EE was walking oxygen bottle off rack toward him and exerted more force with his right arm.
An employee was removing a motor from a sand stacker and needed assistance. The employee called another over to help him. The employee lost his hand grip on the motor and injured tried to catch the motor (appx 200 lbs)& pulled it toward his body with his knees bent. The motor hit into his leg just above the knee causing swelling and a contusion.
Employee was assisting in the repair & replacement of the hub & flights on the rock scrubber at the wet plant. Equipment was locked out & tagged out. Two employees were lifting the hub into the rock scrubber with a loader & were removing the hub into the scrubber. Hub was heavier than expected & his finger was pinched between the hub & frame of scrubber causing a laceration.
EE was grinding channel iron and the wheel caught edge of gap, causing grinder to kick back.
Climbing into loader by pulling himself with his arms. Injury date 4/24/08, seen by Dr. 5/7/08, went recordable 6/5/08 with an injection of cortizone. No time off from work or work restrictions.
Employee was cleaning silo and fell from height of about 8-10' onto a 5' pile of sand.
