Employee was replacing pipe when a hand rail broke causing him to fall.

Employee was scattering belt structure when he experienced low back pain. Employee stated that this may be a reoccurrence of an old Army injury.

Employee was changing oil in raw coal viberator when he inadvertently stepped back off discharge end.

Employee was installing a spot bolt when a rock fell, struck the drill pot then bounced striking the employee in the mouth.

Employee was preshifting when he found a curtain down. Employee strained his back while pulling the curtain. Employee started missing 07-02-12.

Employee was taking water line loose when water gushed out causing him to fall. Employee started missing 08/08/12.

Employee was stepping down from servicing the radiator of the dozer. He possibly slipped and the nozzle for the windshield washer penetrated his skin resulting in a laceration on his left arm.

Employee was lifting/throwing supplies when he felt pain in his back. Employee started missing 04-19-12.

Employee was helping to build cribs when another employee threw a crib block and hit his hand. Employee required surgery 11-08-12.

Employee was trimming a belt when he stumbled and cut his leg with his belt knife.

Employee was setting timbers when slate bar flipped up and hit him on his right leg. Injury required stitches.

Employee was packing tools through mandoor when he slipped and tried to catch himself from falling. Injury required stitches.

Employee was removing an old tracking tag from his hard hat when he inadvertently cut his left thumb.

Employee was pushing against rail motor with lifting jack when jack came out catching his right hand against the rib. Resulting in a fracture.

Employee was repositioning a piece of metal to get punch back out when he got the end of his thumb caught on the guard on the iron worker.

Employee was attempting to splice belt when he inadvertently cut his right leg. Injury required stitches.

Employee was building a crib back, when a piece of draw rock struck him on the back of his right hand. Injury required stitches.

Employee was taking bolts out of legs on belt structure. When he moved the structure over his finger rubbed over a piece of rock cutting his finger. Injury required 3 stitches.

Employee was bolting a brow up when a piece of rock came down the drill steel, broke off and struck him on the left jaw.

Employee was struck by rub rail handle that broke as a result of being caught on tailpiece. Injury required stitches.

Employee was attempting to make a D-belt when he inadvertently cut the palm of his right hand. Injury required 4 stitches.

Employee was packing a roll of cable, slipped and fell hitting his right eye on the spool of cable.
