Employee reported his injury two days after the occurrence to the personnel department. He stated that he was helping to install a pump when he hurt his back.

Employee was installing a spot bolt when a rock fell, struck the drill pot then bounced striking the employee in the mouth.

Employee was preshifting when he found a curtain down. Employee strained his back while pulling the curtain. Employee started missing 07-02-12.

Employee was taking water line loose when water gushed out causing him to fall. Employee started missing 08/08/12.

Employee was stepping down from servicing the radiator of the dozer. He possibly slipped and the nozzle for the windshield washer penetrated his skin resulting in a laceration on his left arm.

Employee was lifting/throwing supplies when he felt pain in his back. Employee started missing 04-19-12.

Employee was throwing roof bolts when he twisted his back. Employee started missing 04-16-12.
