medical treatment and was placed on restricted duty.
Employee was struck on the shoulder by a 4"" metal ball that fell from a conveyor belt magnet discharge chute.
Employee was installing structual iron deck, it hung up on mounting bolts, He was prying into place and caught hand under the iron.
Shovel was loading haul truck on the blind side when a 3 foot (approx) rock rolled out of the shovel and struck the bed of the truck behind the cab causing the truck to bounce. This caused the employee of the truck to jar his neck and bang his kness on the steering column.
Employee was stepping off of LT018 and twisted his ankle. Medical treatment was sought and he was placed on restricted duty.
Employee was walking from the drill to the truck. He stepped on a rock and twisted his right knee and felt a little pain.
While descending from the rear deck of a Jumbo Drill, the employee's foot entered a small opening in the deck. This caused the employee to temporarily lose his balance and fall upright 3 feet to the concrete flooring. This resulted in the employee landing on the instep of his left leg, damaging the tendons in his knee.
Walking down stairs and missed the second to last stair.
Hit rock in roadway and jarred arm dislocating left shoulder while traveling the ramp in a haul truck.
Employee was moving a bin of samples using a pallet jack. The forks of the pallet jack got stuck on something. She was pulling on the handle of the pallet jack when it came loose, she let go of the handle and it hit her in the chest.
While crawling under a Cat 777 haul truck with a tire bead breaker, injured bumped his left knee on a rock under the rear axle.
Employee was laying on the grating outside of the chute and reaching into the chute for conveyor #6 belt assisting co-worker. Employee used his left arm on the scraper mount to support and stabalize himself while he lifted the scarper blade with his right arm. While he was doing this he felt a pain in his left forearm.
The employee was descending some metal grating type stairs, in wet conditions, when his right foot slipped causing him to twist his left knee. The doctor diagnosed him as having a torn medial collateral ligament.
Employee fell while getting off a tractor hitting and cutting his knee requiring three sutures.
Employee was loading core sample bags into the dryers and after about five hours she felt a strain to her right pinky finger. She was seen by a doctor on 04/14/06 and was put on restricted duty until 04/21/06.
While climbing on truck EE felt pain in lower back. He was treated and placed on restricted duty.
LIFTING 25 LB. BOXES ON A CART & ELBOW FELT LIKE IT PULLED OUT OF THE SOCKET. POST ACCIDENT DRUG TEST RESULTS WERE NEGATIVE.
While fabricating machine guards for mill, employee pinched finger between two guards breaking the tip of the bone on the right middle finger.
Crew was tripping out of the hole when breakout wrench slipped off of pipe and kicked back. The wrench struck employee in left thumb and fractured the bone.
Bent over to pick up a pallet and felt pain in her back.
Employee was pulling on a bull hose that was stuck in the mud. The hose suddenly released reulting in a lower back strain. The employee was on restricted duty for 4 days. His first Dr. visit was 03/06/06 and was released for full duty on 03/12/06
Employee states (while setting casing) The drill shifted allowing the blocks to fall from the face. As he held the blocks to reset feed frame, his thumb was crushed between the foot plate and the board.
Employee was helping install a guard on some drive belts of a pump and motor. He was holding the guard in place and felt a twinge in his lower back and side. The employee was put on light duty on 03/15/2006.
Stepping off a tractor the employee felt his foot pop. He worked for a while but his foot was hurting so he had it checked and it was broken.
As employee was walking to the rear of a lube truck, he slipped on a rock that was hidden under the snow and twisted his ankle.
Operating a haul truck when she struck her head on top of the cab after hitting a bump. Company was told about the restricted duty until the 2/7/06.
Employee was installing a turbo on a haul truck when he slipped and struck his knee against the engine. Medical treatment was sought on 01/24/06 and employee was placed on restricted work.

Employee slipped on grating while going from roughers to catwalk. He was taken to a medical facility and received stitches for his cut thigh. He was placed on light duty.
Emploee was removing a 5 gallon propane bottle from the back of a flat bed pick-up. He felt a strain in his arm when he set the bottle down on the ground. (This did not become reportable until 1/24/06)
Employee slipped while trying to adjust a mirror and hurt her knee. She was taken to a medical facility and placed on light duty. A follow up with Doctor was completed on 1-5-06. Employee was left on light duty at that time. Another followup is scheduled for 1-13-06.
Employee was leaving the scraper cut. He went over uneven ground which caused the scraper to bounce, resulting in a lower back injury.
The employee was attempting to unplug a shot crete discharge hose, the employee had pressured up the hose doing so, the hose had flexed upward striking him in the forehead and nose. There was NO hose connection in this to warrant a whip check or restraint.
Employee states while putting away stock in warehouse, she lifted three pieces of steel and felt a pop on her left wrist, resulting in a sprain of the left wrist.
Employee states he stepped off of a service truck onto a rock causing him to ""twist"" his ankle resulting in a sprain/strain.
Employee was changing small pump when it slipped and pinched his pinky finger. Employee received sutures to close wound.
The wind caught the door of a track drill while the employee was exiting the equipment and caught his left ""little"" finger in the door of drill. He was wearing leather gloves at the time. Employee was taken to a medical facility, received 5 stiches, and returned to work.
Employee was checking oil level on rockbreaker, when he felt something in his eye. He was taken to a medical facility, given medication for the eye, and released to full duty.
Operating haul when the truck hit the rib causing her head to strike the ROPS resulting in four sutures.
While loading a bolt into the carousel of the bolter, the bolt slipped and the plate hit the right forearm. Laceration with 7 sutures.
Employee was walking by a counter in the warehouse carrying a package of chop saw blades when his left ring finger hit the counter ledge, lacerating the knuckle. He received stitches and was released to full duty.
While pulling hose from CIL containment tank, employee received contact burn from lime after stepping into containment tank and getting lime into work boots.
While performing a PM on a locked-out de-energized mixer, employee was hand adjusting the paddles and got caught between the pulley and belt. 10 sutures and a fracture to the right distal bone of the pinky.
While moving a bucket on a pallet of backhoe teeth, the metal banding snapped back and lacerated the right hand. 5 Sutures. Full-Duty release. MSHA Reportable.
normal duty: EE caught his hand between the tool cart and tray while handling his tools.
While traveling down ramp with loader object entered right eye. Doctor removed embedded object.
The acid tails pipeline was plugged and EE was attempting to clear it. He attempted to remove one (1) of four (4) bolts from a six inch coupling, which was holding two sections of the line together, while the line was pressurized with compressed air. The line came apart striking EE on the head and knocking him to the ground.
Moved wrong with a sample bag in my hands.
Worker was exiting the tractor portion of a ""belly dump"" and in the process stepped on a rock, twisting his right ankle. The worker reported the injury to the Project Supt. on 8/28/06, but chose not to seek treatment. On 8/31/06 employee stated he wanted to seek treatment and physician placed a splint on the injured area, but did not restrict workers duty.
Employee was climbing off the welding truck when he lost his balance. He went to catch himself and grabbed a sharp edge. This caused a laceration to his finger. He recieved 8 sutures to close the wound.
Tech was sliding fork over on the fork lift when the forks were binding up resulting in a smashed finger and laceration which required stitching
While scraping muck out of a hoist cylinder boss, employees hand connected with a metal edge. The employee was taken in for medical treatment and was given 6 sutures.
Employee was using a utility knife to cut a plastic wire tie. As the knife went through the tie, the momentum carried and he cut his left hand above the thumb. Medical treatment was sought, and employee received four sutures.
While descending a ladder, employee lost his footing causing him to land awkward, twisting his left ankle. Light duty & prescription.
Employee removed ash tray from motor grader seat pedestal and reached arm in through opening and contacted air condtioner fan blade causing avulsion and fracture to tip of right middle finger.
The claimant was sweeping off a welding table, when he grabbed a pipe stub, lacerating his right ring finger.
Employee received laceration on right index finger when it was caught in rigging. Employee was helping rig forklift to be lifted by crane from one level of mill to another. Laceration required stitches.
Employee was fabricating int he u/g shop while grinding he got a flake of metal embedded in his right eye. He was taken to NENRH where it was removed. A full duty release was given.
Changing chipper point on small air chipper. Control trigger was hit with foot causing chipper to turn on. Chipper point was ejected from tool hitting employee on the nose. Cut on nose required six stitches to close.
Employee fell and cut his thumb while exiting a doorway on the shovel. He was taken to a medical facility and received stitches. Employee was released to full duty.
Employee was trying to get pipt fitting off of pipe, hit pipe off concrete. Pipe bounced up and hit him in the mouth.
The employee was holding on to the edge of the door of a pickup speaking with the driver of another pickup who also had his pickup door open. A gust of wind blew catching one door blowing it into the other door. The employee's pinky finger was caught between the two doors. The employee suffered a laceration which required stitching.
Stepping upon concrete pad leg slipped in mud.
Filling sample bags bent over to shovel and back started to get sharp pains. It was hard to walk or sit.
Employee was using a 2# hammer and screwdriver attempting to unplug lime silo feeder. Hammer bounced of end of screwdriver, striking employee's right thumb against housing on feeder.
Employee caught his finger in a pin lock while installing a bed cable.
Employee was backing up his loader when it hit the rib of the drift. When the loader came to a stop he struck his face on the instrument panel. He has four broken teeth and a laceration to his upper lip requiring 5 sutures.
Employee stated while removing ladder to access scaffold, he accidently bumped scaffold jack with ladder, causing scaffold jack to dislodge. Scaffold jack fell and struck employee on left forearm, causing laceration.
Employee was holding the lid of a powder box open on a mobile powder truck. Another employee in the manbasket swung the basket to the side pinching the injured employees left hand between the boom and the door of the powder box. There was 14 sutures placed in the laceration and he was released full duty.
Employee was attempting to install a pump sleeve on a pump barrel. The barrel slipped and pinched his little finger on his right hand between the sleeve and the pump housing. The injury required eight sutures to repair and the employee was returned to work full duty.
Crew was adding a piece of washover casing on an angle hole. Employee's left middle finger was pinched between casing and derrick. Fracture to tip of finger. x-ray, splint, and return to work
When moving bolt bin into place, finger was wedged between wall and bolt bin. Right 5th finger was cut. Cut required 6 stiches to close cut.
Employee was picking up a piece of casing from a pile. The pile shifted slightly, and his right middle finger was pinched between 2 pieces of casing. The finger tip from the nail to the end was lacerated and required several stitches.
Employee was in the process of installing ground support. He was putting a 4' X 8"" screen on the bolter when he accidently knocked some loose ground down with the screen. A sharp rock struck him on the right forearm causing a laceration which required 5 sutures to close.
Employee was removing a pin from the ripper frame on a dozer & pinched the palm of his left hand causing a laceration. This required 12 sutures to close.
Employee was walking in a tight area between a mantrip vehicle and the rib of the drift. There was a sharp edge on the fuel tank. As he was walking by he ran his left forearm over the burr which caused a laceration. The laceration requied 21 sutures and he was released full duty for his next shift.
While cutting a bolt with a torch, a piece of slag ran down sleave of coveralls causing a quarter size 3rd degree burn on right forearm.
EE tripped over 3' waterhose after filling water tank straining his upper back. EE received vicoden from Dr making this reportable.
Employee was attempting to open a push button door handle on a grader when he dislocated his thumb. Medical treatment was sought and employee was released to full duty. He was placed in a splint.
Employee had opened the door of a tool box that is mounted on the side of a flat bed pick-up. The truck was close to a berm and when the employee stepped on the berm his feet slipped and he fell into the door of the tool box. Original Xrays were negative. He went back on 02/21/06 and a cat scan showed two cracked ribs. This makes this reportable.
Employee was waiting for a sample to come off of dryer with her eye protection and PPE on and as the sample fell off conveyor belt she was showered with dirt and dust and some went into her right eye.
Employee was in the process of cleaning out the cab of his UG haul truck. He went to throw garbage away and lacerated his left index finger on a sharp edge of the garbage can lid. He was transported to the hospital and has four (4) sutures placed in his finger.
Employee was hammering in a piece of rebar when she missed and struck her finger against the rebar. Medical treatment was sought and sutures were used to close the wound.
Employee was attempting to connect a camlock to a water truck. The connection slipped of the truck and struck the employee on the bridge of the nose, causing a fracture.
Employee began work on 5/16/06. 5/19/06 employee reported areas around his eyes being irritated from his safety glasses and was given a different type of safety glasses. 5/22/06, employee sought medical attention and was diagnosed with contact dermatitis resulting from an allergic reaction to the safety glasses.
At approximently 3:30 pm on June 30th supervisor was on the 4020 level west sump area. He noticed a mist coming from the acid wash totes in the back of the drift. When he entered the wash bay he noticed a mild throat and eye irritation and became nauseous. He was examined by a Dr. and given a full duty release.
