Tracking system was down due to a broken fiber optic cable.

Over-exertion in pulling or pushing objects

Employee was at the bottom of slope uncoupling the supply cars from the slope car. When he pulled the lever to uncouple the cars he felt a sharp pain in his lower/mid back and he had numbness in his left leg.

Employee was pulling on belt in the tandem roller when his feet slipped out from under him.

Employee put a regulator hole in brattice and when he turned around a piece of draw rock fell and struck him on his hard hat.

Employee was helping to get a 60"" top roller out of beltline, lifting it over their heads. The roller got stuck so they pulled it with a slate bar and when it came free it struck him on the knee. The roller was too close to the top to use a chain ratchet.

Employee was shoveling up a coal spill when he felt pain in his groin area.

Employee was bending and installing roof bolts in #5 heading, no problem, but when he left bolter to go to return bolter in #2 heading he felt pain in his groin and had trouble walking from place to place.

Employee was working on miner and ran into a bolt hanging down, he felt pain in his shoulders and neck area. Later he drove a lo-trac down the belt to put power in and hit a hole and the pain got worse. Employee started missing 10/01/15.

Employee was traveling down the roadway when he ran over a rib that had rolled out causing him to hit his head on the canopy. Employee started missing 08/24/15.

Employee hit his head on highline and felt pain in his neck. Employee started missing 03-05-15.

Over-exertion in lifting objects

Employee moved a blue tub to get a light charger for an employee when he felt pain in his back.

Fall to the walkway or working surface

Employee was assisting in pulling lolo up, it had cleared the corner of the intersection and he stepped over next to corner when the brow fell off the corner striking him around the waist/hip and knocking him down causing him to twist his knee. Employee started missing 06/01/2015.

Operator was pushing coal with dozer when a rock came out of the silo stacker tube and went through the side glass.

Employee stated that he was hit by a rock. An initial report was not filled out.

Fall from machine

Employee was tying a pump onto the car with ratchet binder when he slipped and fell off the side of the flat car, scraping his knee and shin bone. Injury required stitches.

Employee was throwing crib block under a low boy to unload a mobile bridge carrier and hit tongue on the supply motor with his left hand and ring finger.

Employee was struck by a water line being pulled by equipment.

Employee was straightening a bent bolt, put it in the head of the bolt machine and accidentally hit the rotation. The bolt had a burr that caught his glove and wrapped his hand around.
