Rock fall occurred in the #1 entry in the first left panel off of 2nd right off 002 sections sub-mains at break #22 through #24. Fall measured approx. 20' wide x 60' long x 4'-5' thick.
Hoist was down due to two batteries going bad.

Rock fall occurred at break 37 in the 1 North Mains #1 entry on the overcast for 003 section intake and 001 section return. Fall measured approx. 20' wide x 90' long x 4'-6' thick.
Hoist would not operate in the down cycle, found a solenoid loose on hydraulic control block.
After completing the 4 right crosscut, miner backed out to tailpiece and the 4 right cut fell in pulling bolts out into the #4 entry intersection. The fall measured approx. 20' wide x 25' long x 2'-5' thick.
Rock fall occurred on the track 1 North Mains between break #24 and #25. Fall measured approx. 8' wide x 20' long x 2'-2.5' thick.
Hoist car was down due to main electrical breaker failed.
The directional solenoid valve malfunctioned and the slope car would not travel to the surface. The solenoid was replaced and the slope car was put back into operation.
Employee was cutting skirt board rubber and inadvertently cut his left leg.
Employee strained his back lifting two bags of rock dust.
Employee was twisting stopped up nozzle & it flew off striking him on the left hand/thumb. Employee started missing 08-21-07.
Employee cut off a steel pencil rod and when he came back the piece of rod in the rock punctured his right elbow. He received first aid treatment after the injury. The puncture wound healed and on August 17, 2007 his right elbow began to swell. He received an antibiotic shot and antibiotic pills and was admitted to the hospital on August 22, 2007.
Employee was putting power box in a break when the back of a scoop caught the end of a piece of track rail causing it to swing around and hit him on his left leg.
Employee and assistant started ground faulting breakers at 13 drive 750 box. The first breaker he went to ground fault was a 995 volt head drive spare; when employee went to test breaker it blew out burning both his hands and forearms.
Employee put his foot on bolter cable to keep cable reel from jerking cable down from the top. A loop of cable was hidden in the mud; the loop wrapped around his leg and twisted his knee. Employee started missing 04-17-07.
Employee was operating miner in #6 entry when the rib rolled off, striking him across the hip knocking him down and partially landing on his left leg. The rib roll was approx. 10' long x 3' wide x 3"" thick. This resulted in a fracture to his leg and ankle.
Employee was troubleshooting highline trouble when he came into contact with electricity. Employee started missing 02-10-07.
Employee opened air lock doors for 002 section and picked up tool box and turned to walk and felt pain in his right knee. Employee later required surgery and started missing 03-07-07.
Employee was welding on a pipe when his foot slipped off a large pipe he was standing on. He was using 100% fall protection with full-body harness, which stopped his fall, but he bumped his left leg on the pipe & suffered a strained back muscle.
Employee was demobing the shop when a piece of galvanized metal slipped from his hand causing a laceration on the left 2nd finger.
Employee was putting collar pipe in between legs of arch steel. One leg was set and the other leg was being set. When employee lay the pipe in, he had his right hand on the end of the pipe and the leg that was not set snap back in place which smashed his right hand between collar pipe and the arch steel crushing his hand and causing lacerations.
Employee was attempting to cut a piece of conduit to make a hammer holder for his belt when his knife slipped resulting in a laceration to his left hand. Injury required 4 stitches.
Employee was walking beside feeder, reached his hand across the sideboard and got a small piece of metal in his right hand. Injury resulted in 7 stitches.
Employee was getting rock out of the #8 belt tailpiece when he twisted his leg and foot.
Employee was walking toward roof bolter when he tripped and caught himself resulting in a hair line fracture to his left thumb.
Employee was bolting in #5 heading; as he reached to get a roof bolt a piece of rock fell between the rib and the 1st row of bolts striking his little finger on his right hand. Injury required 2 stitches.
A section of belt rail structure became lodged in the low structure and the employee had the carrier man back the dolly up to free the belt rail. When the dolly hit the belt rail the other end flew around and struck the employee in the left leg and knocked him down. The injury resulted in a hairline fracture to the lower leg.
Employee slipped on ice in the slope straining his right knee.
Employee was shoveling belt when a piece of draw rock fell and struck him in the face. Injury resulted in 19 stitches.
Employee cut metal band off bundle of roof bolt plates, then slipped and fell on the band resulting in a laceration to his left arm. Injury required 6 stitches.
