Struck against stationary object

Made contact with the screen box, with right hand, ring finger and dislocated/ fractures the tip of the finger

Employee received a laceration on left brow, receiving 6 stitches

Contact with heat

While working in 105 degree temps, EE over heated and was taken to the hospital where EE was diagnosed with heat exhaustion. Fluids were administered.

Over-exertion (Not Elsewhere Classified)

Belly belt stopped on screen had to shovel it out not a lot of room and lay on side and pull material out. After EE came to me and told me that arm was sore. When the soreness didn't go away we decided we should report it, which occurred on 7/6/20. At that point it was a first aid and didn't become recordable until 7/24/20.
