Fall onto or against objects

Employee was climbing loader steps to enter cab when EE cut EE's leg. The cut became infected and caused a staph infection.

Employee was draining water out of outboard motor, when EE started the engine, a spark ignited gasoline fumes from spilt gasoline and caught EE's left forearm on fire, causing a burn.

Our employee was repairing a 3/4"" cable on a winch when the cable struck them in the mouth, causing injury to teeth and a laceration to top lip.

Finished repairs to single screw, turned to take a step & stepped into a cleanout hole, bucket he was carrying blocked his view of hole. His body went forward and his right leg got caught in the hole. Was hanging upside down. Co-worker helped him back upright. His leg was removed from hole & made it down the rest of the pipe. Hole covered.

Was on the bow of the boat trying to catch cable to place it on the boat to make a connection. Suddenly, Other ee put the boat into reverse, causing EE to go overboard, hitting the cable with his right arm and right shoulder. Office was not made aware of the accident by Foreman until 12/17/07, and 7000-1 not complete until today.

EMPLOYEE WAS ON THE DREDGE TIGHTENING A BOLT ON THE STONE BOX USING A CLOSED-END BOX WRENCH AND A CHEATER PIPE. HE STATES HE TIGHTENED A BOLT ONE STROKE AND THEN WHEN HE WENT TO TIGHTEN IT AGAIN THE PIPE SLIPPED AND HE FELL BACKWARDS, HITTING A PIECE OF 6"" CHANNEL IRON WITH HIS LOWER BACK AND RIGHT HIP.

EE WAS GETTING READY TO TAKE BOAT OUT TO THE DREDGE. EE LIFTED AND PUSHED BOAT OFF THE BANK WHEN HE FELT A PULL IN HIS BACK.

THREE EMPLOYEES WERE MOVING PARKING LOGS AROUND THE YARD. THEY HAD NEW CHAIN (15 FT) TIED TO THE BUCKET. THE CHAIN LOST ITS SLACK ON ONE SIDE OFTHE BUCKET, SO EMPLOYEE BENT DOWN TO GET THE RIG HT AMOUNT OF SLACK IN THE CHAIN. WHEN HE STOOD UP IS WHEN HE FELT A POP IN HIS BACK.
