Caught in, under or between running or meshing objects

EE was removing paper on pulley, belts running. Belt caught glove pulling it between 2 pulleys causing a laceration & hair line fracture to left hand 2nd finger tip. Required sutures & restrictions of limited use of left hand until 3-4-13 then, keep dressing clean & dry.

Struck by falling object

Lowering spacer bars off of Wms Mill 1 platform to another EE at ground level. EE lowering bar hands slipped, caught bar weight on left thigh when EE on ground pulled bar. Burr on bar cut thigh. Required sutures & tetanus. EE did not notice burr & did not make accommodation for wet conditions.

Fall to the walkway or working surface

EE had just applied LVM on slippery area by OMW & was leaving area when he slipped and fell.He tried to catch himself with rt arm injuring his rt shoulder & arm. Diagnosis: rt shoulder and arm contusion. Light duty until next week.
