Caught in, under or between a moving and a stationary object

Was assisting bags loading into load to tilt chamber of OMW. He opened guard reached into chamber and the tilt chamber came down catching his right arm between tilt chamber and conveyor. Laceration requiring 4 sutures. No work for 1 day.

EE was loading a bulk truck; when for reasons unknown, the driver removed wheel chucks & drove away from loading point without being instructed to do so by EE. The retractable lanyard swung EE off truck. EE's main complaint was pain in areas consistant with full body harness.

Stepping down on step ladder, lost her balance & twisted left knee. Inflammation of left knee. Restricted, wear immobilizer, no work for 4 days.

Removing roll up door spool. When he cut door support loose it struck him on top of left arm & wrist. Sustained left arm & wrist fracture. Taken to ER & was admitted.

EE attempting to sling motor onto loadall forks. Sling slid along forks pinching left ring finger between motor & handrail. Laceration required sutures. Restrictions, limited use of left hand, keep dressing clean & dry. Recheck in 1 week.

EE was transferring powder. Disconnected hose & was looking inside hose when suddenly powder started shooting out of hose. Right eye corneal abrasion requiring prescription.
