Struck by... (Not Elsewhere Classified)

EE bending down picking up cardboard and forklift carrying pallet struck her right hip & elbow causing same level fall. EE didn't want to see doctor. After 2 days had soreness & requested to see doctor. Diagnosis: body trauma, no fractures or broken bones. This is lost time due to medication prescribed and not the actual injury.

Struck against stationary object

while working with a jackhammer, came in contact with rebar and cut finger

Over-exertion in lifting objects

Employee was bending and lifting material during the work day.

EE cutting straps off bundle of bulk bags when box cutter slipped up the strap cutting his left index finger. Required 4 sutures and EE was released with no restrictions.

Caught in, under or between a moving and a stationary object

Employee was moving forks together on a forklift. Hand slipped while moving fork over, fork came down and pinched employees right thumb causing a laceration.
