Walking and tripped on strap & debris fell on left knee bumping it & head(hard hat on) on pallet of empty bulk bags. Inflammation in neck & soreness in left shoulder and knee. Able to work with restrictions.
Performing maintenance on bulk loading spout. Adjusted cable & when spout began traveling up cable, the attachment came apart & eye bolt scraped right middle finger, causing laceration. Required 4 sutures and restricted with no use of right hand.
Climbed up platform to clean high bin eyes, reached under light conduit, when he felt sharp pain in lower back. Diagnosis low back strain and has restrictions.
Preparing to load bulk truck, picked up controller by cord, with right hand and received shock. Let go of cord & arm was numb. Diagnosed as electrical burn on right hand. No climbing or heavy lifting with right arm.
Pushed emergency stop to clear jam,then pushed e stop & restart. Had hand on machine & tape head came forward pinching right middle finger. Partial amputation & skin graft of right middle finger. Restricted no lost time.
Bending over to lift empty boxes felt sharp pain in lower back. Diagnosis hip flexor muscle strain. Light duty.
Turning conduit and felt a sharp pain run through his right wrist. Also, connecting hoses and felt sharper pain in same area of same wrist. Diagnosis: wrist ligament/tendon strain with lifting restriction.
Forklift driver pushed conveyor into EE's right leg was caught in between the seat he was sitting at and the conveyor.
Backing skid steer and hit a front end loader. Diagnosis mild cervical and lumbar strain with no restrictions. Sent back to doctor with mild pain on 4-22-10 and was referred for physical therapy. Became reportable on 4-22-10.
While in route to grab a leaking bag, he had to go around employee in pathway & struck left hand & wrist on metal frame. Hand contusion & wrist sprain. Restricted.
Thawing frozen lines & changing motor. While changing motor, left foot slipped. Okay that day, but next day foot swollen & hurting. Left ankle sprain, grade 2. Restricted with air brace & crutches.
Removing a stuck pallet. Employee notified supervisor on 9-10-10 that he had been diagnosed with a hernia on 9-1-10. He said it was caused by an injury on 7-30-10, which injury had not been reported to the company.
Removing belts using pry bar when pry bar slipped and twisted his left arm. Tendonitis of left elbow no restrictions. Become reportable injury 4/29/10 when doctor gave him cortisone shot.
