A rock fall was discovered on 2 East Mains in the #2 entry at break #3. Fall measured approximately 19' wide x 30' long x 6' thick.

A rock fall was discovered on 1 left off 1 South East Mains in the #7 entry crosscut 28. Fall measured approximately 20' wide x 20' long x 6' thick.

A rock fall was found between overcast on 2 West Mains in the #1 entry at mouth of the 2 left submains. Fall measured approximately 20' wide x 30' long x 5' thick.

A rock fall was found at break #31-#32 on 5 belt on the 1 North Mains. Fall measured approximately 19' wide x 40' long x 3'-5' thick.

A rock fall was found one break out-by 2 West seals on 1 North Mains. Fall measured approximately 20' wide x 40' long x 4'+ thick.

Elevator was down due to preventative maintenance.

Slope car was down due to electrical problems.

A rock fall was discovered at 9:21pm on the #1 Left Mains in the #4 entry at cross cut #7. The fall measured approximately 20' wide x 20' long x 7'-10' thick.

Rock fall was found at 8:05am. Fall measured approximately 20'-24' wide x 9 breaks long x 6'-8' thick.

Rock fall was discovered in 1 West Mains at break #51 between the #3 and #4 entry. Fall measured approximately 20' wide x 30' long x 5'6' thick.

Found by weekly examiner and MSHA Inspector.

Elevator was down due to a relay not working.

Rock Fall occurred on 1 Southeast Mains in #2 entry. Rock Fall measured approximately 45-60 foot long X 20 foot wide X 6-10 foot thick.

Rock Fall occurred on 2 Left off of 1 Left Mains between Break 14-15 in # 6 entry. Rock Fall measured approximately 60' long X 20' wide X 7' thick.

Rock Fall occurred on 2 Left off 1 South East Mains # 3 entry BRK # 32. Measured approximately 20' wide X 30' long X 8' thick.

Elevator was down due to high water in the pit.

A rock fall was discovered in 3 Left off 3 Left bleeder. Fall measured approximately 20' wide x 60' long x 4' thick.

A piece of key stock had been welded on the end of bend pulley shaft of the 602 clean coal overland belt. Approximately an hour later the fire was discovered.

A rock fall occurred on the #1 South East Mains in the #7 entry at crosscut #27. The fall measured approximately 20' wide x 80' long x 8'-10' thick.

Employee was traveling 1 south east main section and found rock fall at Break 34-35. Fall is approx. 175ft long, 20ft wide and 6-10ft thick.

Rock fall occurred on the 002 section. Fall measured approximately 19' wide x 200' long x 8' thick.

Elevator was down due to door switch malfunction.

Employee was pulling draw rock when a piece struck his hand.

Employee was walking by the Power Center when a piece of draw rock fell striking his right leg.

Employee was changing a tailpiece bearing, he turned to pick up a tool when he felt pain in his back.

Employee was hooking rope on lolo dolly when a piece of draw rock fell and hit him on the back of his leg. Employee started missing 10-17-11.

Employee was throwing belt structure across the belt while the belt was off. The belt rail hit side of back bridge, bounced back and hit employee on the right side of jaw.

Employee was attempting to cross a stopped belt when he fell injuring his right shoulder.

Employee was installing a roof bolt when the bolt kicked out and struck his arm.

Employee laid his hand on the miner cable stand off when a piece of rock fell from between the rib and first row of bolts striking his right hand, pinky finger.

Employee was picking up a timber when he felt pain in his mid-section/groin area.

Employee was operating scoop when a piece of draw rock fell out between bolts and hit him on the right shoulder.

EE was bolting roof when a draw rock fell and hit him on the right shoulder. Bruising it he worked until 6/21/11 was off 2 days came back on 6/24/11.

A rock fell striking employee on the head. Employee started missing 06-16-11.

Employee was attempting to pull lolo with the miner when the clevis broke and struck him on the left side of his head. Employee started missing 06-09-11.

Employee was getting off of super steer when he slipped and fell causing him to twist his left knee.

Employee was helping miner men to change places when a piece of rock fell and struck him on the arm and hand.

Employee was crawling down the belt on lolo and his foot must have slipped off the side. They were in the process of setting bits in the miner and when they backed the miner up to roll the heads it hit the Mobile Bridge Carrier (MBC) causing it to twist and rubbed employees foot.

Employee was struck by a moving object.

Employee was walking to get supplies when he stepped in a hole and strained his right knee. Employee started missing 03-02-11.

Employee was exiting a scoop when he hit his elbow on the canopy post. Employee had surgery 01-12-12.

Employee was making a belt set up and when he went to throw a roller under the bottom belt his finger got pinched against the rib.

Employee was cutting belt and in the process cut his leg.

Employee was getting up from sitting and writing down some notes when he started to fall he put his hand down to catch himself and struck an oil can.

Employee was removing a rock from the boom of the miner when the rock fell and caught his finger in between the rock and a miner bit.

Employee was clearing fallen draw rock off of the top of the continuous miner when he hit his hand on a piece of the rock.

Employee was hitting an ease out with a hammer. He was holding ease out with his left hand when ease out broke. His hand slipped on ease out cutting his little finger on his left hand.

Under investigation

Employee was struck by a metal chip from impact of a hammer against a chain hook.

Employee was attempting to lift the left boom hinge guard on a new continuous miner with a bar. His hand slipped causing the guard to fall and strike his left hand.

Employee was pre shifting when he turned around he hit his ear on a strap that was hanging down.

Employee inadvertently hit a paint can with his drill steel causing it to burst and cut his arm.

Employee was riding in the mantrip on the inby end facing in on the rib side, when the mantrip struck something, causing it to fly up and strike the employee on the left forearm.

Employee was splicing 3B belt with a utility knife. The knife slipped out of the conveyor belt then the knife struck him in the right leg. The injury required five (5) stitches.

Employee was shoveling tailpiece when his shovel hit belt and bounced back and hit him in the mouth.

Employee laid a bow saw down after using it and when he picked it back up, he grabbed it the wrong way and cut his hand.
