Mine experienced a rock fall measuring approximately 20' high x 20'-22' wide x 45' long.

The #3 left pillar was being mined and the last run was being taken when a piece of rock measuring approximately 2' thick fell on the head of the miner, causing the miner to get stuck. Then a piece of rock approximately 4' thick x 20' wide fell on the miner causing it to be wedged between the roof and the bottom.

A piece of rock fell in break, one break above 42 switch end. The rock fell to the edge of track. The rock was approximately 12"" sandstone, 24"" slate and measured approximately 7.5' wide x 10' long x 36"" thick. The rock fall pulled 2 roof bolts.

An unplanned face ignition occurred in the #1 right break on 002 section.

Roof fall occurred in #3 entry on the #1 North Main 8 breaks inby #5 head drive. The fall measured approximately 100-120' long, 20' wide and 5-8' thick.

An unplanned rock fall occurred in the #5 entry of the #5 left panel, 8 breaks inby the section headdrive located off the #3 Left Sub-Mains off the #3 West Sub-Mains. Miner was extracting coal on 1st run on the right wall cut when intersection fell in and rode thru the breaker timbers. Rock fall measured approximately 40'x20'x42"".

A roof fall occurred in the #6 track entry 10 breaks inby the empty loop switch. Rock fall measured approx. 16'x20'x5'.

A rock fall occurred in the #2 entry of the South Mains located between break #44 & 45 inby the No.4 head drive. The fall measured 20'x20'x5', the area had been bolted with 5' resin bolts and driven in September 1994.

A rock fall occurred on 001 section approximately 9 breaks from the face in a gob eye in the solid wall of the #5 entry. The rock fall measured approximately 15'x18'x40"".

Employee found rock fall while conducting preshift examination. Rock fall had occurred in the #3 entry of the #3 West Submain between break 25 & 26 from the #7 Headdrive which is located in the #4 entry of the #1 North Mains. Rock fall measured approximately 20' x 30' x 6'.

Caught in, under or between a moving and a stationary object

The miner was mining in 3 right pillar, made 2nd run and backed out to timber off when employee started yelling, co-workers ran back to him they found the mbc track was on top of employee's left foot. Accident resulted in fractures.

Struck by falling object

Employee was removing block from brattice, he began to get up, twisted his knee and the block fell down and struck his left knee. Employee later required surgery. Employee started missing 10-17-05.

Struck by rolling or sliding object

Employee was restocking miner cable in junction box when a slab of draw rock rolled from the rib, striking him on the right leg/foot/ankle resulting in a fracture requiring surgery 08-19-05. Employee started missing 08-18-05.

Over-exertion (Not Elsewhere Classified)

Employee was bolting with single head roof bolter when his right knee started to hurt and swell. His knee has been operated on before, approx. 5-6 years ago. Employee started missing 08-11-05.

Struck by falling object

The pin on the truck end of the duster worked out, allowing the duster to set down on the wheels. The duster was being jacked up to see what needed to be done to correct the problem when the jack creeled over and the duster struck the employee on his left foot resulting in a fracture.

Caught in, under or between a moving and a stationary object

Employee was hanging cable in the #4 entry on right side of bolter, with back turned away from bolter. Coworker was backing bolter out of the #4 entry and watching the cable on the left side of the bolter. Coworker never saw employee and backed over his foot and lower leg.

Struck against a moving object

Employee was tramming ram car down roadway when he hit a hole in the roadway and felt pain in back. Employee started missing 06-25-05.

Struck by falling object

Foreman was two rows of timbers back when miner was extracting coal (first cut) on right wall in the #5 entry when the intersection fell in and rode thru the breaker timbers catching his left leg.

Fall onto or against objects

Employee was working to remove a dryer lid when he lost his balance falling approximately 4-5 feet and fractured his right wrist. Employee later required surgery. Employee started missing 8-9-05.

Struck by falling object

Employee was tramming single head bolter out of #2 heading when a piece of draw rock fell from between roof bolts striking employee on right foot, resulting in a fracture.

Struck by powered moving object

Employee was struck by a scoop while working on another scoop which was stuck/broke down in an intersection. Employee was down in the deck of scoop #1 exchanging a hydraulic hose and had his feet sticking out when another scoop came along and struck his left foot (heel) requiring approximately 30 stitches.

Employee was drilling hole for roof bolt when his glove became entangled with the drill steel, winding his left index and middle fingers around the drill steel.

Over-exertion (Not Elsewhere Classified)

Employee was unloading rollers from the supply car and as he turned to throw a roller he felt sharp pain in lower back. Employee started missing 02-22-05.

Struck by falling object

Employee was tramming miner across section when rock fell off miner striking employee on left ankle resulting in a fracture.

Employees were jacking a diesel motor which had de-railed back on to the track. When they slewed the motor over, it pushed a flat car off the track catching the right foot of another employee.

Struck by rolling or sliding object

Employee was delivering bolt supplies to the bolter in the #2 entry. When he got off and walked around scoop to unload supplies, a small rib roll occurred (bottom of the rib flaked off) and struck employee in left foot/ankle area. Material measured 2""x11""x14"" and weighed approximately 3-6lbs. Employee started missing 05-16-05.

Over-exertion (Not Elsewhere Classified)

Employee was walking down slope and stepped in mud and twisted left knee. Note: this is a re-occurrance of an old injury which occurred on 11-21-03. Employee started missing 06-20-05.

Over-exertion in pulling or pushing objects

While using a hammer to pry a top roller loose from belt structure (de-belting), employee experienced pain in lower stomach resulting in employee having to have hernia surgery. Employee started missing 04-27-05.

Over-exertion in lifting objects

Employee was lifting a bag of rockdust when he felt pain in his lower back. Employee started missing 03-08-05.

Struck by flying object

Employee was helping make a belt set-up and was operating the front carrier. As the miner was pulling the lolo up to tighten the belt the connecting link being used to connect the chain together broke, and a piece of the link hit employee in the forehead resulting in (8) eight stitches.

Employee was cutting beltline when knife cut through beltline, resulting in a laceration to his left arm/wrist requiring 4 stitches.

Employee was cutting belt when knife cut through belt and cut his right leg, resulting in 16 stitches.

While pulling cables back for power move, employee was using hammer to restrain rope in scoop bucket, hammer slipped through hole in scoop bucket mashing index finger on left hand. (Resulted in stitches)

Employee was splicing belt when his knife slipped cutting his right index finger resulting in 3 stitches.

Struck by falling object

Employee was installing roof bolt in hole and had push part of the bolt up into the hole when he dropped head of bolter down to finish pushing bolt up, the bolt fell back out of the hole striking employee on his left index finger, resulting in a fracture to the end of his finger.

Struck against stationary object

Employee was in the process of hooking the charger up for the battery buggy which had to be put on charge and as he bent over to pick up the charger cord, he caught the corner of his right cheek/eyelid in a cable hanger that was hanging down, requiring him to have stitches.

Employee was changing a hydraulic hose on plunger jack of ram car. He was pushing hoses back into brackets that hold them in place, used hammer to knock them in place when he struck finger. Accident resulted in a hairline fracture to his index finger on his left hand.

Struck against stationary object

Employee was attempting to break a plug loose on a pressure pump when the pipe wrench slipped and the employee struck his hand against the pump fracturing his ring finger on his left hand.

Caught in, under or between a moving and a stationary object

Employee was tightening a nut on a bolt with a rachet while someone else was holding the head of the bolt with a wrench. The rachet slipped off and caught employees hand between rachet and floor resulting in a fractured middle finger on his right hand.
