Struck by falling object

Employee was removing skid steer bucket and buckets pins would not release. He and another employee curled the bucket to access the bucket pins to tap them in all the way. While working on pins, the bucket became unhooked and fell two feet onto employee's foot above his steel toe.

Fall to the walkway or working surface

Employee initially had a first aid injury in Dec. 2015. On Feb 15, 2016 his injury became recordable due to surgery on his shoulder for a slip and fall on ice from his Dec 2015 injury.

Employee was complaining of chest pain, an ambulance was called. EKG normal & blood pressure high so employee was taken to the hospital.
