Rock fall occurred in break 4 off 2 left off 1 West Mains in the #4 entry. Fall measured approximately 20' wide x 60' long x 7' thick.

Employee tripped over a rock and fell striking his shoulder on the mine floor. Employee started missing 03-14-11.
Employee was installing a draw head on his RRD motor before coupling to the rock duster. A second supply motor was coming up the track and the operator did not notice that the switch was thrown toward the side track where the employee was working. The second motor hit the RRD motor and knocked it into employee.
Employee was bolting and when he reached to get the steel out of the roof he sprained his shoulder. Employee started missing 02-04-11.
Employee walked under an overcast and he raised up too soon and hit the last I beam. Employee started missing 04-18-11.
Employee and another were lifting a motor with a bar and later complained of back pain.
Employee was bolting top when a piece of draw rock fell between rib and first bolt.
Employee was attempting to remove a double tire off ram car with a 3/4 ratchet and cheater bar. The ratchet slipped off and struck his left leg/knee. Employee started missing 01-25-11.
Employee was shoveling 3A belt when the rib rolled out and struck him on the left foot. Employee started missing 09-30-10.
Employee was walking beside bolt machine when he fell on his left foot.
Employee was scattering belt structure for a belt set-up and when he stood a belt stand up against the rib it rolled out and struck him on the right hand and neck.
Employee was walking through 3 right crosscut when rib rolled off and struck him on the lower right leg.
Employee was bolting on the operator side of the working place when the rib rolled out and struck him on the back and leg. Employee started missing 09-13-10.
Employee was going up track when his rail runner hit the guard with the canopy letting the guard fall down and pin him in the operators compartment.
Employee was replacing a hydraulic fitting when his foot slipped injuring his right knee. Employee later required treatment and started missing on 01/06/14.
Lotrac operator was going through scoop doors at the end of the track. Lotrack operator saw employee standing next to the door and when the operator went through the door he heard employee make a grunting noise. He then looked around and saw employee on top of the lotrac bent over. The lotrac operator had to back lotrac up so that the employee could get off the lotrac.
Employee encountered smoke coming from face drive. When he went to check it out, he got into the smoke and it caused him to have difficulty breathing.
Employee was making up setup belt, he was cutting a conveyor belt with a knife, the knife slipped cutting his right leg. Injury requiring stitches.
Employee was setting timbers, rock fell from rib to first bolt, rock struck employee on lower back and his ankles.
Employee was getting off loader stepped on a rock and twisted left knee. Started missing work on 07-19-10. Returned 07-26-10 light duty.
Employee was walking toward miner when he stepped on a rock and creeled his ankle and fell on his knee. Employee started missing on 07/19/10
Employee had his hand on top of the roof bolter canopy and was going to swing boom out when a piece of draw rock fell out and hit him on the hand.
Employee stated that he was walking down lolo when his lower back started hurting.
Employee was loading shear head jack when his foot slipped and he fell out of motor. Employee started missing 05-24-10.
Employee said his left knee had been hurting for the last two weeks. He stated that he didn't know what caused it but that he thought he hurt it working on an overcast and that he was on his knees a lot. Employee started missing 05-05-10.
Employee was walking through #2 right entry when a piece of waterline busted, knocked his feet out from under him, causing him to fall and twist his knee.
EE was laying track when he fell and hit his hand on a rail tie.
Employee got off slope car at bottom of slope. While he was walking he slipped and fell hurting his left knee.
Employee was pulling belt in head drive with a yellow rope. The rope broke and came through the deck and hit him in the eye.
Employee dropped rub rail off miner onto his left foot.
Employee was walking on a roadway when he bumped into the end of an installed torque tension bolt.
Employee was knocking the boom pin out of the miner when a piece of metal chipped off the pin and struck him on the wrist. Injury required stitches.
Employee was cutting end of dust hose off when he inadvertently cut himself.
Employee was trying to get the lo-trac started, when a piece of rock fell and struck his arm.
Employee was swinging the boom of roof bolter out to put up a rope bolt. As he was swinging the boom, his left hand was on the canopy and as a result his fingers were mashed between the canopy and the top.
EE placed his hand on a rock for support and he slipped cutting his left forearm on the rock. Laceration required stitches.
Employee was replacing dryer screen when his finger got caught between screen and wood block.
EE was installing rebar when his left hand was caught between #6 & #11 rebar causing a laceration that required stitches.
Employee tripped on hoses in the floor and put hand out to catch himself. Hand was cut on metal panning.
Employee was bolting top when a piece of draw rock fell and struck his right hand index finger.
Employee was changing tire on Lo Track. New tire was laying on rib/ He was lifting tire up when tire slipped and turned around mashing fingers against rock behind tire. Injury required stitches.
EE was cutting a piece of panning with side cutters and as he pulled the pliers to tear the panning his hand hit another piece of panning causing a laceration that required stitches on his left hand.
Employee was attempting to jack up a wrecked car when the car fell sideways and caused the bar to strike him across his nose.
Employee was cutting a piece of belt for skirt rubber to go in the dolly. Injury required 5 stitches.
Employee was turning a cutterhead motor when it rolled over and mashed his finger.Injury required stitches.
Employee got his right thumb caught between fender of scoop and tire. Injury required 13 stitches.
Employee was loading ram car jack in mantrip when the jack slipped and caught his finger.
Employee went to get a nail board that was leaning up on the rib when it fell and struck him on the leg. Injury required 11 stitches.
Employee had set face spads and was in the process of posting section, when a piece of draw rock measuring approximately 2' x 2' fell out between bolts, hitting him on the left hand.
Employee was attempting to tie beams to the top when a jack slipped out causing the beam to fall. As the beam fell, it struck him and caused him to fall back into the scoop bucket injuring his back.
Employee was attempting to get a head jack pin out with a sledge hammer. He missed the pin and struck his finger with the wooden handle.
Pneumoconiosis
This is an accumulative effect of many years of repetitive motion. Employee started missing 09-14-10.
