Attempting to remove bolt with wrench from the coverplate on Wms Mill sump pump, the wrench slipped which caused employee's weight to shift to right shoulder. The pain was not evident at first but got worst during the night. Dr suspected rotator cuff tear, the MRI results confirmed on 10/30/08 (Dr diagnosis). Employee opted to physical theraphy vs seeing orthorpedic doctor.

EE was hepling to remove a mill roll attached to crane ball & positioned half way on platform. They tried to push the roll it begain to turn clockwise. He tried to move but roll hit him on his left leg & left foot. Bruised left leg & foot. Knee swollen but there is no evidence of fracture, dislocation,or subluxation. He is on light duty and scheduled for knee MRI.
He was climbing ladder to disintegrator to clean out chute. Stepped on platform, feet shot out from under him causing him to fall 5 to 6 feet. He landed on his back, on concrete. Diagnosis: left rotator cuff tendinitis. No work for 1 week.
EE was carrying a 5hp electrical motor for replacement on a piece of equipment. He felt a dull pain in the right side groin area and did not initially want to see doctor. Pain did not subside, so on 12 11 08 the employee decided he wanted to see a doctor. He is on light duty with no lifting more than 5lbs. Recheck on 12 19 08 - hernia.
Employee suffered a same level fall, he tried to extend arms to brace against fall. His injury was bilateral elbow fractures. Employee expected to have surgery Monday Nov 17, 2008.
Incident was not reported to HR in a timely manner by employee. Power outage occurred from bad weather conditions in August of 08. DC fan shut down causing bad visibility. EE bumped into a support beam injuring left arm. Several conflict of appts., finally met with orthopedic Dr on 11-6-08 and he gave him a cortisone shot with limited use of left arm until next appt.
Roderick was driving forklift moving a pallet when dust entered left eye. He was wearing safety glasses. Diagnosis: scleral abrasion and medication was prescribed. He had restrictions, but is back to normal duties.
Two employees were walking by R/R tracks checking to see if railcar was empty when one employee stepped on a rock. He twisted & sprained his left ankle. Became a reportable Aug. 13th when doctor recommended physical therapy.
EE was carrying an air cylinder on his shoulder. He lowered cylinder to work bench and right index finger got caught between cylinder and work bench. Laceration requiring sutures. Limited use of hand. No lost time.
Employee had a prior incident on 2/27/08 that has now become a reportable on 10/1/08. In Feb 08 he was pushing a lever on a hopper when he felt a pop in his right elbow. He was complaining of pain so he was sent to the Dr on 10/1/08 and diagnosed was tendernitis in the right elbow with restrictions of no lifting pushing or pulling greater than 10lbs with the right arm.
Employee was walking from breakroom going to catch a product sample and slipped and fell in the travelway (rained during the night). He sustained an avulsion fracture to his left wrist. He is working with restrictions and wearing a splint.
Adjusted lower limit switch for carriage travel on Lantech Wrapper. Was moving carriage into home position and tension roller caught hand pulling it into frame of carriage cutting his right hand pinky & ring fingers. Required sutures. No lost time, just restricted.
Employee was using a razor knife with left hand cutting stretch wrap from a pallet & cut his right index finger. Required several sutures. No restrictions, returned to normal duties.
Ridgeway was cleaning a dust line and dust got into his right eye even though he had safety glasses. He flushed the dust out of his eye. It was irritated, so he was sent to the Dr. He was treated for conjunctivitous in the right eye and given eye drops for seven days. no restrictions.
EE reported experiencing some pain in right wrist while performing her duties: stapling cardboard on walls, hand brake wheel turning and moving bulkhead door on railcars. Dr diagnosed tendonitis on 10-22-08.
Injured was originally working manual and semi-automatic equipment in a rotating environment in his home pkg dept.> then he was tranferred to another manual pkging line job, to sew bags with little or no rotating of the sewing tsks. The new job required him lift to up to 50lbs bags repetively throughout the months causing him to have soreness in hands and rt shoulder.
She has to perform lots of computer work using keyboard and mouse with right hand, took computer home and continued with use of right hand,complaining of pain on top and bottom of right wrist, pain going down right wrist to elbow,diagnosis mild tendinitis due to over use syndrome
