Employee was backing carrier into #3 entry at tailpiece. As he was backing around tailpiece a piece of rock busted from the bottom striking him on his left foot. Injury ultimately required partial amputation of the injured foot.

Employee was lapping highline on a highline sled when he twisted his back. Employee started missing 10-22-14.

Over-exertion in lifting objects

Employee was loading belt structure when he felt pain in his lower back.

Struck by flying object

Grease fitting broke. Injected grease in right forearm.

Employee stepped off the tailpiece and twisted his left knee. Employee later required surgery and started missing 05-15-14.

Employee was pulling a knot out of the cable at the end of the lolo. Employee started missing 12-15-14.

Employee was running the continuous miner sitting on his knees and when he attempted to stand he felt pain in his knee.

Struck by falling object

Employee was setting timbers when a previously set timber was knocked over causing it to strike him on the back of his right leg. Employee started missing 03-03-14.

Employee was looping up belt. When he began to pull on the belt dog it slipped off the belt and struck him on the left forearm.

Over-exertion in lifting objects

Employee was loading bottom stands. Two stands were stuck together; as he pulled them apart and started lifting a stand up he felt pain in his lower back.

Fall to the walkway or working surface

Employee slipped and fell hurting his foot.

Struck by falling object

Employee was positioning bolter to strap kettle bottom, rock fell striking right hand. Injury required surgery.

Struck against stationary object

Employee was moving a roof bolt strap, while pulling one strap his wrist hit another strap cutting it. Injury required five stitches.

Employee walked into a bent roof bolt plate resulting in a laceration to his left shoulder. Injury required stitches.

Employee was getting a belt splice board off of the miner and hanging it up to the top so that it wouldn't get damaged. He hung one side up and it came around and hit him in the mouth.

Employee hit his left thumb with a hammer while trying to hit a cap wedge to install a timber. Injury required stitches.

Employee was loading the roof bolting machine when 4' straps got tangled and broke loose sliding into his forearm.

Employee misjudged edge of coal pile, dozer nose dived off coal pile. Front and right door glass broke, a piece hit his right leg. Injury required staples.

Employee was pulling a piece of loose draw rock when he pulled up slate bar hit him in the mouth. Chipped tooth.

Employee was putting roof bolt up, strap or plate knocked hard hat off and bolt hit him in top of head. Injury required three staples.
