Preshifter found rock fall in belt neutral at break 21. Fall measured approximately 120' long x 20' wide x 3'-6' high, it extended from mid point of #3 intersection to the right side of the crosscut between #1 and #2 entries.

A rock fall was found by mine examiner at break 40, 1st North Mains. The fall measured approximately 30' x 20' x 5' thick.

Hoist went down while making check, took coupling off and put back together, couldn't find anything wrong and it started working ok. Made hoist check again and everything was working fine.

A rock fall occurred on 002 section main intake in 2 West Mains in the #6 entry at break 32. The rock fall measured approximately 40' wide x 20' long x 4' high. Rock fall was found while conducting weekly exam.

The hoist engineer was making checks and failed to set the rollback back to the proper setting, causing the brakes to lock.

Rock fall occurred between break #15-#16 in the 1st North Mains in the #5 entry. Fall measured approx. 30' long x 3.5' thick.

A roof fall occurred in the #1 East Mains, break 53 in the return entries of #1 & #2. The fall measured approximately 5' thick x 20' long x 80' wide. This area is bolted with 5' glue bolts and has cribs set between 2 & 3 entries.

Hoist went down, hub and splined shaft on main hydraulic pump stripped. New hub, shaft and seal was replaced and hoist was placed back into operation at approx. 4:30pm.

Struck by falling object

Employee was walking up to a stopping that had just been knocked down by a scoop when a piece of rock fell from between bolts and struck him on the back.

Employee was installing a strap when his canopy hit the top causing a rock to fall, rock struck his index and middle fingers on his right hand. Index finger had to be amputated.

Employee was lifting lolo up with a slate bar to put lolo pin in, he later said that he felt pain in his back. Employee started missing 11-03-06.

Employee was sawing timbers when a piece of draw rock fell out and struck him on the back.

Employee was getting ready to pull bolter to next roll when a piece of drawrock fell striking him on the right shoulder/upper back. Employee started missing 01-09-07.

Employee was bolting in #2 right break when he bumped his knee on the ground. Employee started missing 10-06-06.

Over-exertion in wielding or throwing objects

Employee turned to throw his jumper cable when he twisted his left knee.

Employee was riding mantrip buggy from the face to the track when a piece of belt came in contact with his left knee/leg. Injury resulted in a fracture.

Over-exertion in lifting objects

Employee was helping to construct an overcast when he strained his lower back. He started missing 11-07-06.

Fall from machine

Employee was crossing lo lo and fell to the bottom, resulting in a fracture to his left ankle. Employee started missing 09-01-06.

Over-exertion in pulling or pushing objects

Employee was pulling on cables and strained lower stomach. Injury resulted in a hernia that required surgery. Employee started missing 09-20-06.

Fall from ladders

Employee was cleaning buss bars on hoist, ladder slipped causing him to fall approx. 15'. Injury resulted in a fractured left foot/ankle.

Fall to the walkway or working surface

Employee was assisting in installing beams, took off running, tripped and fell injuring his back. Employee started missing 02-06-06.

Employee was helping install a new cable on the 90 ton Linkbelt crane when the cable puller came loose, allowing the cable to fall, striking him on the right shoulder.

Employee was throwing a rock into mucker bucket and caught right ring finger in between rock and mucker bucket.

Struck by flying object

Employee was removing the segments for the dozer using a 10 lb sledge hammer. The hammer hit the segment and a piece of metal broke off and became embedded in his lower left leg.

Employee was hooking cable to crane hook and caught left ring finger between hook and cable.

Employee was helping drive pin in miner, hammer deflected off steel and hit on hard hat cutting his left ear. Injury resulted in 9 stitches.

Employee was attempting to put bolt in hole and plate cut his left wrist. Injury required 5 stitches.

Employee was cutting a rock dust bag open and inadvertently cut his left thumb. Injury required 8 stitches.

Caught in, under or between a moving and a stationary object

Employee was standing beside back bridge when bridge began to move, it slid a little sideways pinning his right foot under it.

Employee was attempting to remove a lolo tailpiece bearing with a sledge hammer and bar and cut his left index finger. (Required 16 stitches)

Employee was busting rock in conveyor of carrier when he started to remove the rock, the rock flipped over catching his left hand ring finger between the rock and metal. Accident resulted in a fracture.

Employee was pulling a loose rib when rock fell hitting belt rail causing corner of rail to fly up and hit him on the leg. Required 7 stitches.

Employee was tightening lug nuts on scoop with a cheater bar and ratchet, the ratchet slipped and caught his right index finger, requiring 8 stitches.

Employee was bolting when a piece of draw rock fell, striking employee's forearm resulting in stitches.

Employee was cleaning out the synfuel collector sampler and when he closed it the latch handle snapped down mashing his finger. Injury required stitches.
