Employee pinched off 3/8 inch off tips of middle, index, and pinky finger. Employee violated company's lock out tagout policy and reached hand into rotary valve that was in operation. Contact with valve caused injury.

ABRASION TO ARM REACHING FOR BAG, WHEN IN CONTACT WITH A PACKAGING CONVEYOR BELT, HE IS WORKING RESTRICTED DUTY.

BAGS WERE TOO FULL AT THE TIME AND EE WAS TURNING TO SPEAK WITH THE DUMPER OPERATOR AND WAS NOT WATCHING WHEN HIS THUMB ENTERED THE SEWING MACHINE IN A FEED.

Struck against a moving object

Employee received minor back strain while operating wheeled front-end loader. Employee stated that seat shifted when entering the stockpile.

Over-exertion in lifting objects

Employee was restacking pallets when he felt a pain in his lower groin. Treating physician diagnoised a strained groin muscle and placed employee on 3 weeks of restricted duty until condition improves. The employee lost no time.

Struck by powered moving object

Forklift had plastic caught on forklift, forkilft traveling past ee and it caught ee's leg causing him to fall down.
