Over-exertion in lifting objects

EE and another worker were lifting a belt for a conveyor repair. After completion of work, EE sat in EE's office and felt discomfort in EE's back. On following days EE stated condition was improving. On 3-6-25 EE stated that on 3-3-25 EE bent over in the shop and felt strong pain. On 3-6-25 EE's back was hurting severely.

EE and a co-worker were unloading a roll around new in box tool box out the back of pickup truck. Pulling and lifting the box EE had pain in the right shoulder.

Maintenance worker was bent over cleaning a shaft using a 4 1/2""grinder, when EE stood up EE twisted to put down the grinder and pulled EE's back muscle.

EE was plasma cutting the top of an empty 55 gallon isopropyl alcohol when the fumes combusted catching EE's shirt on fire giving EE a first degree burn on left arm and chest. The drum had been washed prior to the cutting but was not washed out well enough and enough time for the fumes to evacuate the drum.

production employee was stacking 50 lb. bags of material and while positioning bag on pallet EE's right shoulder popped.

Hammermill operator was putting the cover of the hammermill pushing it back in place when EE heard a pop in EE's back. After putting the cover on EE was going to climb in the frontend loader and was unable.

EE was cutting a bucket elevator belt with a utility knife. EE had placed the belt in a vise to cut the belt, EE proceeded to cut the belt gabbing the top of the belt with EE's left hand and started cutting the belt when the knife went through the belt cutting EE's finger.

Two maintenance workers were moving I-Beams when one worker pulled on one end pinning the other worker's hand between the two I-Beams.

Struck by flying object

While driving a forklift he ran over a ceramic media ball projecting it striking the wall it bounced off the wall striking him under the right eye on the cheekbone. Which caused an Right Facial Contusion.

Struck against stationary object

on Jan 22, 2014 ee was shutting a pump down for the day when he slipped on wet material striking his left wrist on the pump. He reported the accident on Jan 23, 2014 and was taken to Mercy Health Clinic and was treated for a contusion to his left wrist.

Worker, EE was bolting a pipe back together when he bumped his knee on a flange. Sent him to doctor, doctor found a bruise on his knee cap. No lost time at work.

Employee was removing Whizzer Cones from equipment when the cone became jammed. The employee pulled the cone when it gave way and struck his left upper wrist and cutting it open.

Employee attempted to move a 2,500 lb pallet of Novacite with his arms down a roller caster table. In his attempt the employee heard a pop at his right elbow and pain.

Employee, being trained on Bag Packaging System, noticed the material feed slowing down and estimated that the pinch valve, that allows the feed, was malfunctioning. He decided to try to correct it himself and place the Index and Middle Finger of his left hand in the pinch valve, pressure was released from the valve and closed on the his fingers pinching them.

Employee was opening safety gate to the palletizer to stock with pallets when gate struck him in the left eye causing a minor cut to the left eyelid.

EE WAS PATROLLING THE PLANT WHEN HE HAD A SEIZURE AND FELL DOWN AND STRUCK HIS FACE. HE HAD NOT BEEN TAKING HIS MEDICATION, AS STATED BY EE AND HIS WIFE.

EE WAS TRYING TO FREE A PIECE OF A BAG THAT WAS CAUGHT IN A SCREW CONVEYOR UNDER THE PALLETIZER. THE SCREW LACERATED THE SIDE OF HIS RIGHT INDEX FINGER.

Inhalation of radiations, caustics, toxic and noxious substances

RECEIVED A LETTER FROM DR. STATING THAT EE HAS SILICOSIS.
