Caught in, under or between running or meshing objects

Employee opened a door, reached into equipment with right hand & mill rollers grabbed glove pulling hand into mill damaging multiple fingers. EE was transported to Emergency Room. Three fingers were amputated and one finger was sutured.

Bodily reaction, (Not Elsewhere Classified)

EE noticed soreness/pain around pre-existing injury, torn right Achilles tendon, 11-20-18. EE reports inflammation of said injury due to using stairs to access a platform. EE reported injury 11-26-18 and medical treatment at ER & Orthopedic doctor. Diagnosis - tendon not torn, no surgery needed, tendon simply needs to heal. Cast/walking boot & crutches provided.

Fall to the walkway or working surface

Dismounted loader to cut off conveyor at stop button panel & slipped in mud, having same level fall. Left knee hurting, so taken to ER. Diagnosis left knee pain - given ace bandage, medication & told to elevate & rest. Released for 11-7-18.

Over-exertion in lifting objects

Temporary worker attempted to lift a case & stack it on a pallet when EE felt a sharp pain in EE's back. Diagnosis: lower thoracic pain & spasm. Restrictions of may lift up to 10 lbs, no twisting & bending.
