Used metal plate as a spacer and it broke free and hit the employees hand causing a laceration.

Unclassified, insufficient data

Employee reported an injury of low back pain, a medical physican determined this to be non-work related. On 7/2/13 Utah Labor Commission determined a portion of injury was work related, a motion for dismissal was filed with the court of appeals. On 12/12/13 a binding agreement between the 2 parties was signed, that removes liability from the organization.

Struck against stationary object

While changing screens employee bumped elbow on equipment. Employee did not have any medical symptoms following incident. Employee later had symptoms evolve and sought medical care without notifying employer. Injury did not require medical treatment, until later visit when employee had fluids drained and received Steroid Rx treatment.

Over-exertion in lifting objects

EE had been experiencing pain in shoulder after shoveling and lifting.
