Struck by... (Not Elsewhere Classified)

Helping a vendor un-strap the truck. Loosening up the binder, when it came up and struck the EE in the face.

Struck by flying object

Changing jaw belly belt and cable caught troughing roller frame and broke hook that was attached to the belting. Cable broke loose and struck leg.

Fall to the walkway or working surface

Employee was postioning the screen and was using the handrail as support when the handrail gave way.

EE woke with severe chest pain at approx. 3:30 a.m. and believed it to be really bad heart burn, he got ready for work and then drove to the office. Upon arrival, he started experiencing more of the pain with some tingling in leg and arm. When another EE arrived he asked her to call 911. EE was coherent and communicating with EMT's at departure from the mine.
