Caught in, under or between a moving and a stationary object

Employee attempted to reposition a mobile compressor without assistance. Lost control and ran compressor into a stack of wooden pallets, pinching right index finger between the pintle hitch on the compressor and the stack of pallets.

Struck against a moving object

CLEANING UP WITH BOBCAT, PICKED UP HEAVY DISK, TRIED TO ARRANGE DISK ON BOBCAT AND INJURED LOWER BACK. (HE IS STILL WORKING BASED ON HIS WORK RESTRICTIONS.)

Over-exertion in pulling or pushing objects

Pulling piece of pipe from rack and injured shoulder.

EE slipped and fell to his right knee when rigging slipped. This caused a spain to his knee. EE improperly rigged cable-come-a-long to beam. Rigging slipped causing him to lose his balance.

She was trying to unclog suction cup on Bemis Bagger. She reached her left hand in to remove green filter from suction cup while machine was still running. Her wrist was squeezed causing a bruise to her wrist.

Struck against stationary object

Employee bumped left knee on steering wheel when he entered the enclosed cab of a front-end wheel loader. Physician prescribed 7 days of restricted duty.

Struck against a moving object

Cleaning railroad tracks with a Bobcat and hit tracks with bucket on Bobcat. The impact is what hurt his left shoulder. Did not loose any time from work and no restricted duty. Shoulder still hurting, so doctor sent him for physical therapy starting 8-16-06.

PALLET WAS JAMMED, WERE UNJAMMING THE PALLET AND THE PHOTO-EYE ACTIVATED THE SQUARING CYLINDER. EE'S FINGER GOT CAUGHT BETWEEN PUSH PLATE AND PALLET.

Over-exertion (Not Elsewhere Classified)

Employee on pkging line placing empty jugs into position. Complained of tingling and numbness in her arms and hands. Thought it was a family history of heart condition and/or her personal diagnosis of lupus and sought personal medical treatment. Investigation determined it was work related. Tested and diagnosed with bilateral carpal tunnel syndrome.
