Struck against a moving object

Employee Rolled a Skid-steer, was released to full duty. Continued having pain, became recordable from MRI results on October 26, 2018 showed Tear in L. shoulder tendon.

Struck against stationary object

Employee was adjusting the jaw wedge plates and couldn't get the nut loose with cordless impact. Employee decided to use an open ended wrench and got the nut to come loose, as it broke free elbow struck the corner of the jaw and cut elbow open. Employee received stitches and a prescription for an antibiotic.

Fall from machine

Entering loader, reached for the handrail on the door while stepping to top stair. Missed door handrail & fell backwards. Caught self with hand. This action caused a heavy load on EE's right shoulder. MRI shows a Bicep tear. Delay in reporting as Insurance has been debating if the tears are Chronic or Acute.

Struck against stationary object

Hit head on a water pipe and received a laceration that required staples.

Employee passed out, Ambulance came and could not find anything wrong. We drove the employee to the hospital, checked out and determined it was because lack of nutrition. This was reported to MSHA hotline but turned out to be a personal health issue and non work related.
