Employee was putting a cover lid over head motor on a miner. As ee was putting it in place, there was a bolt under cover that ee hit finger on, causing to drop the cover on finger.

Employee was lifting a bucket jack up to put on a scoop. The jack slipped and fell against scoop and caught left index finger.

Employee was moving scoop tire with a slate bar, the bar slipped causing the employee to fall against the concrete.

Employee was helping to pull mechanism across the floor when the 3/4 inch rope came loose causing the employee to fall backwards into the frame of the fine refuge vibrator.

The employee was in the #3 right crosscut when a piece of rock had fallen, striking the employee on the right leg, just above ankle.

Employee picked up the miner cable to move it out of the way when EE felt pain in right shoulder.

Employee was walking from bottom of slope up to mantrip when EE hit head on the top causing pain in EE's neck, shoulder and back.

Employee picked up lolo tailpiece chain to move it out of the way when EE felt pain in back. Employee started missing 04/06/16.

Employee stated that a piece of draw rock fell and hit EE on the back.

Employee was getting a rock off of the miner. EE took a step and foot slipped on a piece of rock causing EE's left knee to twist.

Employee was walking to the head drive. EE walked beneath the highline and rose up before clearing it and jammed head. Employee started missing 02/16/16.

Employee was helping another employee load concrete block when EE aggravated hernia. Employee started missing 02/01/16.

Employee was bolting in the #3 left crosscut when a piece of rock fell off the canopy of the bolter, striking EE on the back of EE's leg.

Employee was lifting a drill pot out of a mantrip to load onto another mantrip, with assistance from another employee. When the assisting employee picked up one side of the drill pot shifting all the weight to the injured employees side and EE stated that felt a tearing and ripping sensation in left arm, shoulder, neck and back.

Employee was lifting hood of lo-trac when handle slipped from hand striking left ring finger.

Employee was lowering extension ladder when the ladder slipped and caught right thumb on the handrail.

Employee was struck in the face when a scrapper that was stuck suddenly dislodged.
