At 19:20 the IP's heavy equipment caught fire. After EE's fire suppressant device failed EE opened the door to EE's vehicle, letting in the smoke before closing the door. After realizing EE should not stay in the vehicle any longer the IP evacuated the vehicle and went into EE's supervisors car awaiting medical aid.

Haul Truck 629 was loaded at a shovel and traveling uphill to a dump when smoke and flames were observed in the engine bay. The haul truck came to a stop the operator descended the haul truck.

While conducting routine inspections, EO detected a smoldering inside of the coal building as they proceeded to investigate. EO made their way from the front of the coal building to the back, during this process EO sank into a hot ash pile up to their waist area, resulting in burns to their lower extremities & lower arm area.

Removing a seized bolt from the Terex screen plant. Grinder was the best option to remove. While cutting the bolt, EE was grinding when sparks attached to EE's clothing under EE's safety vest and caught EE's t-shirt, flannel shirt and safety vest on fire.

There was a fire in the GAR facility. The employee was fighting the fire and suffered smoke inhalation. The employee went to the hospital and received Vitamin B IV. They were just getting over Pneumonia. The employee was released from the hospital after 2 hours.

Miner was performing rounds inspecting conveyors in the Mill and noticed smoke coming from one of the pullies. Miner pulled the stop cable and pulled a fire ALARM pull station to activate emergency response. Miner was wearing PAPR at the time and inhaled a small amount of smoke. Miner evacuated the building and transported to the Mine clinic for evaluation.

On 11/13/24 Maint Tech was cleaning parts on bench and noticed a rag smoldering under loader. Tech reached down to pat out rag and their gloves caught fire. Tech was wearing mechanics gloves with latex gloves underneath. Tech quickly removed gloves but received non-life threating burns to both hands. Was transported to burn unit and will miss work to heal.

A customer truck pulled into the plant. While waiting on instruction to be loaded a fire started under the cab of the truck. An employee used a fire extinguisher to try and put out the incipient fire. The wind blew smoke & dry fire extinguisher powder toward the employee which the employee inhaled. A local fire department put out the fire which burned for approximately 40 minutes.

The EE was operating the thermal dryer from 12 AM to 12 PM WW shift. The EE lost the large fan in the drying chamber and went to turn on the sprays. As the EE entered the Roll Feed Room, flames from an access door contacted EE. EE sustained burns on 50% of their body.

Hydraulic hose burst as bed was being raised on Terex TR70 Rock Truck. Hydraulic oil immediately burst into flames and engulfed the truck in flames. Driver had to jump from vehicle and received minor burns to arm and neck when exiting truck.

Either a hose or cooler burst (unknown at this time) spraying the turbo with compressor oil leading to a fire on the rig (extinguished in under 30 minutes). Driller leapt from the rig breaking their left heel, spraining right ankle, and second degree burns on left forearm and right hand (grabbed the handrail to leap over). Accident is investigation underway this week.

Employee was trying to thaw out the tail pulley at the wash tower of C207. Employee poured brake-line antifreeze on the belt at the tail pulley of C207 and lit it on fire. Employee then poured more brake-line antifreeze on the fire which caused it to blow up catching EE on fire and burning EE's lower legs.

Employee was cutting a dragline drag cable for removal and replacement using oxygen/acetylene torch. Oxygen supply line ruptured at connector on the base of the torch. Flash fire occurred and burnt employee on the right abdomen. Diagnosed as 2nd degree burn.

The employee had previously used brake cleaner to clean the oil and grease from the area. After failing to remove the washer, the employee attempted to use a min-torch to heat the washer. When the employee ignited the mini-torch, the residual brake cleaner vapors ignited and cause a small "fire ball" that burned two fingers of the left hand.

Miner was operating LR1033 in New Fold escape drift when turbo caught on fire. Miner deployed fire suppression system, it did not put the fire out. Suppression system possibly malfunctioned or did not operate due to wet streams being too concentrated. Miner then used 10lb extinguisher to suppress flames. Miner was taken to local hospital for evaluation and released the same day.

While washing out the basement of the prep plant sizer building with a water hose, employee was standing in front of a propane heater. EE didn't realize EE was as close as EE was to the flame. The back of EE's shirt ignited. EE ran outside and laid in a puddle to extinguish the flames.

Dozer started leaking hydraulic fluid. Employee pulled the dozer to a safe location, Witness #1 noticed flames on the right side engine compartment. Witness #1 called the "Mayday" and instructed Operator to activate the fire suppression on the dozer. NOTE: Incident occurred 12/7. Injury reported 12/16.

EE was plasma cutting the top of an empty 55 gallon isopropyl alcohol when the fumes combusted catching EE's shirt on fire giving EE a first degree burn on left arm and chest. The drum had been washed prior to the cutting but was not washed out well enough and enough time for the fumes to evacuate the drum.

While fighting a fire in the clinker barn, a Fire Fighter sustained 1st and 2nd degree burns while fighting a conveyor belt fire in the clinker barn. The fire fighter was treated for the injury and taken off work for 2 weeks.

Tech was using a torch to cut out a tank on a 993K loader. Sparks and spurs came off as tech was cutting landing on pant right pant leg. Tech didn't realize pant leg was on fire until it had burnt tech. Received minor burns on finger tips as tech was putting out the fire on pant leg. Tech had taken off gloves to pat out fire.

Employee arrived at the quarry to discharge the used oil into the brush pile, thinking the oil would have cut the gasoline they added to the tank the day before. When they did this they got the liquid on their shirt and a flash fire occurred burning their right lateral abdomen.

EE was using a torch to cut an opening in steel above EE. Either the flame from the torch or a piece of slag created an opening in the hydraulic line, which began spraying oil and caught fire. When EE turned away the flame/fire entered EE's welding shield. EE then jumped down from where they were working and caused pain in the right foot after landing.
